An Approach to the Diagnostic Study on Annavaha Srotodusti in Urdwaga Amlapit...ijtsrd
Modern era’s changing lifestyle along with changing food culture and depending upon one’s body constitution Amlapitta is one of the most common diseases seen in the society. Most of the gastrointestinal disorder are owing to results from abnormal functioning of Agni, faulty dietary habits like excessive intake of pungent, spicy food, irregular meals pattern and habits like smoking, alcohol, psychological stress. It is very troublesome disease and it can give rise to many serious problems if it is not treated in correct time. Amlapitta is composed of two word Amla and Pitta. Acharya Sushruta has mentioned the Prakrita Rasa of Pitta is Katu and Vikritarasa or Vidagdha Rasa of Pitta is Amla. Hence when the vidagdhata of pitta increases the disease Amlapitta manifest. Amlapitta is one of the Annavahasrotas vyadhi caused by the vitiation of Tridoshas when any of the dosha causing Mandagni leads to Vidagdhajeerna manifesting as Amlapitta. Here, in this study, examination of upper GIT by means of an endoscope for the purpose of diagnosis of Urdwaga Amlapitta is done. Utility of Homoeopathy in Cases of Nephrolithiasis with Counstitutional Approachijtsrd
Nephrolithiasis also known as renal calculi is a frequent urinary tract condition. Most patients with renal calculi experience recurrent nephrolithiasis. Recurrence rates are estimated at about 10 per year. Most of the people seek for the allopathic treatment for acute cases. The treatment course in the case of renal calculi is flush therapy through I.V. fluids , Lithotripsy or surgical aid etc. these methods are generally painful, harmful and sometime risky for the patients compare to the homoeopathic mode of treatment. Homoeopathic system of medicine manages such case quickly, mildly, gently and permanently without consuming much time. So, this study was conducted to confirm the role of homoeopathy in cases of nephrolithiasis with constitutional approach. Effectiveness of Curry Leaf and Mint Decoction on Indigestion among Geriatric...ijtsrd
The prevalence of digestive problems in older people and its link to hospitalization and death in older people living in the community are both poorly understood. The primary sign of ageing is the gradual loss of physiological integrity, which results in dysfunction and increased mortality risk. A Quasi experimental research design and convenience sampling technique was adopted. 60 Geriatric samples with upper gastrointestinal tract problems were selected. Out of which 30 samples were within the experimental group and remaining 30 under control group. The objectives of the study were to assess the level of indigestion among geriatric clients, to assess the effectiveness of curry leaf and mint decoction on indigestion among geriatric clients and to associate the posttest level of indigestion among geriatric clients with selected demographic variables. The study was conducted at Kondancherry village, Thiruvallur district, Tamil Nadu. Pretest was conducted for both the experimental and control group. Demonstrated the control samples with, how to prepare a drink using curry leaves and mint decoction to experimental group and conducted post test for both the experimental group and control group after seven days of interval. The investigator found that the demographic variable marital status 2=8.103, p=0.044 had shown statistically significant association with posttest level of indigestion among geriatrics at p 0.05 level. Helicobacter pylori Frequency in Polycythemia Vera Patients without Dyspeptic...asclepiuspdfs
Introduction: In polycythemia vera (PV) patients, peptic ulcer and gastroduodenal erosions are more common than the general population, but there are insufficient data on the frequency of Helicobacter pylori (HP) and its role in etiopathogenesis. In this study, we aimed to compare the prevalence of HP infection in PV patients without dyspeptic complaints with a healthy control group without dyspeptic complaints. Materials and Methods: Fifty patients with PV without dyspeptic complaints and 50 controls without dyspeptic complaints were enrolled in this study after informed consent obtained. Stool samples of selected patients were analyzed using HP stool antigen test (True Line®). Results: There was surprisingly striking difference between HP prevalence in PV patients without dyspeptic complaints and asymptomatic healthy controls (64% vs. 2%) (P < 0.05). There was no significant relationship found between HP presence and age, gender, treatment modalities, complete blood count, positivity of JAK2 V617F, serum erythropoietin level, and splenomegaly in PV patients (P > 0.05). Conclusion: As the susceptibility of HP infections in PV patients are higher, it is recommended to have close surveillance of these patients by screening HP presence. In addition, when HP positivity is determined, the eradication of HP is essential to prevent possible future gastrointestinal lesions in patients with PV.

