Class Trip 2 Waiver Form

This waiver form is for a student participating in a field trip from North Jakarta International School to Carita. The parent acknowledges that their child is voluntarily participating in the trip and assumes all risks. They understand that school rules will be followed and their child will obey all supervisor instructions for safety. The parent agrees not to hold the school, employees, or Board of Governors liable for any expenses, injuries, or accidents not due to negligence of the school's employees during the field trip.
