This document discusses the issue of fragmented health data stored across different systems that creates "silos of information". It proposes empowering consumers to aggregate their health data from various sources and systems to gain a unified view of their health information and history, in order to enable better health outcomes and experiences. The fragmentation of data is seen as an inefficiency that centralized consumer access to all their records could help address.
