Reporting System is a general term applied to a wide range of applications that are
used for collecting, storing, analyzing and providing access to data in order to help
the decision makers to improve quality of care.

1. Change of shift report
2. Telephone reports
3. Telephone orders
4. Transfer reports
5. Incident report
6. Legal report
CRITERIA OF GOOD REPORT
 Can be made promptly
 Clear, Concise and Complete
 All pertinent, identifying data included
 Mention all people concerned, situation and signature of person making
report
 Easily understood
