Introduction
Diabetes is a multifaceted disease that affects over 29 million individuals in the United States although most of them are not aware they are sick. The prevalence of diagnosed diabetes is higher among certain minorities such as Alaska Natives, American Indians, Hispanics, Asian Americans and non-Hispanic Black. Its reduces muscle, liver and adipose tissue insulin sensitivity to as well as a decrease pancreatic b-cell function leading which can impair insulin secretion. Diabetes mellitus (DM) can be in form of Type 1 DM, gestation DM, type 2 DM, Pre-DM or medication-induced DM while all bear high levels of blood glucose. Lately, new antidiabetic medications have been adopted for the treatment of T2DM, including dipeptidyl peptidase-4 (DPP-4) inhibitors, glucagon-like peptide 1(GLP-1) receptor agonists, anamylin analogue, and sodium-glucose cotransporter 2 (SGLT2) inhibitors. Of all cases of diabetes, 90% - 95% is accounted by type 2 DM (Olokoba, Obateru & Olokoba, 2012). It is characterized by the body failing to use insulin properly because of the relative deficiency in insulin or insulin resistance. Failure to manage type 2 diabetes appropriately can cause grave complications such as neuropathy, retinopathy and cardiovascular diseases. .
Problem
Diabetes Mellitus type 2 has become an epidemic in the adult population and managing the glucose levels has become a serious problem with close to 285 million people worldwide affected (International Diabetes Federation. 2011). The number of Americans people affected by type 2 diabetes mellitus has continued to grow by 1.9 million cases every year. The problem has been rampant on older people in the United States who account for 10.9 million cases. Among the factors identified as drivers of type 2DM epidemic is physical inactivity, increased age of the U.S. population, increase in the sub-populations prone to diabetes and obesity epidemic. The cost of type 2DM is enormous and in 2012, approximately $176 billion was used in direct medical costs and $69 billion in lost due to lack of productivity.

Abstract
Background: Using health coaching to improve the quality of life and health outcomes of the patients with
diabetes mellitus, has emerged as a possible intervention. However, the few published randomized controlled trials
using health coaching for patients with diabetes mellitus have reported mixed results. The present meta-analysis
aimed to determine the effectiveness of health coaching on modifying health status and quality of life among
diabetic patients and to clarify the characteristics of coaching delivery that make it most effective. Methods: This
study searched for articles on randomized controlled trials of health coaching interventions targeting type 2 diabetic
patients that were published in the English language from January 2005 through December 2018 in the Cochrane,
Medline, PubMed, Trip, and Embase databases. Patients in the control group received usual diabetes mellitus care,
and those in the experimental group received health coaching based on usual diabetes mellitus care. The primary
outcomes included Hemoglobin A1c (HbA1c) and cardiovascular disease risk factors, including systolic blood
pressure, diastolic blood pressure, triglyceride, high-density lipoprotein cholesterol (HDL-C), low-density
lipoprotein cholesterol, total cholesterol, and body weight. The secondary outcomes included quality of life,
self-efficacy, self-care skills, and psychological outcomes. Results: Health coaching intervention has a significant
effect on HbA1c [mean difference (MD) = -0.35, confidence interval (CI) = -0.47, -0.22, I2 = 83%, P < 0.001] and
HDL-C (MD = -0.50, CI = -0.93, -0.07, I2 = 10%, P = 0.02). The most effective strategy for health coaching
delivery associated with improvement of HbA1c was decreasing the number of sessions and increasing the duration
of each session. However, no significant difference was found for weight, SBP, diastolic blood pressure,
triglyceride, low-density lipoprotein cholesterol, or total cholesterol. Mixed results were reported for the effect of
health coaching on quality of life, self-efficacy, self-care skills, and depressive symptoms outcome. Conclusion:
Health coaching intervention has a significant effect on HbA1c and HDL-C, and the most effective strategy is
decreasing the number of sessions while increasing session duration. However, these results should be interpreted
with caution as the evidence comes from studies at some risk of bias with considerable heterogeneity and
imprecision.
Researchers used the Health Belief Model (HBM) to account for respondents’ lack of knowledge and the negative effects of cultural variations on their actions. Using the most up-to-date ideas during planning and development is essential for achieving desired outcomes (Ghosh & Saboo, 2022). Education is a useful tool in treating insulin resistance in people with diabetes. As a strength of the research, patients with diabetes with education had better glycemic control, higher medication adherence, and more developed self-management abilities, as documented by Liu et al. (2021). Unfortunately, it is difficult to draw firm conclusions from the studies because of the limitations that exist in some of them. Some research, for instance, relies on participants’ self-reports, which could be inaccurate or biased, which is one of the study’s shortcomings. The difficulty in comparing the efficacy of education to that of other interventions or standard care is compounded by the fact that some studies need a control group. Furthermore, the effects of education on other outcomes, such as quality of life or healthcare utilization, are rarely evaluated in studies (Tucker et al., 2021). So, more studies are required to evaluate the long-term effects of education on insulin resistance treatment and its cost-effectiveness compared to alternative interventions. In addition, further research is needed to determine the best methods for customizing patient education to meet each person’s unique requirements.

up care to prevent comorbidities. With an increased population suffering from diabetes, mainly type 2 diabetes, traditional
treatments are ineffective, and a new treatment approach should be adopted. While this is deemed a plausible solution to curb the
increase of diabetes, research indicates that 70% of quality improvement initiatives fail within twelve months of implementation
(O'Donoghue et al., 2021). Therefore, stakeholders must follow proposed improvements methods closely to achieve meaningful
and sustainable change. To combat widespread chronic diseases such as diabetes, strategies such as self-management support,
intensified treatment, encouraged physical activity, and patient education plays a crucial role in managing a patient's condition.
The disease heavily relies on one self-management abilities. The proposed strategies aim to achieve patient adherence to prevent
other health effects that can be otherwise be contained and ensure that mental distress often experienced by diabetes patients is
adequately dealt with.