Abstract—Hydatid cyst disease is a zoonotic disease caused by the larval stage of Echinococcus granulosus and Echinococcus Multilocularis. In human beings, apart from involving the liver and lungs commonly, it also affects other organs like brain, kidney and spleen. Rupture of Hydatid cyst into abdominal cavity causes disseminated abdominal hydatidosis which is a rare complication. Here this rare case was presenting as a 48 years old female patient of disseminated intra-abdominal hydatidosis. Disseminated abdominal hydatidosis occurs secondary to traumatic or surgical rupture of a hepatic cyst. Ultrasonography or Computed Tomography findings are helpful in making a definitive diagnosis. For localized hydatid cysts in liver or lungs, the management of choice is preferably surgical while the treatment for disseminated intra-abdominal hydatidosis remains medical. Albendazole is the treatment of choice for disseminated abdominal hydatidosis.
Study of Barthel Score among CKD Patients Belonging from Tribal Areas in Tert...ijtsrd
Chronic Kidney Disease CKD is one of the independent diseases which can lead to sever disability and it is a major emerging public health concern worldwide because it often leads to poor patient outcome 1 . Some of the associated factor with impaired functional status with CKD patients has not been fully elucidated, but some traditional such as cardiovascular diseases hypertension, myocardial ischemia , cerebrovascular diseases, and diabetes mellitus as well as non-traditional factors such as malnutrition-inflammation syndrome and depression may involve. A cross-sectional and longitudinal study has shown that risk of low functional status is directly proportional to kidney impairment 2, 3 . Thus, individuals with chronic kidney disease CKD have 40-70 higher risk of functional limitation than those without CKD 4 . In one study to assess the functional status of the CKD patients by using Barthel index found that 50 patients were dependent for the basic activities of daily life 5 In the current study, we hypothesize that there is a close relationship between the presence of CKD and the functional status of renal patients. A Study On Clinical Profile Of Sepsis Patients In Intensive Care Unit Of A Te...dbpublications
Background : Sepsis is life-threatening organ dysfunction caused by a dysregulated host response to infection which is one of the most important cause of mortality & morbidity in critically ill patients. In this study clinical profiles of the sepsis patients admitted in ICU in this part of India have been evaluated. Methods & Materials: This prospective hospital based observational study was undertaken in the department of Emergency Medicine ICU of Gauhati Medical College & Hospital, over a period of one year from August 2014 to July 2015 after obtaining institutional ethical committee clearance.
RESULTS: Clinical profiles of 50sepsis patients, with male preponderance (56%) & mortality rate 36% were studied. Mean age was 48.36 years (SD ±17.16). fever & tachycardia were present in all patients. 30 patients (60%) required ventilatory support, 28 patients (56%) required inotropic support, 10 patients (20%) required dialysis. Gram negative bacteria were found to be the predominant pathogens associated with sepsis(73.4%) where most common organism responsible was Klebsiella (36.8%). Conclusion : assessment of clinical signs & initial serological & radiological investigations are of utmost importance to detect more critically ill patients as early as possible to intervene earlier for saving the life of the sepsis patients.
1) The document provides clinical guidelines for diagnosing and treating small intestinal bacterial overgrowth (SIBO).
2) It defines SIBO as an excessive amount of bacteria in the small intestine causing gastrointestinal symptoms. A small intestine culture showing >103 CFU/mL or a positive breath test are used to diagnose SIBO.
3) The guidelines recommend breath testing using glucose or lactulose as a conditional recommendation for diagnosing SIBO in patients with irritable bowel syndrome, motility disorders, or previous abdominal surgery who have symptoms like abdominal pain or bloating.
