Conclusion Homeopathy is effective in improving lung capacity and health status. Benefits remain evident after a year. This suggests that homeopathy could make an important contribution to post treatment tuberculosis pulmonary impairment.

The aim of the study was to impart the selected nursing interventions applied to the COPD patients in medical

the mean post test score was 1861 and the difference between pre test and post test knowledge score was 5.54.
Based on the objectives of the study the findings of the level of health related quality of life and activities of
daily living among COPD patients shows that increased health related quality of life and activities of daily
living. The study shows that the COPD patients in post test were having post score1861 mean difference
5.54.standard deviation 2.35.p value (0.02). Selected nursing interventions are effective in increasing the health
related qualityof life and activities of daily living among COPD patients.. The findings of the study revealed that

The study found that in adults presenting with acute asthma exacerbations, the addition of the ketolide telithromycin to standard therapy resulted in greater improvement in asthma symptoms and lung function compared to placebo, especially in those who tested positive for atypical bacterial infections like Chlamydia pneumoniae and Mycoplasma pneumoniae. This document summarizes a case study of low salinity water injection in the Zichebashskoe oil field in Russia over 7 years. The field data was history matched using a mathematical model that simulates fines migration during waterflooding. Modeling showed a minor incremental oil recovery (<0.1%) and decrease in produced water compared to conventional injection, likely due to significant water production before low salinity injection began and injectors placed in the aquifer rather than oil zone. Further modeling of a hypothetical reservoir showed low salinity injection could improve recovery by ~10% with less water production.

It was a randomized, double-blind, placebo-controlled trial conducted in a tertiary care hospital over 2 years. Patients presenting with acute asthma exacerbation were randomized to receive standard therapy with either montelukast or placebo. The primary outcomes of lung function, duration of hospital stay, and secondary outcomes showed no significant differences between the montelukast and placebo groups. The study concluded that montelukast provided no added benefit over standard therapy alone for acute asthma exacerbation. Larger multicenter trials are still needed.
EXACERBATION OF COPD _ 11
Acute exacerbations of chronic obstructive pulmonary disease (AECOPD) are associated with high costs, morbidity, and mortality. They result in enormous healthcare expenditures and lost productivity. AECOPD episodes are also linked to accelerated long-term loss of lung function in continuous smokers. While symptoms may recover within a week, full recovery of health-related quality of life can take months. Noninvasive mechanical ventilation should be tried for patients with respiratory acidosis or increased work of breathing from AECOPD as it reduces intubation rates and improves health outcomes. Preventing and properly managing AECOPD is important for reducing its impacts.

WHO Collaborating Centre is supposed to take FOUR work-plans and the second one is – `Organization of capacity building workshops and training programs on the role, scope, practice and evidence-based use of Yoga in non-communicable diseases’.
2nd Capacity Building Workshop was conducted on the theme “Yogic Management of Cancer, Bronchial Asthma & Stroke” at Morarji Desai National Institute of Yoga, New Delhi from 28th - 30th November, 2016.
The Chief Guests for the inaugural was Dr. Sudhir Gupta, Additional Dy. Director General (NCD), Directorate General of Health Services, Govt of India and Dr Kim Sung Chol, Regional Adviser, Traditional Medicines, World Health Organization Regional Office for South-East Asia.
The capacity building workshop was organised by Dr Ishwar V Basavaraddi Director MDNIY with eminent resource persons and it aimed to train Master Trainers who would carry out the same activity in their locality /organization /concerned State and be Brand Ambassadors for propagating the role of Yoga in non-communicable diseases.

Background- The chronic obstructive pulmonary disease is a chronic inflammatory disease and a leading cause of morbidity and mortality worldwide. Smoking is the major risk factor in COPD. Smoking damages the air sacs, airway and the lining of the lungs and due to this lung have trouble moving enough air in and out making hard to breathe. Smoking may act as a trigger factor for many people who have COPD and can either cause an exacerbation or flare-up of symptoms. The present study aims to determine the association of smoking status with different stages of COPD and clinical symptoms in a North Indian population. Methods- The present study was conducted on 160 stable COPD patients in the department of Respiratory Medicine, King George Medical University, Lucknow. Results- Out of 160 patients enrolled there were 41.8% smokers, 24.3% non-smokers, and 33.7% ex-smokers. The present study found a significant association (p<0.02) of smoking status with different stages of COPD, although non-significant association (p=0.96) was observed between smoking status and clinical symptoms. Conclusion- The significant association of smoking status was observed with different stages of COPD while the non-significant association was observed with clinical symptoms in the present study in north Indian population. Smoking cessation will be helpful in reducing the progression and management of this disease in smokers. support and care networks that are most appropriate for the patient and can have quality-
of-life benefits beyond physical improvements.

among all patients with severe emphysema. This trial did, however, identify subgroups
that had a survival advantage. The best candidates for LVRS are patients with
predominantly upper-lobe disease and a low exercise capacity after pulmonary
rehabilitation. Bullectomy has not been well studied in randomized trials, but it may be
considered for patients with at least one-third of the thorax occupied by bullae.
For patients with advanced disease another therapy to consider is lung
transplantation. Lung transplant referral is indicated for younger patients with COPD that
have progressive symptoms despite maximal medical therapy, including smoking
cessation. Lung transplant for COPD has been shown to improve quality of life, but effect
on mortality has not been clearly demonstrated and is more controversial. For further
analysis of trials addressing treatment strategies in COPD, please refer to the key
references (Table 232-10).