Quality Improvement Methods
• The word "quality improvement" refers to the practice of enhancing
the intended outputs of an existing process. Typically, this would need
previous knowledge of the process and the areas that may be
improved.
• Once a problem has been identified, it is critical to develop a plan of
action to improve the outcomes in that area. Recent studies have
indicated the sufficient evidence-to-clinical practice gap in diabetes
care (Mukerji et al., 2019). Upon discovering this, several plans of
action are required to improve the gaps in care delivery towards
diabetes patients.
• While most providers concentrate on the physical aspect of the
patient's health, research indicates that diabetes patients are often
affected by depression and diabetes distress ( Gary et al., 2019). This
results in underdiagnosis and undertreatment of diabetes patients,
which impedes patients' chances of managing their health condition.
• With the identification of this, challenges within primary care must be
addressed to ensure that there is sufficient screening for both
depression and diabetes distress.

One of the clinical conditions affecting numerous people globally is diabetes. Around 35
million people in the United States are diagnosed with diabetes annually. Based on the world
statistics, diabetes pervasiveness has increased promptly among people of low and middle
incomes. Accordingly, people living in countries such as India and China have the highest
prevalence of diabetes. Globally, approximately 5.0 million deaths resulted from diabetes and
diabetes-related disorders. Again, the countries spend billions of dollars on health expenditures for
diabetic patients and other approaches to minimize its prevalence through education and other
evidence-based practices. Besides, more than 400 million people globally are likely to develop
type 2 diabetes mellitus. The executive summary will describe some of the challenges facing
healthcare, such as type 2 diabetes, and ideal strategies and measures to mitigate such health issues.

Globally, type 2 diabetes mellitus is a chronic health issue affecting numerous people.
From statistics, persons diagnosed with the disease have increased significantly in the past few
decades. The Center for Disease Control has pointed out a high incidence of type 2 diabetes
mellitus among children, the elderly, and teenagers (Mayer-Davis et al., 2017). Some of the risk
factors aligned with diabetes mellitus encompass behavioral practices, lifestyle, and genetics. A
proposed change to lessen the prevalence of type 2 diabetes mellitus entails implementing
programs including the Lifestyle Change Program and Diabetes Management Education and
Support (DMES). Such a proposed change will play a central role in raising awareness and giving
people critical information regarding type 2 diabetes, its prevention, and management (Munshi et

Pharmacologic Weight Loss: An Underutilized Practice in the Fight Against Obe...CrimsonPublishersIOD
Obesity is considered one of the most contemporary threats to non-communicable disease such as cardiovascular disease, diabetes, musculoskeletal disorders and even some types of cancers. Its worldwide prevalence has nearly tripled between 1975 and 2016. In 2016, more than 1.9 billion adults aged 18 years and older were categorized as overweight, and of these over 650 million adults were obese. However, Weight management medications (WMM) are currently underutilized as an adjunct to behavioral and lifestyle interventions. By way of example, only 2% of eligible veterans received prescriptions for pharmacologic weight loss in the 2014-2015 fiscal years, and up to 1% of obese U.S. individuals filled a prescription for a WMM between 2009-2013. There are currently five FDA-approved medications for long-term weight loss medications. We analyzed 24 randomized clinical trials of the five drugs and interpreted findings. Of those 24, lorcaserin (Belviq®), naltrexone and bupropion (Contrave®), and phentermine and topiramate (Qsymia®) had four studies each, while liraglutide (Saxenda®), and orlistat (Xenical®) had six studies each. Underutilization of pharmacologic weight corrective therapies that have been statistically and clinically proven to be valuable tools in reducing obesity and its related risk factors. Studies of the five FDA-approved drugs have demonstrated clinically significant positive effects on weight loss with differing effects on both cardiovascular and glycemic markers/risk factors.

Background: Diabetes mellitus is among the most common chronic non-communicable diseases. The development of microalbuminuria in type 1 diabetes increases the risk for renal and cardiovascular disease.
Methods: A cross sectional study was conducted at the Primary Health Care Clinics at King Fahad Armed Forces Hospital, Jeddah, Saudi Arabia. A total of 334 Saudi with type 1 diabetes were randomly selected.
Results: Total of 334 patients with T2DM included in this study; 102 (30.5%) male and 232 (69.5%) female with mean age 25.8±3.4. MA was present in 99 (29.6%). MA was not significantly more prevalent in female (69.4%) with female predominance (sex ratio male: female) 1:2.3. HTN with MA was significantly more prevalent in 51(51.5%) of MA group with odd ratio 1.7 (1.2-2.4), p=0.001 with no siginificant difference between both gender. Patients with MA have significant higher HbA1c than patients with normal buminuria and there was a significant difference between gender (p< 0.0001) and when compared to HbA1c groups (p=0.002).
Conclusion: The frequency of microalbuminuria in patients with type 1 diabetes in this study is high. It is mandatory to have adequate diagnostic, therapeutic and educational resources in addition to competent physicians who can manage microalbuminuria in diabetic patients by using a continuing, comprehensive and coordinated approach.