The Role of Gedunin Against Aquaporin 2 for the Possible Treatment of Diabete...YogeshIJTSRD
Diabetes is one of the major causes of death in the world, affecting around 422 million people or 8.5 percent of the worlds population . Diabetes mellitus, or diabetes, is a disease that causes excessive blood sugar levels. Diabetes mellitus is a condition where a person does not produce enough insulin or does not link directly to insulin, and resulting in excessively high blood sugar glucose levels. Diabetes is classified into several types Prediabetes, Type 1 diabetes, Type 2 diabetes and gestational diabetes. In diabetes research, numerous bioinformatics technologies are used. The current research was carried to study the effect of naturally occurring compounds against Aquaporin 2 as the target protein molecule with the help of molecular docking for diabetes diagnosis. AQP2 is found in the apical cell of the main ducts of the kidney and in the vesicles intracellular in the cells. Docking experiment were performed out by using aquaporin 2 4nef . The technique of molecular docking was utilized to investigate the potential of naturally occurring compounds such as aglycone, gedunin, and harman with the target protein Aquaporin 2 4nef . In diabetes, gedunin may function as a therapeutic drug against the disease causing protein Aquaporin 2 4nef . Further research and then certain clinical and pre clinical studies may be allowed to discover drugs that can be used to treat diabetes.
The Impact of Digital Media on the Decentralization of Power and the Erosion ...ijtsrd
The impact of digital media on the distribution of power and the weakening of traditional gatekeepers has gained considerable attention in recent years. The adoption of digital technologies and the internet has resulted in declining influence and power for traditional gatekeepers such as publishing houses and news organizations. Simultaneously, digital media has facilitated the emergence of new voices and players in the media industry. Digital medias impact on power decentralization and gatekeeper erosion is visible in several ways. One significant aspect is the democratization of information, which enables anyone with an internet connection to publish and share content globally, leading to citizen journalism and bypassing traditional gatekeepers. Another aspect is the disruption of conventional media industry business models, as traditional organizations struggle to adjust to the decrease in advertising revenue and the rise of digital platforms. Alternative business models, such as subscription models and crowdfunding, have become more prevalent, leading to the emergence of new players. Overall, the impact of digital media on the distribution of power and the weakening of traditional gatekeepers has brought about significant changes in the media landscape and the way information is shared. Further research is required to fully comprehend the implications of these changes and their impact on society. A Study on the Effective Teaching Learning Process in English Curriculum at t...ijtsrd
“One Language sets you in a corridor for life. Two languages open every door along the way” Frank Smith English as a foreign language or as a second language has been ruling in India since the period of Lord Macaulay. But the question is how much we teach or learn English properly in our culture. Is there any scope to use English as a language rather than a subject How much we learn or teach English without any interference of mother language specially in the classroom teaching learning scenario in West Bengal By considering all these issues the researcher has attempted in this article to focus on the effective teaching learning process comparing to other traditional strategies in the field of English curriculum at the secondary level to investigate whether they fulfill the present teaching learning requirements or not by examining the validity of the present curriculum of English. The purpose of this study is to focus on the effectiveness of the systematic, scientific, sequential and logical transaction of the course between the teachers and the learners in the perspective of the 5Es programme that is engage, explore, explain, extend and evaluate. The Role of Mentoring and Its Influence on the Effectiveness of the Teaching ...ijtsrd
This paper reports on a study which was conducted to investigate the role of mentoring and its influence on the effectiveness of the teaching of Physics in secondary schools in the South West Region of Cameroon. The study adopted the convergent parallel mixed methods design, focusing on respondents in secondary schools in the South West Region of Cameroon. Both quantitative and qualitative data were collected, analysed separately, and the results were compared to see if the findings confirm or disconfirm each other. The quantitative analysis found that majority of the respondents 72 of Physics teachers affirmed that they had more experienced colleagues as mentors to help build their confidence, improve their teaching, and help them improve their effectiveness and efficiency in guiding learners’ achievements. Only 28 of the respondents disagreed with these statements. With majority respondents 72 agreeing with the statements, it implies that in most secondary schools, experienced Physics teachers act as mentors to build teachers’ confidence in teaching and improving students’ learning. The interview qualitative data analysis summarized how secondary school Principals use meetings with mentors and mentees to promote mentorship in the school milieu. This has helped strengthen teachers’ classroom practices in secondary schools in the South West Region of Cameroon. With the results confirming each other, the study recommends that mentoring should focus on helping teachers employ social interactions and instructional practices feedback and clarity in teaching that have direct measurable impact on students’ learning achievements. Design Simulation and Hardware Construction of an Arduino Microcontroller Bas...ijtsrd
