HISTORY
This case involved three shifts of nurses over a 24-hour period working in a long-term care (LTC) facility. The facility included three separate units with 40 beds per unit. A licensed practical nurse charge nurse was assigned to each unit. One registered nurse supervisor was responsible per shift for the oversight of all three units.
Ms. Kathy Chin had been admitted to the LTC facility in October with heart failure, bipolar disorder with depression, constipation, history of gastrointestinal bleed, glaucoma, and discomfort. She required total care and was not able to effectively express her needs to the LTC staff. In January she had a bowel impaction, and Fleet enemas were administered. She was discovered unresponsive in her room the next afternoon and was transported to the hospital. Blood work revealed that she had a urinary tract infection. She was administered antibiotics and discharged back to the LTC facility.
Six weeks later, Ms. Chin had another episode of constipation. The day shift Nurse Supervisor, Ms. Angela Guilarte, notified Ms. Chin's physician, Dr. Brian Fisher. Dr. Fisher ordered an x-ray of Ms. Chin's abdomen, and it revealed severe constipation with fecal impaction. Dr. Fisher ordered clear liquids only and “Fleet enemas until clear.” This was the same regime that was previously ordered and administered to Ms. Chin in January.
A licensed practical nurse, Ms. Margaret Reyes, took the order by telephone, notified day shift Nurse Supervisor Guilarte, documented the order in Ms. Chin's chart, as originally received from Dr. Fisher, and on the medication administration record [MAR] “enemas continuously until clear.” Practical Nurse Reyes, who received the order for “Fleet enemas until clear,” questioned the order and discussed it with Nurse Supervisor Guilarte. They determined that the enemas should be administered every 3 hours until the return was clear but did not clarify this with Dr. Fisher or pass this information on to the next shift.
The enemas were administered over a 12-hour period by three licensed practical nurses working various shifts. Again Ms. Chin was found unresponsive, and she was transferred to the hospital where she died 6 hours after admission.

Mr. Jones provided much of the care for his wife during her illness. Although her care was time-consuming and fatiguing and kept him at home much of the time, he was grateful that he could care for her. He now is alone in their home, is very lonely and uninterested in preparing meals or eating, and lacks energy to return to his former community and social activities or even to interact with his son and family.
The hospice nurse contacted Mr. Jones for follow-up bereavement counseling. She told him that although he had “passed” a routine physical examination the week before, she was concerned about his continuing sadness and lack of energy. The nurse reassured him that it was not uncommon to grieve for many months after a major loss. She asked him if he thought his wife would have had a similar experience if he had been the first to die. His response was that his wife would have had an even more difficult time adjusting. The nurse and Mr. Jones then spent some time reflecting on his loss and feelings, and talking about his response. The nurse’s initial question and Mr. Jones’s resulting insight that his grief was not as bad as his wife’s would have been helped him transcend his immediate experience of loss and find some meaning in his grief.
This illustration is an example of an inward expansion of self-boundaries indicative of self-transcendence. Other expressions of self-transcendence might help Mr. Jones facilitate his own healing and regain a measure of well-being.
In terms of outward expansion, Mr. Jones, with some encouragement, might reach out to his son’s family to begin to reconnect to the world outside himself. Walking to and from his home to theirs could expand his sensory world and provide opportunities to interact with other people and with nature along the way. Spending time with his grandchildren could be enlivening through the joy young children can bring to an older person, as could a sense of satisfaction derived from being helpful to his son and daughter-in-law.

When I was working in SICU (Surgical Intensive Care Unit), I was assigned to a patient who was admitted to ICU after an episode of bradycardia and loss of consciousness on the medical surgical floor. A rapid response was initiated and she was intubated and brought to our unit. She was Haitian and came from a very different ethnic background. At the time I was not familiar with their culture or beliefs nor did I know her health history and what originally brought her to the hospital or the reason for her admission.
After a couple days of being assigned to her and taking care of her and her family, I began to understand a little more. She had been diagnosed with breast cancer the previous year and had undergone chemotherapy which unfortunately was unsuccessful. The cancer had spread throughout her body including her esophagus which was later found out to be the reason for the intubation. A tumor had attached itself to her airway and she was unable to breath on her own.
