The National Heart, Lung, and Blood Institute provided funding of up to $3,000 to 30 We Can! community sites to support their programs promoting healthy eating, physical activity, and reduced screen time among youth ages 8-13 and their parents/caregivers. The selected sites proposed a variety of activities, such as soccer and nutrition programs, health fairs, community field days, and family fitness expos, to educate over 25,000 people across 18 states on adopting healthy behaviors. Details were provided on four of the selected sites and their proposed programs.

Unavoidably in healthcare there are policy implication and barriers that arise in care. Infinity is driven to enrich the lives of those who are at the end-of-life. However, barriers to Medicaid, Medicare and in the state of Nevada, Culinary Health insurance all have their own policies in which patient eligibility often factors into their quality of life. Social workers could greatly improve the effectiveness of policy and service efforts designed.
There were 9 community health worker (CHW) programs represented in Kent County, Michigan that employ a total of 51 CHWs on average. The majority of CHW programs address issues like diabetes, hypertension, maternal/child health, health literacy, asthma, nutrition, and obesity. CHWs provide services like outreach, health promotion, systems navigation, and case management in various locations like clients' homes, community events, and agencies. Most CHW programs receive funding from local grants, state grants, foundations, and federal grants. Barriers to sustainability include funding uncertainty and lack of management support for CHWs in some agencies.
Ohio's Medicaid program made health care available to more people than ever before in 2014, but there's more work to be done.
The slides include an update on enrollment and health care access in Ohio, what's next for Ohio's Medicaid program, why personal stories are critical in our efforts to support health care access, and how you can get involved to keep Ohio’s families and communities healthy in the coming months.
Kaiser Permanente San Francisco's 2014 Community Investment Report details how the organization contributed close to $4 million to support community health programs in San Francisco. The report describes grants provided to numerous local organizations addressing priorities like access to health care, healthy eating and active living, safe and healthy living environments, and broader healthcare system needs. Major grants were provided to initiatives improving chronic disease management, increasing access to healthy foods and physical activity, supporting at-risk youth, and connecting people to employment in healthcare.
This document summarizes the efforts of the REACH Charleston and Georgetown Diabetes Coalition to decrease diabetes-related amputations among African Americans. The coalition conducted community skill-building and health systems changes. Through training health professionals and lay educators, they provided diabetes self-management education and foot care education to over 45,000 people. Health systems implemented quality improvement teams and a diabetes registry. Preliminary results found a 44% reduction in amputations for African Americans compared to 1999, saving over $2 million per year.
UHPP provides health care solutions for underserved Utahns through policy work, public outreach, education, and enrollment assistance. In 2015, UHPP experienced its best year yet with over 175,000 Utahns enrolled in insurance through healthcare.gov. Revenue increased from 2014 due to more foundation and federal grants, allowing UHPP to expand its staff and programs. While open enrollment success increased insurance access, thousands remain uninsured due to the Medicaid coverage gap, and UHPP's work continues advocating for solutions to insure all Utahns.

The goals are to assist with recovery, reduce cultural barriers, and connect clients to community resources. At-risk populations served include refugees suffering from trauma and adults with serious mental illness or co-occurring disorders from Southeast Asian backgrounds. API aims to provide socially just, culturally competent care through an ethnically diverse staff and use of interpreters.

toward improving the quality of healthcare for the elderly population with chronic disease in our area,
focused on provider and community efforts to increase health literacy and reduce avoidable 30-day
hospital readmissions.
Over the past 20 months, our coalition has studied and analyzed drivers of readmission, applying focused
interventions addressing these drivers. We are ready for the next step - bringing the valuable resources
found in Acadiana to the attention of the community in order to increase awareness and provide a public
forum for achieving better health for seniors in our communities.
