1. Preliminary hazard analysis (PHA) is a semi-quantitative analysis performed early in a project to identify potential hazards, accidental events, and their severity. It is used to focus risk analyses and as input for more detailed analyses.
2. A PHA team identifies hazards from system descriptions, energy sources, and accident databases. Hazards include mechanical, electrical, chemical and other sources.
3. The PHA identifies hazards and accidental events, estimates their consequences and frequency, then ranks risks and identifies required hazard controls and follow-up actions. The results are documented in a PHA worksheet.