I instantly became attached to the patient and family and enjoyed learning about their culture and belief system. Many people including doctors, nurses and other medical professionals couldn’t understand why a patient who was terminal would want everything done and continue aggressive treatment. Many gave up on her and the family which is why I believe they requested that I take care of her every shift I worked. Special arrangements were made between the family and my director to accommodate their wishes.
Each shift I worked, I took care of her. Her daughter and her husband never left her side other than for bathing or other emergent matters that couldn’t be handled without them. Eventually the primary physicians would stop rounding on her daily and healthcare professional started discussing her prognosis, treatment and plan of care. No one understood why we were continuing to treat this individual as they called the treatment "futile".
As time went on, the hospital decided to call in Risk Management as they had a different belief and understanding regarding by patient's wishes and medical management. This is when I realized I did, I understood her and her family and knew I needed to be her patient advocate and her voice. I fought for her treatment and plan of care which was aggressive not palliative. She expressed to me she wanted to live, even if it meant her last days were spent in the hospital. “Mrs. Walsh, a woman in her 70s, was in critical condition after repeat coronary artery bypass graft (CABG) surgery. Her family lived nearby when Mrs. Walsh had her first CABG surgery. They had moved out of town but returned to our institution, where the first surgery had been performed successfully. Mrs. Walsh remained critically ill and unstable for several weeks before her death. Her family was very anxious because of Mrs. Walsh’s unstable and deteriorating condition, and a family member was always with her 24 hours a day for the first few weeks.
The nurse became involved with this family while Mrs. Walsh was still in surgery, because family members were very anxious that the procedure was taking longer than it had the first time and made repeated calls to the critical care unit to ask about the patient. The nurse met with the family and offered to go into the operating room to talk with the cardiac surgeon to better inform the family of their mother’s status.
One of the helpful things the nurse did to assist this family was to establish a consistent group of nurses to work with Mrs. Walsh, so that family members could establish trust and feel more confident about the care their mother was receiving. This eventually enabled family members to leave the hospital for intervals to get some rest. The nurse related that this was a family whose members were affluent, educated, and well informed, and that they came in prepared with lists of questions. A consistent group of nurses who were familiar with Mrs. Walsh’s particular situation helped both family members and nurses to be more satisfied and less anxious. The family developed a close relationship with the three nurses who consistently cared for Mrs. Walsh and shared with them details about Mrs. Walsh and her life.
The nurse related that there was a tradition in this particular critical care unit not to involve family members in care. She broke that tradition when she responded to the son’s and the daughter’s helpless feelings by teaching them some simple things that they could do for their mother. They learned to give some basic care, such as bathing her. The nurse acknowledged that involving family members in direct patient care with a critically ill patient is complex and requires knowledge and sensitivity. She believes that a developmental process is involved when nurses learn to work with families.
She noted that after a nurse has lots of experience and feels very comfortable with highly technical skills, it becomes okay for family members to be in the room when care is provided. She pointed out that direct observation by anxious family members can be disconcerting to those who are insecure with their skills when family members ask things like, “Why are you doing this? THE IMPORTANCE xOF CROSS-MONITORING TEAM MEMBERS' PERFORMANCES
HISTORY
This case involved three shifts of nurses over a 24-hour period working in a long-term care (LTC) facility. The facility included three separate units with 40 beds per unit. A licensed practical nurse charge nurse was assigned to each unit. One registered nurse supervisor was responsible per shift for the oversight of all three units.
Ms. Kathy Chin had been admitted to the LTC facility in October with heart failure, bipolar disorder with depression, constipation, history of gastrointestinal bleed, glaucoma, and discomfort. She required total care and was not able to effectively express her needs to the LTC staff. In January she had a bowel impaction, and Fleet enemas were administered. She was discovered unresponsive in her room the next afternoon and was transported to the hospital. Blood work revealed that she had a urinary tract infection. She was administered antibiotics and discharged back to the LTC facility.
