Purpose
Previous studies show that treated and cured pulmonary
tuberculosis patients do suffer from pulmonary impair-
ment, lower health related quality of life, disability and
long term morbidity, thus responsible for a majority of the
disease burden. Despite this, no effective management is
available for most of the patients. Therefore, the present
study was undertaken to evaluate the impact of homeop-
athy on pulmonary, functional and quality of life status of
patients with pulmonary tuberculosis who have completed
treatment.
Methods
Patients who were cured and had completed anti-tubercu-
losis treatment within a period of 5 years were enrolled in
a randomized double-blind placebo-controlled trial. Indivi-
dualised homeopathy treatment was given to 61 patients
and identical placebo to 57 patients. Symptomatic changes,
pulmonary function tests, and health related quality of life
were assessed prior to treatment, after 6 months of inter-
vention, and followed up for a year after completing the
intervention.
Results
Significant improvement was observed with the homeop-
athy treatment in FEV1 (p<0.001), forced vital capacity
(p<0.001), and FEV1/FVC ratio (p=0.002). Symptom
scores for cough and breathlessness were significantly
lower with homeopathy than with placebo (p<0.001). At
the end of treatment, patients on homeopathy had
increased body weight (p<0.0001), and better quality of
life (p<0.05) compared with placebo (p=0.003). Benefits
were maintained in the homeopathy group after a year
whereas symptoms (p<0.01) and impact score (p<0.001)
deteriorated in placebo. Physicians visits were reduced in
the homeopathy group by 58.0% (p =0.002) compared to
placebo (p<0.0001).
Conclusion
Homeopathy is effective in improving lung capacity and
health status. Benefits remain evident after a year. This
suggests that homeopathy could make an important
contribution to post treatment tuberculosis pulmonary
impairment.

A study to assess the effectiveness of selected nursing interventions on health related quality of life and activities of daily living among COPD patients in selected tertiary hospital, Chennai, Tamilnadu, India. The aim of the study was to impart the selected nursing interventions applied to the COPD patients in medical and pulmonary wards. The conceptual framework used in this study was Widenbach’s Helping art Theory. An Evaluative approach was used for the present study. Using purposive sampling technique 50 samples were selected from Chettinad Hospital and Research Institute, Tamil Nadu, India. The tool used was self administered questionnaire. The collected data was analyzed using descriptive and inferential statistics. The findings of the study revealed a significantly increase in the COPD patients health related quality of life and activities of daily living after given the selected nursing interventions. The mean pre test score was 1867 and the mean post test score was 1861 and the difference between pre test and post test knowledge score was 5.54. Based on the objectives of the study the findings of the level of health related quality of life and activities of daily living among COPD patients shows that increased health related quality of life and activities of daily living. The study shows that the COPD patients in post test were having post score1861 mean difference 5.54.standard deviation 2.35.p value (0.02). Selected nursing interventions are effective in increasing the health related qualityof life and activities of daily living among COPD patients.. Morarji Desai National Institute of Yoga (MDNIY), New Delhi under Ministry of AYUSH, GOI was designated as a World Health Organisation Collaborating Centre for Traditional Medicine (Yoga) in April, 2013. WHO Collaborating Centre is supposed to take FOUR work-plans and the second one is – `Organization of capacity building workshops and training programs on the role, scope, practice and evidence-based use of Yoga in non-communicable diseases’. 2nd Capacity Building Workshop was conducted on the theme “Yogic Management of Cancer, Bronchial Asthma & Stroke” at Morarji Desai National Institute of Yoga, New Delhi from 28th - 30th November, 2016. The Chief Guests for the inaugural was Dr. Sudhir Gupta, Additional Dy. Director General (NCD), Directorate General of Health Services, Govt of India and Dr Kim Sung Chol, Regional Adviser, Traditional Medicines, World Health Organization Regional Office for South-East Asia. The capacity building workshop was organised by Dr Ishwar V Basavaraddi Director MDNIY with eminent resource persons and it aimed to train Master Trainers who would carry out the same activity in their locality /organization /concerned State and be Brand Ambassadors for propagating the role of Yoga in non-communicable diseases. Dr Ananda was invited as a Resource Person for the workshop for orienting and enlightening the participants on the role, scope, practice and evidence-based use of Yoga in non-communicable diseases with special emphasis on Bronchial Asthma.

