Understanding and managing costs and risks are core administrative tasks. Regardless of the mission of the health organization, leaders must consider cost control a priority. In addition, they must be able to make decisions even when important information is incomplete or missing. When leaders do not have all the necessary data, they must appraise the risk to determine if it is worth taking and what the best strategy for dealing with it might be. In light of this, insurance is all about managing risk. It is important for leaders to understand how it is structured in both private and public settings.
This week we will be determining the risk and assessing the cost of a proposed venture. You will have the opportunity to take on the role of the presenter as well as that of the reviewer, which will allow you to experience two professional perspectives on the same problem. You will also be delving into the implications of accessibility and distribution through the lens of health insurance.

Imagine you are now the Secretary of Health and Human Services; you have a magic wand and you can
create the perfect healthcare system. What components would it have? Would it include:
1. improving health outcomes for individuals, families and communities,
2. defending your population against threats to their health,
3. protecting your population against financial the consequences of bad health,
4. providing access to all with equality and no disparity, and
5. making it possible for people to make decisions in their own plans of care as well as have input into
the decisions that affect your country’s overall health system?
If you answered yes to these components, your definition matches the World Health Organization’s
Components of a Healthcare System (2010).
How This Course & Content Have Real-Word Application
We are witness to history and are living in one of the most active times in our country’s history for healthcare
reform. In 1966, the Medicare Act was signed into law by President Johnson, the most significant piece of
healthcare legislation in our country to that point. Fast forward from 1966 to 2010 and the passing of the
Affordable Care Act, which arguably is the second most impactful piece of legislation on U.S. health care
since the Medicare Act.
Medicare has grown significantly since 1966 and is now about 14% of our national budget, covering 47 million
Americans (Kaiser Family Foundation, 2015). Government health plans (Medicare, Medicaid, Tri-Care,
Veteran’s Administration) are growing and are on pace to insure more lives in the near future than lives
covered by commercial plans (Cigna, United, Blue Cross, etc.)

A 72 year old woman was admitted to the Neurological Intensive Care Unit following a cerebral hemorrhage which left her with severe brain damage and ventilator dependent. One year before this event, the patient and her husband had drawn up "living wills" with an attorney. She was diagnosed by her treating physician as being in a permanent unconscious condition. The patient's living will specified that the patient did not want ventilator support or other artificial life support in the event of a permanent unconscious condition or terminal condition.
The patient's husband is her legal next of kin and the person with surrogate decision-making authority. When the living will was discussed with him, he insisted that the patient had not intended for the document to be used in a situation like the present one. Further discussion with him revealed that he understood that the patient would not be able to recover any meaningful brain function but he argued that the living will did not apply because her condition was not imminently terminal. He further indicated that he did not consider his wife to be in a permanent unconscious condition. The immediate family members (the couple’s adult children) disagreed with their father’s refusal to withdraw life support.
The treatment team allowed a week to pass to allow the husband more time to be supported in his grief and to appreciate the gravity of his wife’s situation. Nevertheless, at the end of this time, the husband was unwilling to authorize withdrawal of life support measures consistent with the patient's wishes as expressed in her living will.

24. Short-Term-Work Memory
My biggest take away this week was how restricted our short–term/working memory really is. We can only hold 4 to 7 distinct memories at one
time in the short–term memory (as the first video stated). When I took the memory quiz my memory score was 76%. I think this is a decent
score, but if I focused better I could have gotten a better score. I think this score is relatively average or maybe even above average hopefully! I
consider myself to have a pretty good memory overall and enjoyed the quiz. I believe I am generally good with names and faces, as well as
recalling memories from my childhood. Also, I am good at memorization and recalling random facts from a long time ago. I can recall many
memories and fun times I have had in the past easily
