This document discusses process safety management and process hazard analysis. It defines hazards and accidents that drove the development of OSHA's Process Safety Management standard. The standard aims to proactively identify and prevent chemical releases through requirements like process hazard analysis, operating procedures, training, and emergency response. Process hazard analysis methods like What-If, checklist, HAZOP, FMEA, and fault tree analysis are introduced and their pros and cons discussed. While no method can identify all accident scenarios, process hazard analysis plays an important role in examining industrial processes and reducing accident frequency and severity.