Association of smoking status with COPD in north indian population

Background- The chronic obstructive pulmonary disease is a chronic inflammatory disease and a leading cause of morbidity and mortality worldwide. Smoking is the major risk factor in COPD. Smoking damages the air sacs, airway and the lining of the lungs and due to this lung have trouble moving enough air in and out making hard to breathe. Smoking may act as a trigger factor for many people who have COPD and can either cause an exacerbation or flare-up of symptoms. The present study aims to determine the association of smoking status with different stages of COPD and clinical symptoms in a North Indian population. Methods- The present study was conducted on 160 stable COPD patients in the department of Respiratory Medicine, King George Medical University, Lucknow. Results- Out of 160 patients enrolled there were 41.8% smokers, 24.3% non-smokers, and 33.7% ex-smokers. The present study found a significant association (p<0.02) of smoking status with different stages of COPD, although non-significant association (p=0.96) was observed between smoking status and clinical symptoms. Conclusion- The significant association of smoking status was observed with different stages of COPD while the non-significant association was observed with clinical symptoms in the present study in north Indian population. Patient education regarding the ability to recognize the symptoms of an exacerbation should be emphasized. PULMONARY REHABILITATION Pulmonary rehabilitation is an important part of outpatient COPD care after an admission for AECOPD, and should be considered at the time of discharge for all patients with chronic lung disease with the goal of alleviating symptoms and optimizing functional capacity. Evidence supports that entering pulmonary rehabilitation within 10 days of hospital discharge is safe. Furthermore, patients enrolled in early pulmonary rehabilitation experienced improved exercise tolerance and health status at 3 months. Beyond functional capacity, pulmonary rehabilitation programs often focus on establishing social support and care networks that are most appropriate for the patient and can have qualityof-life benefits beyond physical improvements. PRACTICE POINT Evidence supports that entering pulmonary rehabilitation within 10 days of hospital discharge is safe, and patients enrolled in early pulmonary rehabilitation experience improved exercise tolerance and health status at 3 months. SURGICAL TREATMENT OPTIONS AND TRANSPLANT EVALUATION Surgical treatment options for COPD include lung volume reduction surgery (LVRS), bullectomy, lung transplantation and investigational approaches. LVRS involves bilateral removal of 25% to 30% of total lung volume. The National Emphysema Treatment Trial, published in 2003, demonstrated that LVRS improved exercise capacity but not survival among all patients with severe emphysema. This trial did, however, identify subgroups that had a survival advantage. The best candidates for LVRS are patients with predominantly upper-lobe disease and a low exercise capacity after pulmonary rehabilitation. Bullectomy has not been well studied in randomized trials, but it may be considered for patients with at least one-third of the thorax occupied by bullae. For patients with advanced disease another therapy to consider is lung transplantation. Lung transplant referral is indicated for younger patients with COPD that have progressive symptoms despite maximal medical therapy, including smoking cessation. Lung transplant for COPD has been shown to improve quality of life, but effect on mortality has not been clearly demonstrated and is more controversial. Background: Chronic Obstructive pulmonary disease (COPD) is an increasing cause of morbidity and mortality world-wide. MMP 9 is an acute phase reactant secreted by the liver in response to infection, inflammation or tissue damage. Methods: This case-control study was conducted on 35 healthy controls and 40 COPD patients at a tertiary care hospital in north India. MMP 9 levels were measured in serum by ELISA Kit. Results: The present study showed that mean MMP 9 levels in serum was significantly higher in COPD group as compared to control group (p<0.0001) and the levels increased with the increasing severity of the disease. Conclusion: Our study confirms that MMP 9 levels were significantly higher in COPD patients as compared to controls and their levels increased with the increasing severity of the disease. Measuring MMP 9 levels in combination with other biochemical markers can be helpful in monitoring disease outcome and management of the disease. Diabetes mellitus is a group of metabolic diseases characterized by chronic hyperglycemia and Sandler coined the term “Diabetic Lung" for the abnormal pulmonary function detected in diabetic patients due underlying pulmonary dysfunction. Yoga therapy may help in achieving better pulmonary function along with enhanced glycaemic control and overall health benefits. Aim: To study the effect of adjuvant yoga therapy in diabetic lung through spirometry. Settings and Design: Randomized control trial