Six weeks later, Ms. Chin had another episode of constipation. The day shift Nurse Supervisor, Ms. Angela Guilarte, notified Ms. Chin's physician, Dr. Brian Fisher. Dr. Fisher ordered an x-ray of Ms. Chin's abdomen, and it revealed severe constipation with fecal impaction. Dr. Fisher ordered clear liquids only and “Fleet enemas until clear.” This was the same regime that was previously ordered and administered to Ms. Chin in January.
A licensed practical nurse, Ms. Margaret Reyes, took the order by telephone, notified day shift Nurse Supervisor Guilarte, documented the order in Ms. Chin's chart, as originally received from Dr. Fisher, and on the medication administration record [MAR] “enemas continuously until clear.” Practical Nurse Reyes, who received the order for “Fleet enemas until clear,” questioned the order and discussed it with Nurse Supervisor Guilarte. They determined that the enemas should be administered every 3 hours until the return was clear but did not clarify this with Dr. Fisher or pass this information on to the next shift.
The enemas were administered over a 12-hour period by three licensed practical nurses working various shifts. Again Ms. Chin was found unresponsive, and she was transferred to the hospital where she died 6 hours after admission.
THE NURSE'S STORY
I came to work early that evening and was told that the licensed practical charge nurse for one of the units had called in sick. I realized that I would have to cover the patients for the licensed practical nurse as well as provide supervision for the other two units. I received a short verbal report from Ms. Zellner, the evening shift registered nurse supervisor, who reported Ms. Chin's bowel impaction and the order to administer Fleet enemas until clear. Mr. Jones is a 65-year-old man whose wife died 6 months ago after a long illness. The couple had been married 45 years, and they were devoted to each other. They had three children who are now in their 30s. Two of the children live several hundred miles away, but one son lives with his wife and two preschool children less than 1 mile from Mr. Jones’s home.
Mr. Jones provided much of the care for his wife during her illness. Although her care was time-consuming and fatiguing and kept him at home much of the time, he was grateful that he could care for her. He now is alone in their home, is very lonely and uninterested in preparing meals or eating, and lacks energy to return to his former community and social activities or even to interact with his son and family.
The hospice nurse contacted Mr. Jones for follow-up bereavement counseling. She told him that although he had “passed” a routine physical examination the week before, she was concerned about his continuing sadness and lack of energy. The nurse reassured him that it was not uncommon to grieve for many months after a major loss. She asked him if he thought his wife would have had a similar experience if he had been the first to die. His response was that his wife would have had an even more difficult time adjusting. The nurse and Mr. Jones then spent some time reflecting on his loss and feelings, and talking about his response. The nurse’s initial question and Mr. Jones’s resulting insight that his grief was not as bad as his wife’s would have been helped him transcend his immediate experience of loss and find some meaning in his grief.
This illustration is an example of an inward expansion of self-boundaries indicative of self-transcendence. Other expressions of self-transcendence might help Mr. Jones facilitate his own healing and regain a measure of well-being.
In terms of outward expansion, Mr. Jones, with some encouragement, might reach out to his son’s family to begin to reconnect to the world outside himself. Walking to and from his home to theirs could expand his sensory world and provide opportunities to interact with other people and with nature along the way. Spending time with his grandchildren could be enlivening through the joy young children can bring to an older person, as could a sense of satisfaction derived from being helpful to his son and daughter-in-law.
Offering at a future time to use the skills he learned while caring for his wife through volunteering with hospice would be an example of transcending temporally. Integrating his memories of Mrs. Jones into his current life would be another example of temporal self-transcendence.
Transpersonal self-transcendence is another important experience for Mr. Jones. Although he was unable to attend church services for several years, he had in the past found worshiping with others a source of comfort. His spiritual life might even be expanded to consider new spiritual dimensions such as that found in the p.

When I was working in SICU (Surgical Intensive Care Unit), I was assigned to a patient who was admitted to ICU after an episode of bradycardia and loss of consciousness on the medical surgical floor. A rapid response was initiated and she was intubated and brought to our unit. She was Haitian and came from a very different ethnic background. At the time I was not familiar with their culture or beliefs nor did I know her health history and what originally brought her to the hospital or the reason for her admission.
