The Wagner classification system grades diabetic foot ulcers from 0 to 5 based on severity: Grade 0 refers to high risk feet without ulcers, while grades 1-2 are superficial or deep ulcers not involving bone or ligaments. Grades 3-4 involve deeper tissues along with cellulitis or abscess formation and grade 5 refers to an extensive gangrenous infection involving the entire foot.