done as interdisciplinary collaborative work between Yoga Therapy, Pulmonary Medicine and Endocrinology departments of MGMC & RI, Sri Balaji Vidyapeeth Puducherry. Materials and Methods: 72 patients of diabetic lung as confirmed by spirometry (<70% of expected) were randomized and divided equally into control group who received only standard medical treatment and yoga group who received yoga training thrice weekly for 4 months along with that. Yoga therapy protocol included yogic counseling, preparatory practices, Asanas or static postures, Pranayama or breathing techniques and relaxation techniques. Hathenas of the Gitananda Yoga tradition were the main practices used. Spirometry was done at the end of the study period. Data was analyzed by Student’s paired and unpaired ’t’ test as it passed normality. Results: There was a statistically significant (p < 0.05) reduction in weight, BMI along with a significant (p < 0.01) improvement in pulmonary function (FEV1, FVC) in yoga group as compared to control group where parameters worsened over study period. Conclusion: It is concluded from the present RCT that yoga has a definite role as an adjuvant therapy as it enhances standard medical care and hence is even more significant in routine clinical management of diabetes, improving physical condition and pulmonary function. Introduction: This assignment discusses the anticipatory and alternative approaches for long-term conditions from different aspects. Patch one – learning outcome one: self-management strategies. Approximately three million people are believed to have chronic obstructive pulmonary disease (COPD) in England, with around 30,000 lives lost as a result each year making it the fifth biggest cause of mortality and morbidity in the UK (Stewart et al., 2011; Wilson et al., 2015). Self-management can play a vital part in the management of COPD and can also give the patient a greater sense of autonomy, thereby improving their quality of life (Robbins et al., 2013; Sallnow, Kumar, & Kellehear, 2013). Self-management plan should be comprehensive, individualized and tailored to suit each patient as they have varying symptoms and needs that require individualized plan in order to achieve optimum health outcome. For that reason, there should be a great cooperation, education and person-centered support from specialized health professionals who are COPD knowledgeable and properly trained (Cornforth,2013 ). Based on that, clinician’s goal should be directed towards working with COPD patients collaboratively to discuss and negotiate their tolerated healthy behaviors, the disease nature, its management and encouragement of possible life style modifications, for instance, more exercise, healthy eating, correct inhaler technique and basically smoking cessation( Cornforth,2013). Patients who have early-stage COPD may not display their symptoms in a way that they recognise (Robbins et al., 2013; Sallnow, Kumar, & Kellehear, 2013); but as COPD worsens there is an increase seen in wheezing, coughing, sputum production, and dyspnoea (Sharp, Moran, & Kuhn, 2013). In some patients these symptoms may become so severe that long-term oxygen therapy is needed (Sharp, Moran, & Kuhn, 2013). It is important that patients are able to recognise their own symptoms and thus any changes in them, which can be done by keeping a daily record (Stewart et al., 2011). It is important that patients take their medications as prescribed, even during periods of feeling well (Robbins et al., 2013). Continuous use of medication can help prevent exacerbations; but it is important to check for any interactions that may occur with medicines which are available without prescription (Sallnow, Kumar, & Kellehear, 2013). In addition to regular medications, it is also important that patients receive annual vaccinations such as the flu jab and an anti-pneumococcal vaccination which protects against serious infections caused by pneumococcal bacteria (Sallnow, Kumar, & Kellehear, 2013). When cholera first invaded Europe in 1831, the mortality throughout Europe was generally between 40% and 60%. To the surprise of many, mortality rates reported by homeopathic physicians was generally below 10%, and commonly under 4%. Let me present two typical cholera reports, which have a stamp of officialdom. The first one comes from the territory of Raab in Hungary where in 1831 a Dr Joseph Bakody treated 223 patients with mild to severe cholera, 14 of which were in a state of collapse . He lost a total of 8 patients, a mortality of 3.6%. A similar situation occurred in Cincinnati in 1849. The Board of Health issued an order calling for physicians to report all cases of cholera. Reports of a high mortality rate were received by the Board from the city hospital and allopathic physicians. However, six homeopathic physicians attracted national attention when they reported not a single death out of their first 350 cases of cholera. Two of these homeopathic physicians, Dr Pulte and Ehrmann would eventually report treating 2646 cases with 35 deaths, or a mortality rate of 1.3%. Allopaths reported fatal outcomes in about 50% of their cases.

