INCIDENT DETAILS: Lifting “LED” Wall screen on elevated level

Near Miss Description:
Time: 10:05AM
Location/Plant/Equipment:Level-4th floor
(Explain What Happened _ Use attachments if needed) Workers were lifting the
LED by using the electric winch .The LED screen failed due to the clamp was not properly fasten with
the LED. The height was around four meters.
ESTIMATE OF LOSS POTENTIAL

from engineer.
INJURIES: No injury
PROPERTY / EQUIPMENT/DAMAGE: Waiting report from engineer and QC-dept.
ENVIRONMENTAL DAMAGE: No.

Lack of supervision.

The workers should check the hooks are properly tight .
No worker should stay under the load during lifting.