Background: Chronic Obstructive pulmonary disease (COPD) is an increasing cause of morbidity and mortality world-wide. MMP 9 is an acute phase reactant secreted by the liver in response to infection, inflammation or tissue damage. Methods: This case-control study was conducted on 35 healthy controls and 40 COPD patients at a tertiary care hospital in north India. MMP 9 levels were measured in serum by ELISA Kit. Results: The present study showed that mean MMP 9 levels in serum was significantly higher in COPD group as compared to control group (p<0.0001) and the levels increased with the increasing severity of the disease. Conclusion: Our study confirms that MMP 9 levels were significantly higher in COPD patients as compared to controls and their levels increased with the increasing severity of the disease. Measuring MMP 9 levels in combination with other biochemical markers can be helpful in monitoring disease outcome and management of the disease. Abstract:
Context: Recent studies provide ample evidence of the benefits of yoga in various chronic disorders. Diabetes mellitus is a group of metabolic diseases characterized by chronic hyperglycemia and Sandler coined the term “Diabetic Lung" for the abnormal pulmonary function detected in diabetic patients due underlying pulmonary dysfunction. Yoga therapy may help in achieving better pulmonary function along with enhanced glycaemic control and overall health benefits.
Aim: To study the effect of adjuvant yoga therapy in diabetic lung through spirometry.
Settings and Design: Randomized control trial done as interdisciplinary collaborative work between Yoga Therapy, Pulmonary Medicine and Endocrinology departments of MGMC & RI, Sri Balaji Vidyapeeth Puducherry.
Materials and Methods: 72 patients of diabetic lung as confirmed by spirometry (<70% of expected) were randomized and divided equally into control group who received only standard medical treatment and yoga group who received yoga training thrice weekly for 4 months along with that. Yoga therapy protocol included yogic counseling, preparatory practices, Asanas or static postures, Pranayama or breathing techniques and relaxation techniques. Hathenas of the Gitananda Yoga tradition were the main practices used. Spirometry was done at the end of the study period. Data was analyzed by Student’s paired and unpaired ’t’ test as it passed normality.
Results: There was a statistically significant (p < 0.05) reduction in weight, BMI along with a significant (p < 0.01) improvement in pulmonary function (FEV1, FVC) in yoga group as compared to control group where parameters worsened over study period.
Conclusion: It is concluded from the present RCT that yoga has a definite role as an adjuvant therapy as it enhances standard medical care and hence is even more significant in routine clinical management of diabetes, improving physical condition and pulmonary function.

Effect of Inspiratory Muscle Training on Muscle Strength and
Quality of Life in Patients With Chronic Airflow Limitation:
a Randomized Controlled Trial

This pilot study assessed the effects of an Iyengar yoga program (IYP) on 25 patients with chronic lung diseases like pulmonary arterial hypertension and chronic obstructive pulmonary disease. Patients participated in a 12-week IYP involving biweekly 2-hour classes. Questionnaires assessing health-related quality of life, anxiety, depression, and functional mobility were completed at baseline and after 12 weeks. Statistically significant improvements were seen in anxiety and fatigue scores. Clinically important improvements were observed in mobility, pain, emotion, and overall health scores. Patients' journals also reported enhancements in breathing, energy, sleep, and overall well-being. The findings suggest yoga may provide benefits and warrant further exploration in a larger study.
GOLD16 – MANAGEMENT AND TREATMENT OF COPD

Homeopathy—quackery or a key to the future of medicine?
When cholera first invaded Europe in 1831, the
mortality throughout Europe was generally between
40% and 60%. To the surprise of many, mortality
rates reported by homeopathic physicians was generally
below 10%, and commonly under 4%. Let me
present two typical cholera reports, which have a
stamp of officialdom. The first one comes from the
territory of Raab in Hungary where in 1831 a
Dr Joseph Bakody treated 223 patients with mild to
severe cholera, 14 of which were in a state of collapse .
He lost a total of 8 patients, a mortality of 3.6%. A
similar situation occurred in Cincinnati in 1849. The
Board of Health issued an order calling for physicians
to report all cases of cholera. Reports of a high
mortality rate were received by the Board from the city
hospital and allopathic physicians. However, six
homeopathic physicians attracted national attention
when they reported not a single death out of their first
350 cases of cholera. Two of these homeopathic
physicians, Dr Pulte and Ehrmann would eventually
report treating 2646 cases with 35 deaths, or a
mortality rate of 1.3%. Allopaths reported fatal
outcomes in about 50% of their cases.

