What is Teletriage?
• Teletriage provides remote clinical assessment, advice, and
onward referral for Care Home residents if they become
unwell.
• Teletriage uses observations taken by care home staff, and
a visual assessment of the patient via skype to provide
clinical advice and management for patients within the care
home
• Fully rolled out to all Older People’s Care Homes in Wirral
in July 2017

What is Teletriage?
• Teletriage is available 24 hours a day, 7 days per week –
staffed by nurse practitioners 8am-8pm, and GP OOH
overnight.
• Teletriage is provided by Wirral Community NHS
Foundation Trust, and has works closely with Community
Geriatricians, End of Life Team and other teams.
• Teletriage is jointly commissioned by NHS Wirral CCG and
Wirral Council, with capital funding provided by the
Innovation Agency to implement the service