After a couple days of being assigned to her and taking care of her and her family, I began to understand a little more. She had been diagnosed with breast cancer the previous year and had undergone chemotherapy which unfortunately was unsuccessful. The cancer had spread throughout her body including her esophagus which was later found out to be the reason for the intubation. A tumor had attached itself to her airway and she was unable to breath on her own.
I instantly became attached to the patient and family and enjoyed learning about their culture and belief system. Many people including doctors, nurses and other medical professionals couldn’t understand why a patient who was terminal would want everything done and continue aggressive treatment. Many gave up on her and the family which is why I believe they requested that I take care of her every shift I worked. Special arrangements were made between the family and my director to accommodate their wishes.
Each shift I worked, I took care of her. Her daughter and her husband never left her side other than for bathing or other emergent matters that couldn’t be handled without them. Eventually the primary physicians would stop rounding on her daily and healthcare professional started discussing her prognosis, treatment and plan of care. No one understood why we were continuing to treat this individual as they called the treatment "futile".
As time went on, the hospital decided to call in Risk Management as they had a different belief and understanding regarding by patient's wishes and medical management. This is when I realized I did, I understood her and her family and knew I needed to be her patient advocate and her voice. I fought for her treatment and plan of care which was aggressive not palliative. She expressed to me she wanted to live, even if it meant her last days were spent in the hospital. She would lay there and listen to her children laugh and tell stories about their day and her husband's voice telling her how much he loved her. She was a mother of 10 and had the most beautiful family I had ever met.

Geely is aiming to be the first China carmaker to design and build satellites to support its autonomous driving programme, the latest step by founder Li Shufu in his bid to build an industry leader.
Geely, which owns Swedish brand Volvo Cars, Malaysia’s Proton, and a stake in Mercedes-Benz owner Daimler, will invest Rmb2.27bn ($325m) in a new development centre and factory to manufacture satellites this year, the company said yesterday.
The announcement makes Geely the first known Chinese carmaker with plans to build its own satellites. Mr Li’s move sparked comparisons in China media with Elon Musk, founder of electric carmaker Tesla and private space exploration company SpaceX.
Last month Geely drew comparisons with Volkswagen when Mr Li’s holding group announced plans to merge Geely Automobile and Volvo Cars, moving the company towards becoming the first global Chinese carmaker.
Che Jun, Communist party boss of China’s eastern Zhejiang province, where Geely is based, said that the complex would be built in Taizhou city and that construction had begun.
The centre will design, test and manufacture low-orbit communication satellites, purpose-built to improve geolocation of vehicles and to support their connected functions, Geely said.
Geely has been pouring money into new technologies from self-driving cars to flying taxis, spending Rmb20bn on research and development in the past year.
The investments are part of the group’s spend on global expansion, such as buying a $9bn stake in Daimler.

Nonprofit organizations are increasingly dependent on the involvement of Millennial
constituencies. Three studies investigated their motivations to support charity-linked
events: emotional identification with a cause, self-determination theory (SDT) regula-
tions, and context-related Facebook promotions. This article addresses the recent call to
expand SDT research from a simple analysis of autonomous versus controlled moti-
vation, to studying the effects of all the regulations in the SDT continuum, in particular,
the inclusion of the tripartite dimensions of intrinsic motivation and integrated moti-
vation. Results demonstrated that the greater the emotional identification with the
cause, the stronger was the tendency to support the charity-linked event. Also, the
results in these social media contexts revealed that specific intrinsic dimensions (e.g.,
experience stimulation) are motivators of online and offline support, as is the personal
value nature of integrated regulation. Whereas only autonomous motivational regula-
tions predicted support for the two events organized specifically a for charitable causes,
both autonomous and controlled regulations predicted support of a for-profit event
organized with a charitable cause as an adjunct. These findings can assist practitioners
in designing more effective social media communications in support of charity-linked
events.

Social media is a new domain offering excit-
ing opportunities to investigate research ques-
tions in social psychology (Greitemeyer, 2011;
Kende, Ujhelyi, Joinson, & Greitemeyer, 2015).
