64-page Excerpt from Process Hazards Analysis Book ( https://bin95.com/ebooks/hazop_risk_management.htm ) by Ian Sutton.
Process Hazards Analyses (PHAs) are an integral part of any process safety program. If you cannot identify hazards then you can neither remove them nor mitigate against them. This book describes in detail how to run a Process Hazards Analysis, and how to effectively identify hazards. The book focuses on real-world issues such as the role of the leader, team selection, writing the final report, quantification of risk, and follow-up to findings.
Quotations and topics raised in the book include the following:
* A PHA leader "comforts the afflicted but afflicts the comfortable"
* Don't confuse "common cause" with "root cause"
If it's not there, it can't leak
* Deciding whether to focus on high risk or on high consequence hazards
* The importance of MAWP values
* Distinguish between Findings, Recommendations, and Action Items
This book has been reviewed by the authoritative journal Chemical Engineering. A copy of that review is accessible at the link above.
A hazard is a situation that poses a level of threat to life, health, property, or environment. Most hazards are dormant or potential, with only a theoretical risk of harm; however, once a hazard becomes "active", it can create an emergency. Hazard and possibility interact together to create risk.
Identification of hazard risks is the first step in performing a risk assessment.

HAZOP, or a Hazard and Operability Study, is a systematic way to identify possible hazards in a work process. In this approach, the process is broken down into steps, and every variation in work parameters is considered for each step, to see what could go wrong. HAZOP’s meticulous approach is commonly used with chemical production and piping systems, where miles of pipes and numerous containers can cause logistical headaches.

Analyzing The Near Miss: Are companies overlooking crucial data?Garrett Foley
Near misses are predictive, showing the potential for future accidents. If a company doesn’t record its near misses, it’s missing a wealth of predictive information that could help prevent serious safety problems. For this reason, employees should be trained to identify near misses, and reporting these instances should be as quick and simple as possible. Nothing provides safety managers with more details and more trend data than near misses. They are the canary in the coal mine, helping to predict where, when, and why an accident will happen. With robust near-miss reporting, metal fabricators can catch safety issues long before they turn dangerous or even tragic.
Increase Hazard Discovery and Minimize Errors in your Process Hazard Analyses...R. Maqbool Qadir
This paper presents a mind mapping tool to improve the completeness of hazard identification during process hazard analysis (PHA). PHA methods like HAZOP and FMEA are mapped as tree graphs to visualize causal connections and inferences. Seventeen sources of errors and omissions (E/O) are mapped to the issues they affect and a quality assurance checklist is derived from a fault tree analysis of the HAZOP process. CSB case studies are cited showing the impact of the E/O on hazard discovery in real incidents.
Gear pump operation & maintenance.
REF: https://bin95.com/vocational-training/maintenance/reliability-training.htm
A gear pump uses two meshing, toothed cogs to force liquid from the inlet of the pump through to the outlet.
Being a positive displacement pump there deliver very precise quantities for each revolution, and this means they have good dosing characteristics
regardless of their speed or the pressure into which they discharge. Keywords: internal, external teeth, pressure relief, viscosity.
Excerpt: "The design of a gear pump lends itself to use with clean liquids. Ensure they draw liquid from well above the
bottom of the supply tank in clear liquid space. Both low and high viscosity liquids can be pumped. If food
grade products sensitive to shear (i.e., where the churning action of the pump breaks cells and fibers) are to be
pumped the size of the pump will need to be increased and the speed reduced."

Excellent safety and environmental performance in the process industries does not happen by chance; after all, most process facilities handle large quantities of toxic, flammable, and explosive materials, often at high temperatures and pressure. Such processes are inherently hazardous. Therefore process risk must be properly understood and managed.

A Strategic Approach: GenAI in EducationPeter Windle
Artificial Intelligence (AI) technologies such as Generative AI, Image Generators and Large Language Models have had a dramatic impact on teaching, learning and assessment over the past 18 months. The most immediate threat AI posed was to Academic Integrity with Higher Education Institutes (HEIs) focusing their efforts on combating the use of GenAI in assessment. Guidelines were developed for staff and students, policies put in place too. Innovative educators have forged paths in the use of Generative AI for teaching, learning and assessments leading to pockets of transformation springing up across HEIs, often with little or no top-down guidance, support or direction.
This Gasta posits a strategic approach to integrating AI into HEIs to prepare staff, students and the curriculum for an evolving world and workplace. We will highlight the advantages of working with these technologies beyond the realm of teaching, learning and assessment by considering prompt engineering skills, industry impact, curriculum changes, and the need for staff upskilling. In contrast, not engaging strategically with Generative AI poses risks, including falling behind peers, missed opportunities and failing to ensure our graduates remain employable. The rapid evolution of AI technologies necessitates a proactive and strategic approach if we are to remain relevant.