This study primarily focuses on the design of a high side buck converter using an Arduino microcontroller. The converter is specifically intended for use in DC DC applications, particularly in standalone solar PV systems where the PV output voltage exceeds the load or battery voltage. To evaluate the performance of the converter, simulation experiments are conducted using Proteus Software. These simulations provide insights into the input and output voltages, currents, powers, and efficiency under different state of charge SoC conditions of a 12V,70Ah rechargeable lead acid battery. Additionally, the hardware design of the converter is implemented, and practical data is collected through operation, monitoring, and recording. By comparing the simulation results with the practical results, the efficiency and performance of the designed converter are assessed. The findings indicate that while the buck converter is suitable for practical use in standalone PV systems, its efficiency is compromised due to a lower output current. Sustainable Energy by Paul A. Adekunte | Matthew N. O. Sadiku | Janet O. Sadikuijtsrd
Energy becomes sustainable if it meets the needs of the present without compromising the ability of future generations to meet their own needs. Some of the definitions of sustainable energy include the considerations of environmental aspects such as greenhouse gas emissions, social, and economic aspects such as energy poverty. Generally far more sustainable than fossil fuel are renewable energy sources such as wind, hydroelectric power, solar, and geothermal energy sources. Worthy of note is that some renewable energy projects, like the clearing of forests to produce biofuels, can cause severe environmental damage. The sustainability of nuclear power which is a low carbon source is highly debated because of concerns about radioactive waste, nuclear proliferation, and accidents. The switching from coal to natural gas has environmental benefits, including a lower climate impact, but could lead to delay in switching to more sustainable options. “Carbon capture and storage” can be built into power plants to remove the carbon dioxide CO2 emissions, but this technology is expensive and has rarely been implemented. Leading non renewable energy sources around the world is fossil fuels, coal, petroleum, and natural gas. Nuclear energy is usually considered another non renewable energy source, although nuclear energy itself is a renewable energy source, but the material used in nuclear power plants is not. The paper addresses the issue of sustainable energy, its attendant benefits to the future generation, and humanity in general. Paul A. Adekunte | Matthew N. O. Sadiku | Janet O. Concepts for Sudan Survey Act Implementations Executive Regulations and Stand...ijtsrd
This paper aims to outline the executive regulations, survey standards, and specifications required for the implementation of the Sudan Survey Act, and for regulating and organizing all surveying work activities in Sudan. The act has been discussed for more than 5 years. The Land Survey Act was initiated by the Sudan Survey Authority and all official legislations were headed by the Sudan Ministry of Justice till it was issued in 2022. The paper presents conceptual guidelines to be used for the Survey Act implementation and to regulate the survey work practice, standardizing the field surveys, processing, quality control, procedures, and the processes related to survey work carried out by the stakeholders and relevant authorities in Sudan. The conceptual guidelines are meant to improve the quality and harmonization of geospatial data and to aid decision making processes as well as geospatial information systems. The established comprehensive executive regulations will govern and regulate the implementation of the Sudan Survey Geomatics Act in all surveying and mapping practices undertaken by the Sudan Survey Authority SSA and state local survey departments for public or private sector organizations. The targeted standards and specifications include the reference frame, projection, coordinate systems, and the guidelines and specifications that must be followed in the field of survey work, processes, and mapping products. In the last few decades, there has been a growing awareness of the importance of geomatics activities and measurements on the Earths surface in space and time, together with observing and mapping the changes. In such cases, data must be captured promptly, standardized, and obtained with more accuracy and specified in much detail. The paper will also highlight the current situation in Sudan, the degree to which survey standards are used, the problems encountered, and the errors that arise from not using the standards and survey specifications. Towards the Implementation of the Sudan Interpolated Geoid Model Khartoum Sta...ijtsrd
