This document is a pre-internship evaluation for a student to self-assess their skills in areas such as communication, interpersonal skills, professional work ethic, teamwork, detail orientation, organization, leadership, self-confidence, time management, problem solving and sensitivity to organizational culture. The student is asked to rate each skill on a scale from 1 to 5. Completing this evaluation will help the student improve weak skills during the internship and analyze growth after the internship ends. The student is also asked questions to identify skills they want to learn and strengths they will contribute, and how the internship will prepare them for future work.