Harm in homeopathy: Aggravations, adverse drug events or medication errors?
This study prospectively observed 335 follow-up visits of 181 patients receiving homeopathic treatment between June 2003 and June 2004. The study aimed to assess harm from homeopathic medicines by reporting any adverse drug events. Nine adverse reactions were reported, representing 2.68% of follow-up visits. Most events were minor and transient. One case involved an allergic reaction to lactose, an excipient in the granules. The study concludes that while adverse events to homeopathic drugs do occur, they are rare and not typically severe.

Rome, 3rde5th June 2015, was the setting for the Homeopathy Research Institute’s (HRI)
second conference with the theme ‘Cutting Edge Research in Homeopathy’. Attended by
over 250 delegates from 39 countries, this event provided an intense two and a half day
programme of presentations and a forum for the sharing of ideas and the creation of international
scientific collaborations. With 35 oral presentations from leaders in the field,
the scientific calibre of the programme was high and the content diverse. This report
summarises the key themes underpinning the cutting edge data presented by the
speakers, including six key-note presentations, covering advancements in both basic
and clinical research. Given the clear commitment of the global homeopathic community
to high quality research, the resounding success of both Barcelona 2013 and
Rome 2015 HRI conferences, and the dedicated support of colleagues, the HRI moves
confidently forward towards the next biennial conference
CORE-Hom: A powerful and exhaustive database of clinical trials in homeopathy
The CORE-Hom database was created to answer the need for a reliable and publicly available
source of information in the field of clinical research in homeopathy. As of May 2014
it held 1048 entries of clinical trials, observational studies and surveys in the field of homeopathy,
including second publications and re-analyses. 352 of the trials referenced in
the database were published in peer reviewed journals, 198 of which were randomised
controlled trials. The most often used remedies were Arnica montana (n = 103) and
Traumeel (n = 40). The most studied medical conditions were respiratory tract infections
(n = 126) and traumatic injuries (n = 110). The aim of this article is to introduce
the database to the public, describing and explaining the interface, features and content
of the CORE-Hom database.
Controlled clinical studies of homeopathy
Observations about controlled clinical trials expressed by Max Haidvogl
in the book Ultra High Dilution (1994) have been appraised from a perspective two
decades later. The present commentary briefly examines changes in homeopathy
research evidence since 1994 as regards: the published number of randomised controlled
trials (RCTs), the use of individualised homeopathic intervention, the ‘proven efficacy of
homeopathy’, and the quality of the evidence.
Clinical trial of homeopathy in rheumatoid arthritis
The conclusion of the study that the effect was due to
‘consultation’ and not to the homeopathic remedy appears
to be biased for two reasons:
There was no substantial amelioration of the pathology
in any group to compare and on which to base conclusions.
The placebo effect in such deep pathology cases is superficial
and transient as the patient remains in essence with
the same frame of pathology.
Blisters and homeopathy: case reports and differential diagnosis

A short history of the development of homeopathy in India
Homeopathy was introduced in India the early 19th century. It flourished in Bengal at first,
and then spread all over India. In the beginning, the system was extensively practised by
amateurs in the civil and military services and others. Mahendra Lal Sircar was the first
Indian who became a homeopathic physician. A number of allopathic doctors started
homeopathic practice following Sircar’s lead. The ‘Calcutta Homeopathic Medical
College’, the first homeopathic medical college was established in 1881. This institution
took on a major role in popularising homeopathy in India.
In 1973, the Government of India recognised homeopathy as one of the national systems of
medicine and set up the Central Council of Homeopathy (CCH) to regulate its education
and practice. Now, only qualified registered homeopaths can practice homeopathy in
India. At present, in India, homeopathy is the third most popular method of medical treatment
after allopathy and Ayurveda. There are over 200,000 registered homeopathic doctors
currently, with approximately 12,000 more being added every year.

Cell Therapy Expansion and Challenges in Autoimmune Disease
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.

Despite this, no effective management is available for most of the patients. Therefore, the present study was undertaken to evaluate the impact of homeop- athy on pulmonary, functional and quality of life status of patients with pulmonary tuberculosis who have completed treatment. Methods Patients who were cured and had completed anti-tubercu- losis treatment within a period of 5 years were enrolled in a randomized double-blind placebo-controlled trial. Indivi- dualised homeopathy treatment was given to 61 patients and identical placebo to 57 patients. Symptomatic changes, pulmonary function tests, and health related quality of life were assessed prior to treatment, after 6 months of inter- vention, and followed up for a year after completing the intervention. Results Significant improvement was observed with the homeop- athy treatment in FEV1 (p<0.001), forced vital capacity (p<0.001), and FEV1/FVC ratio (p=0.002). Symptom scores for cough and breathlessness were significantly lower with homeopathy than with placebo (p<0.001). At the end of treatment, patients on homeopathy had increased body weight (p<0.0001), and better quality of life (p<0.05) compared with placebo (p=0.003). Benefits were maintained in the homeopathy group after a year whereas symptoms (p<0.01) and impact score (p<0.001) deteriorated in placebo. Physicians visits were reduced in the homeopathy group by 58.0% (p =0.002) compared to placebo (p<0.0001). Conclusion Homeopathy is effective in improving lung capacity and health status. Benefits remain evident after a year. This suggests that homeopathy could make an important contribution to post treatment tuberculosis pulmonary impairment.