The discussions between ellipsoid and geoid have invoked many researchers during the recent decades, especially during the GNSS technology era, which had witnessed a great deal of development but still geoid undulation requires more investigations. To figure out a solution for Sudans local geoid, this research has tried to intake the possibility of determining the geoid model by following two approaches, gravimetric and geometrical geoid model determination, by making use of GNSS leveling benchmarks at Khartoum state. The Benchmarks are well distributed in the study area, in which, the horizontal coordinates and the height above the ellipsoid have been observed by GNSS while orthometric heights were carried out using precise leveling. The Global Geopotential Model GGM represented in EGM2008 has been exploited to figure out the geoid undulation at the benchmarks in the study area. This is followed by a fitting process, that has been done to suit the geoid undulation data which has been computed using GNSS leveling data and geoid undulation inspired by the EGM2008. Two geoid surfaces were created after the fitting process to ensure that they are identical and both of them could be counted for getting the same geoid undulation with an acceptable accuracy. In this respect, statistical operation played an important role in ensuring the consistency and integrity of the model by applying cross validation techniques splitting the data into training and testing datasets for building the geoid model and testing its eligibility. The geometrical solution for geoid undulation computation has been utilized by applying straightforward equations that facilitate the calculation of the geoid undulation directly through applying statistical techniques for the GNSS leveling data of the study area to get the common equation parameters values that could be utilized to calculate geoid undulation of any position in the study area within the claimed accuracy. Both systems were checked and proved eligible to be used within the study area with acceptable accuracy which may contribute to solving the geoid undulation problem in the Khartoum area, and be further generalized to determine the geoid model over the entire country, and this could be considered in the future, for regional and continental geoid model. DISCUSSION
The study undertaken here is one of the most
common condition found in our day-to-day life. This
condition is not the one which may prove to be life
threatening, but is the one which causes lot of
discomfort as the array of symptoms which it
produces, mainly like Heartburn and Regurgitation.
GERD is the most common condition of upper GIT,
involving lower oesophageal sphincter. GERD is one
of the chronic, common and relapsing condition.
Hence it is necessary to diagnose, cure the disease
and prevent the recurrence in order to improve quality
of life. About 66% of females between age of 10 to
69 year show significant Symptoms as compared to
34% of males in the same age group. It is also
detected in pregnant women. It may be present as
secondary manifestations to some illness or it may be
drug induced. In other mode of treatment, they
concentrate more over controlling the acute attack for
management of disease rather than giving permanent
cure & considering the patient who is suffering. They
try to manage the disease by giving oral as well as
injectable Antacids, in which chance of recurrence is
much higher & sometimes due to recurrent
administrations of PPI, symptoms of Developing
Structural changes may remain unrecognize.
Homoeopathic mode of treatment is superior to other
modes of treatment since it treats the person as a
whole, not the diseased parts or organs. It is the man
who is sick and not his body and as a matter of fact he
needs to be treated.
It is here the concept of individualization comes into
practice, where the physical as well as the mental
characteristics of the individual is taken.
This includes, detailed case taking, in order to make
portrait of disease, which helps in person diagnosis,
disease diagnosis and management of case, both
specific as well as general.
We in Homoeopathy, believes in concept of
Individualization. We decide the potency as per
susceptibility of the patient. Susceptibilityis power of
organism to react against external stimuli. Higher the
susceptibility, High will be the potency.
Therefore, a study has been taken to evolve a suitable

In this study maximum incidence of GERD was seen
in the age group of 30-39 years, which is 24 patients
(48%), in which 14 patients were male and 10
patients were female, out of 50 (100%). 9 (18%) were
in the age group of 20-29, in which 1 patient were
male and 8 patients were female. 7 (14%) were in the
age group of 40-49 years, in which 3 patients were
male and 4 patients were female. 5 (10%) were in the
age group of 60-69 years in which 2 patients were
male and 3 patients were female. And 4 (8%) were in
the age group of 50-59 years, in which 2 patients was
male and 2 patients was female. As per the literature,
Incidence of GERD is increasing with age, commonly
seen in female> male. This study is agreeing with
that.
As seen in the study 33 patients (66%) were female
and 17 patients (34%) were male.
As per the literature, GERD Affects the people
irrespective of sex. This study is agreed with this.
As per study, Nux vomica is given in 4 cases out of
50 cases & Bryonia, Natrum mur, Carbo veg were
given for 3 times. But this doesn’t indicate a group of
medicines, particularly indicated in this condition.
This means that, though disease condition may
remain same, the sick individual will manifest the
symptoms of as per their own individual pattern of
reaction & their own mode of living. Hence, in this
study, Selection of medicine has done on the basis of
individualization.
As per study, out of 50 cases: 38 patients (76%) were
markedly or significantly improved, 9 patients (18%)
were moderately improved, 3 patients (6%) were not
improved.
