This document contains 3 agreements related to a student's on-the-job training (OJT) program: 1) A parent-school-industry agreement authorizing a student to undergo OJT at a specific company and waiving responsibility for any accidents. 2) A job training memorandum of agreement outlining the terms of the OJT between the university, student trainee, and host company, including training activities and evaluations. 3) A waiver signed by the student acknowledging the company's rules and waiving claims against the company and university for injuries due to their own negligence during training.

establishment concerned will not be held liable for any accident or injury arising from
circumstances due to the fault of the student while within its compound. Further, the
company will see to it that the above named student be given an orientation concerning
the company’s safety program.

2. COLLEGE OF BUSINESS ADMINISTRATION AND ACCOUNTANCY
THE JOB TRAINING MEMORANDUM AGREEMENT
This Memorandum of Agreement made and executed between and among:
Pamantasan ng Lungsod ng Marikina, a non-stock, non-profit educational
institution, duly organized and existing under Philippine Laws with office address at
Brazil Street, Concepcion Uno, Marikina City represented herein by ________________
Dean, College of Business Administration and Accountancy, hereinafter referred to as
the UNIVERSITY.
_____________________, Filipino, of legal age, with residence at _______________,
and a _____ year student enrolled at the Pamantasan ng Lungsod ng Marikina, College
of Business Administration and Accountancy, hereinafter referred to as the TRAINEE.

Witnesseth:
The parties hereby bind themselves to undertake this Memorandum of Agreement
for the purpose of complying with the University’s on-the-job training requirement for
student-trainees under the following terms and conditions:
1. The COMPANY hereby agrees to provide an “on the job training” to the
TRAINEE;
2. The training program will be for a minimum period of _____ hours. Such period
may be extended subject to the mutual agreement by the COMPANY and the
TRAINEE;
3. The relationship between the COMPANY and the TRAINEE shall not be that of
an employer - employee but only that of a trainer-trainee;
4. The TRAINEE, during the above mentioned period shall be exposed to various
forms of labor such as but not limited to : ______________________________
in order to gain experience in all operation and duties as outlined in the job
activity sheet. The officer so assigned to train and monitor the TRAINEE shall
make an evaluation on his/her performance at the end of any activity/ies as
provided for on the form;

5. The COMPANY, upon its own discretion, may provide the TRAINEE a
reasonable compensation as it may deem fit;
6. The TRAINEE shall regularly notify the UNIVERSITY through its University
Practicum Coordinator as well as the COMPANY in case of unavoidable
absences from the training area;
7. The COMPANY agrees to permit the UNIVERSITY through its Practicum
Coordinator, to make a personal assessment of the TRAINEE while in the
premises of the company within reasonable hours upon prior notice;
8. The TRAINEE agrees further to observe the rules and regulations of the
COMPANY and abide with all the terms and conditions of this Memorandum
Agreement.

WAIVER

enrolled under Business Practicum at the Pamantasan ng Lungsod ng Marikina – College
of Business Administration and Accountancy with the assistance and cooperation of
______________________ , hereby freely and voluntarily assume the following duties:
1. That I recognize the authority of my cooperating Company and submit myself
to all its rules and regulations;
2. That I waive any claim against the cooperating Company and the
Pamantasan ng Lungsod ng Marikina – College of Business Administration
and Accountancy for any injury that I may suffer due to my own negligence
in the performance of my duties/functions during the course of my training.