Our research examined motivation to support
charity-linked events of nonprofit organizations
that are currently faced with increased compe-
tition for resources and declining government
support (Paulin, Ferguson, Jost, & Fallu, 2014;
Reed, Aquino, & Levy, 2007; White & Peloza,
2009). Presently, they depend on an ageing set
of traditional supporters (Urbain, Gonzalez, &
Le Gall-Ely, 2013). However, their future suc-
cess lies in ensuring the sustainable involve-
ment of the Millennial generation (Fine, 2009),
distinguished from other generations by their
intense exposure at an early age to interactive
technology and social media (Bolton et al.,
2013).

They hadn’t started implementing. She said he spent a good deal of time buy- ing and selling stocks. He seemed to be influenced by the weekly ups and downs of the market. At least temporarily, however, he had raised the quality of the stocks he was buying.

- 1. “Mrs. Walsh, a woman in her 70s, was in critical condition after repeat coronary artery bypass graft (CABG) surgery. Her family lived nearby when Mrs. Walsh had her first CABG surgery. They had moved out of town but returned to our institution, where the first surgery had been performed successfully. Mrs. Walsh remained critically ill and unstable for several weeks before her death. Her family was very anxious because of Mrs. Walsh’s unstable and deteriorating condition, and a family member was always with her 24 hours a day for the first few weeks. The nurse became involved with this family while Mrs. Walsh was still in surgery, because family members were very anxious that the procedure was taking longer than it had the first time and made repeated calls to the critical care unit to ask about the patient. The nurse met with the family and offered to go into the operating room to talk with the cardiac surgeon to better inform the family of their mother’s status. · One of the helpful things the nurse did to assist this family was to establish a consistent group of nurses to work with Mrs. Walsh, so that family members could establish trust and feel more confident about the care their mother was receiving. This eventually enabled family members to leave the hospital for intervals to get some rest. The nurse related that this was a family whose members were affluent, educated, and well informed, and that they came in prepared with lists of questions. A consistent group of nurses who were familiar with Mrs. Walsh’s particular situation helped both family members and nurses to be more satisfied and less anxious. The family developed a close relationship with the three nurses who consistently cared for Mrs. Walsh and shared with them details about Mrs. Walsh and her life.
- 2. · The nurse related that there was a tradition in this particular critical care unit not to involve family members in care. She broke that tradition when she responded to the son’s and the daughter’s helpless feelings by teaching them some simple things that they could do for their mother. They learned to give some basic care, such as bathing her. The nurse acknowledged that involving family members in direct patient care with a critically ill patient is complex and requires knowledge and sensitivity. She believes that a developmental process is involved when nurses learn to work with families. · She noted that after a nurse has lots of experience and feels very comfortable with highly technical skills, it becomes okay for family members to be in the room when care is provided. She pointed out that direct observation by anxious family members can be disconcerting to those who are insecure with their skills when family members ask things like, “Why are you doing this? Nurse ‘So and So’ does it differently.” She commented that nurses learn to be flexible and to reset priorities. They should be able to let some things wait that do not need to be done right away to give the family some time with the patient. One of the things that the nurse did to coordinate care was to meet with the family to see what times worked best for them; then she posted family time on the patient’s activity schedule outside her cubicle to communicate the plan to others involved in Mrs. Walsh’s care. When Mrs. Walsh died, the son and daughter wanted to participate in preparing her body. This had never been done in this unit, but after checking to see that there was no policy forbidding it, the nurse invited them to participate. They turned down the lights, closed the doors, and put music on; the nurse, the patient’s daughter, and the patient’s son all cried together while they prepared Mrs. Walsh to be taken to the morgue. The nurse took care of all intravenous lines and tubes while the
- 3. children bathed her. The nurse provided evidence of how finely tuned her skill of involvement was with this family when she explained that she felt uncomfortable at first because she thought that the son and daughter should be sharing this time alone with their mother. Then she realized that they really wanted her to be there with them. This situation taught her that families of critically ill patients need care as well. The nurse explained that this was a paradigm case that motivated her to move into a CNS role, with expansion of her sphere of influence from her patients during her shift to other shifts, other patients and their families, and other disciplines” (Brykczynski, 1998, pp. 351–359).
