Expo 15 returned to Manchester and was hosted once again by NHS England. Around 5000 people a day from health and care, the voluntary sector, local government, and industry joined together at Manchester Central Convention Centre for two packed days of speakers, workshops, exhibitions and professional development.
This year, Expo was more relevant and engaging than ever before, happening within the first 100 days of the new Government, and almost 12 months after the publication of the NHS Five Year Forward View. It was also a great opportunity to check on and learn from the progress of Greater Manchester as the area prepares to take over a £6 billion devolved health and social care budget, pledging to integrate hospital, community, primary and social care and vastly improve health and well-being.

As part of this, Sweden introduced mandatory patient choice of primary care providers in 2010. Both public and private organizations can now provide primary care. Observations show that access and satisfaction have increased, though medical quality data is lacking. Lower socioeconomic groups have increased use of services more. Around 30-40% of primary care is now privately delivered, including 15-20% by international for-profit companies. Opinions among GPs on the changes remain mixed.
Community Involvement and Commissioning
PPI (patient and public involvement) is imperative for successful commissioning for several reasons: it saves money and ensures pathways are used; it increases the moral and democratic legitimacy of the NHS; and it leads to safer and better designed services. There are several governance options for consortiums including democratic elections to boards and having lay people and community groups represented. Community development and asset-based approaches can improve PPI, health outcomes, and tackle inequalities by strengthening communities and individuals. The 7 step model of community organizing has led to rapid changes in commissioning and health improvements within 5 years.
Community Involvement in Commissioning
PPI (patient and public involvement) is imperative for successful commissioning for several reasons: it saves money and ensures pathways are used; it increases the moral and democratic legitimacy of the NHS; and it leads to safer and better designed services. Effective PPI requires democratic and participatory involvement from both individuals and community groups to influence priorities, service development, and quality standards. Cell Therapy Expansion and Challenges in Autoimmune Disease
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
