A 40-year-old male presented with intermittent generalized headaches. Differential diagnoses included Rathke's cleft cyst, dermoid cyst, colloid cyst, and epidermoid cyst, craniopharyngioma. Further examination revealed a colloid cyst of the third ventricle, which can cause intermittent headaches or sudden death. Gross examination showed gray pultaceous material within the cyst that was soft like cartilage. Microscopic examination revealed various epithelial cell types lining the cyst.