CORE-Hom: A powerful and exhaustive database of clinical trials in homeopathy

The CORE-Hom database was created to answer the need for a reliable and publicly available source of information in the field of clinical research in homeopathy. As of May 2014 it held 1048 entries of clinical trials, observational studies and surveys in the field of homeopathy, including second publications and re-analyses. 352 of the trials referenced in the database were published in peer reviewed journals, 198 of which were randomised controlled trials. The most often used remedies were Arnica montana (n = 103) and Traumeel (n = 40). The most studied medical conditions were respiratory tract infections (n = 126) and traumatic injuries (n = 110). The aim of this article is to introduce the database to the public, describing and explaining the interface, features and content of the CORE-Hom database.

Observations about controlled clinical trials expressed by Max Haidvogl in the book Ultra High Dilution (1994) have been appraised from a perspective two decades later. The present commentary briefly examines changes in homeopathy research evidence since 1994 as regards: the published number of randomised controlled trials (RCTs), the use of individualised homeopathic intervention, the ‘proven efficacy of homeopathy’, and the quality of the evidence.

This article reports cases of blistering diseases successfully treated with homeopathic medicines, which represent an alternative for the treatment of such disorders.

Homeopathy was introduced in India the early 19th century. It flourished in Bengal at first, and then spread all over India. In the beginning, the system was extensively practised by amateurs in the civil and military services and others. Mahendra Lal Sircar was the first Indian who became a homeopathic physician. A number of allopathic doctors started homeopathic practice following Sircar’s lead. The ‘Calcutta Homeopathic Medical College’, the first homeopathic medical college was established in 1881. This institution took on a major role in popularising homeopathy in India. In 1973, the Government of India recognised homeopathy as one of the national systems of medicine and set up the Central Council of Homeopathy (CCH) to regulate its education and practice. Now, only qualified registered homeopaths can practice homeopathy in India. At present, in India, homeopathy is the third most popular method of medical treatment after allopathy and Ayurveda. There are over 200,000 registered homeopathic doctors currently, with approximately 12,000 more being added every year.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0). CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

This slide deck presented by Dr. Kami Maddocks, Professor-Clinical in the Division of Hematology and Associate Division Director for Ambulatory Operations The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma. STATEMENT OF NEED Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL. Long term evaluation of homeopathy on post treatment impairment of pulmonary tuberculosis S Sharma1* , N Sharma2 From International Research Congress on Integrative Medicine and Health 2012 Portland, Oregon, USA. 15-18 May 2012 Purpose Previous studies show that treated and cured pulmonary tuberculosis patients do suffer from pulmonary impairment, lower health related quality of life, disability and long term morbidity, thus responsible for a majority of the disease burden. Despite this, no effective management is available for most of the patients. Therefore, the present study was undertaken to evaluate the impact of homeopathy on pulmonary, functional and quality of life status of patients with pulmonary tuberculosis who have completed treatment. Methods Patients who were cured and had completed anti-tuberculosis treatment within a period of 5 years were enrolled in a randomized double-blind placebo-controlled trial. Indivi- dualised homeopathy treatment was given to 61 patients and identical placebo to 57 patients. Symptomatic changes, pulmonary function tests, and health related quality of life were assessed prior to treatment, after 6 months of intervention, and followed up for a year after completing the intervention. Results Significant improvement was observed with the homeopathy treatment in FEV1 (p<0.001), forced vital capacity (p<0.001), and FEV1/FVC ratio (p=0.002). Symptom scores for cough and breathlessness were significantly lower with homeopathy than with placebo (p<0.001). At the end of treatment, patients on homeopathy had increased body weight (p<0.0001), and better quality of life (p<0.05) compared with placebo (p=0.003). Benefits were maintained in the homeopathy group after a year whereas symptoms (p<0.01) and impact score (p<0.001) deteriorated in placebo. Physicians visits were reduced in the homeopathy group by 58.0% (p =0.002) compared to placebo (p<0.0001). Conclusion Homeopathy is effective in improving lung capacity and health status. Benefits remain evident after a year. This suggests that homeopathy could make an important contribution to post treatment tuberculosis pulmonary impairment.