SUMMARY AND CONCLUSION
GERD is a common health problem which many
times remain undetected due to free and wide spread
use of PPIs & sometimes patient may diagnose the
complaint by themselves & they avoid to go to clinics
unless they cause discomfort.
But the GERD can be very well treated with
homeopathic remedies without any complications. It
commonly affects the quality of life of patient. 50
different cases have been taken, who are suffering
from GERD. (based on inclusion and exclusion
criteria) between the age of 10-69 years, from the
Sainath Hospital, collaborated with Ahmedabad
Homeopathic Medical college for the study. These
cases were followed regularly.
They were studied according to age, sex, according to
type of disease, according to remedy selection and
results.
Chief complains, Causation, Sensation, Location,
modalities & concomitants. Along with other
associated co-morbidities, constitution, past history,
family history, physical generals, and mental are
considered in order to select similimum. Laws of
homeopathy (law of similar, law of single, law
minimum dose and law of individualization) were
followed in all the cases.
Wherever possible majority of cases are studied with
laboratory investigations, which are CBC, Urine R/M,
SGPT, Creatinine, RBS. And in cases with chest pain
& Gabharaman, ECG has also taken in order to rule
out any cardiac cause.
Majority of cases which has studied were taken
primarily allopathic treatment. but due to repeated
attacks and increase severity, they selected
homeopathic mode of treatment.
Here, in each cases remedy was selected on the basis
of law of similarity. Individualization of the patient,
in order to reduce suffering & prevent relapses.
Patients are advised to take regular medicine and
follow the preventive measurement given.
After this study, I reach on following conclusion:
Out of 50 cases, Marked/Significant improvement
found in 38 patients (76%), Moderate
improvement found in 9 patients (18%), No
improvement seen in 3 patients (6%). This proves
that, when homeopathic medicine selected on the
basis Symptom similarity, (considering Mental
and Physical) it gives desirable results.
In my study I have found females are affected
more than males. Majority cases are found in the
age group of 30-39years (48%). Most of the
patients are found to having Classical symptoms
of GERD (Heartburn, Regurgitation) along with
Headache & Constipation.
Though there are so many specific, indicated
medicines for GERD, but in my study, I found
that no two cases are alike. Hence, medicine is
selected on the basis of symptom similarity and
individualization of patients. the recovery,
Significant improvement, Moderate improvement
and No improvement was 76:18:6.
Thus, positive result was obtained.
This shows that, no medicine can be a specific for all
the cases of GERD.
This work considering conscientious and diligent
observations made on 50 cases and its statistical
outcome concludes that homeopathic medicine if
chosen authentically on the basis of Symptom
similarity, can bring about the outstanding results,

irrespective of severity of symptoms, gross local
pathological changes.
Secondly, a single remedy given with the
minimum dose shall bring about the steady but
sure and consistent result in the cases of GERD.
In some cases, where improvement stops,
Administration of Intercurrent medicines, will
help to bring about cure. In my study, I have Used
sulphur, Thuja, Medorrhinum etc as an
intercurrent medicines in cases of moderate
improvement.
As Upper GI Scopy was not carried out in all the
cases, the exact aetiology couldn’t be understood.
Therefore, the effectiveness of each these
homeopathic medicines in different conditions
could not be analysed.
RESULTS
The results obtained from this studyhas utilityin day-
to-day practice. I have found in study that few cases
didn’t response to the medicine selected, but the
result was positive.
In each case, similimum was given, based on
individualization. The present attempt of this study is
to evaluated the role of homeopathic medicines in
GERD, to reduce the sufferings, prevent recurrence.
GERD is one of the most common problem now a
days due to major life style errors in both men &
women, having maximum time visits to the hospital.
The demand of the time to improve clinical
applicability of homeopathic remedies. This is my
sincere endeavour to throw light on the subject and to
initiate more research work in this channel. Despite
the modern trend towards group thinking, group
practice, emergence of very recent concept of
universality and globalization, Individualisation is
still unique and continued to be as an important factor
as before towards sustained growth of mankind and
homeopathy. Homeopathy is principle concerned with
the law of SIMILARITY, so selection of medicine is
based on symptom similarity, no specification or
groupism were considered. Only law of similarity is
considered. I humbly submit this work to
Homeopathic fraternity for their scrutiny, approval,
guidance and application.
