Introduction: Reliability and production
loss elimination are critical
components for the success of any
industry. Ensuring equipment,
machines, and systems perform their
intended functions without degradation
or failure is essential for operational
efficiency and effectiveness.
Key Findings: The document
emphasizes the identification of
integrity reliability issues through
various means such as PMR failures,
SCE failures, and site audits.
Understanding and addressing these
issues are vital for maintaining
operational integrity and efficiency. It
highlights the structured approach of
Production Loss Elimination (PLE)
processes, which are designed to
effectively manage potential or actual
production loss reliability issues.
PLE processes provides a systematic and effective method to manage potential or actual production loss reliability issues. It
involves effective identification, analysis, and mitigation of potential threats, ultimately reducing the risk of production losses and
improving operational performance and efficiency.
PLE Process Key Aims:
 VISIBILITY of emerging production threats and unplanned loss events
 Appropriate level of ANALYSIS is conducted when required
 SMARTER preventative and corrective ACTIONS
 MONITORING to prevent production loss escalation
 using basic tools such as ‘5-Why’ and where necessary escalating to more detailed Root Cause Analysis (RCA) tools.

Structured Approach of Production Loss Elimination (PLE) Processes
High Level Process Flow
PLE COMPONENTS Production Loss Management System (PLMS) is a software tool which
records production targets and losses (planned and unplanned)
Threat is any equipment issue, working practice or external factor that
could directly or indirectly affect production
RCA is a method employ to identify the underlying reasons
behind a problem or production loss. It helps pinpoint the
fundamental causes of inefficiencies or disruption in the
production
CEAT is a process used to monitor and measure uptime and
performance of key equipment in production facilities
Loss Recording is a process used to record, document and
categorize different types of losses that occur in the production
N:B
All actions derived from these component will be recorded
within PLMS and monitored towards closure using available
PLMS reporting
Key PLE components comprise:
 PLMS Entry, Quality Checks and Reporting
 Emerging Production Threats (Identifying and
Managing)
 Loss Recording
 RCA
 Critical Equipment Availability Tracking

PLE RELIABILITY STRATEGY
Breakdown of key components:
 Safety Critical Element (SCE) Performance Standard failures or deteriorating trends identified through
Assurance Planned Maintenance Routines (PMRs) or corrective intervention are investigated and
measures implemented to avoid problem reoccurrence or eventual failure.
 Proactively manage incurred or potential production losses associated with equipment failures or
performance degradation and ensure investigative analysis effort is aligned to quantified losses or
perceived threat.
 Appropriate Maintenance Culture.
 Actual or Potential Problem Recording & Validation.
 Investigation Execution and Management.
 Rectification Work Implementation and Action Tracking.
PLE Reliability Strategy specifically focuses on optimizing the reliability and availability of production system and equipment to
minimize downtime and maximize productivity.
This strategy emphasizes proactive maintenance, predictive analytics, asset performance management and other measure
aimed at ensuring the reliable operation of production assets

Focuses on selecting high-impact problems which will be rectified
by dedicated PLE Reliability Resource to ensure greatest business needs
are addressed.

Documenting the current process, validating how
it is measured and assessing baseline performance. Within agreed detailed
boundaries of agreed charter or procedures

This stage executes full detailed analysis of defined problem, covering:
• Establish, Document & Confirm Timeline.

Based on validated findings, define requirements to ultimately deliver a
Fixed Forever Solution:
• Present Recommendations.
• Generate Specific, Measureable, Achievable, Realistic & Timely
(SMART) Corrective Actions.
• Agree Single Point Accountability & Responsibility for action
completion.
• Agree Target Date for completion.
Control
Monitor recommended solution implementation:
• Where Applicable, Verify Detailed Plan Exists to Close Action.
• Identify Threats to Delivery.
• Formally Close Reliability Investigative Work.

Progress Monitoring
The Reliability Hopper shall accurately detail status of PLE Reliability Team
investigative work. This shall be formally issued to all relevant stakeholders monthly.

 NEPL faces operational challenges related to reliability
and production losses, which can impact its overall
performance and efficiency. Adopting the Production
Loss Elimination Reliability Strategy can help NEPL
address these challenges effectively by implementing
structured processes and proactive measures to
enhance reliability and eliminate production losses.
Organizations are recommended to define "Fixed Forever
Solutions" based on validated findings to address root
causes effectively. These solutions should be sustainable
and aimed at preventing recurrence of reliability issues.
Implementing SMART corrective actions (Specific,
Measurable, Achievable, Realistic, Timely) ensures that
solutions are well-defined, measurable, and aligned with
organizational goals.
