Lesson 1: Introduction to Epidemiology
In fact, epidemiology is often described as the basic science of public health, and for good reason. First, epidemiology is a quantitative discipline that relies on a working knowledge of probability, statistics, and sound research methods. Second, epidemiology is a method of causal reasoning based on developing and testing hypotheses grounded in such scientific fields as biology, behavioral sciences, physics, and ergonomics to explain health-related behaviors, states, and events. However, epidemiology is not just a research activity but an integral component of public health, providing the foundation for directing practical and appropriate public health action based on this science and causal reasoning.(2)
A. Distribution
Epidemiology is concerned with the frequency and pattern of health events in a population:
a. Frequency refers not only to the number of health events such as the number of cases of meningitis or diabetes in a population, but also to the relationship of that number to the size of the population. The resulting rate allows epidemiologists to compare disease occurrence across different populations.
b. Pattern refers to the occurrence of health-related events by time, place, and person. Time patterns may be annual, seasonal, weekly, daily, hourly, weekday versus weekend, or any other breakdown of time that may influence disease or injury occurrence. Place patterns include geographic variation, urban/rural differences, and location of work sites or schools. Personal characteristics include demographic factors which may be related to risk of illness, injury, or disability such as age, sex, marital status, and socioeconomic status, as well as behaviors and environmental exposures.
Characterizing health events by time, place, and person are activities of descriptive epidemiology, discussed in more detail later in this lesson.
B. Determinants
Determinant: any factor, whether event, characteristic, or other definable entity, that brings about a change in a health condition or other defined characteristic.
Epidemiology is also used to search for determinants, which are the causes and other factors that influence the occurrence of disease and other health-related events. Epidemiologists assume that illness does not occur randomly in a population, but happens only when the right accumulation of risk factors or determinants exists in an individual. To search for these determinants, epidemiologists use analytic epidemiology or epidemiologic studies to provide the “Why” and “How” of such events. They assess whether groups with different rates of disease differ in their demographic characteristics, genetic or immunologic make-up, behaviors, environmental exposures, or other so-called potential risk factors. Ideally, the findings provide sufficient evidence to direct prompt and effective publ.

A Registered Dietitian’s Perspective

Abstract
This paper investigates the scientific, economic, societal and ethical need of approaching the decline in bee populations as a public health issue from a registered dietitian’s perspective. Bees provide essential pollination services to crops that have been linked with decreased chronic diseases. Their decline in population could lead to shortages of produce and a continued increase in unhealthy food environments in America.
The Pubmed and Florida International University data bases were used to find academic articles that fit specific criteria. The search terms used included decline in bee populations, biodiversity, agriculture, micronutrients, chronic disease and public health. The population focus was Americans because these individuals are at an increased risk of chronic disease and higher healthcare costs due to unhealthy food environments.
The scientific and economical perspective suggest that production and availability of fruits and vegetables is essential for agricultural revenue as well as decreasing medical costs for chronic diseases. The cultural and ethical perspectives suggest that the standard American diet is the driving force of the decline in bee populations. Demand for processed foods and animal products has supported the expansion of monocultured crops as well as market and political control by large agricultural industry. These agricultural practices provide nutritionally inadequate diets for both bees and humans and work to derail environmental policy. The profession of dietetics has recognized its role in preventing chronic disease and supporting environmental stewardship - a fundamental component in protecting the pollinators responsible for producing the foods that fight chronic disease.
Therefore, a preventative approach to the decline in bee populations would have environmental, medical and economic advantages. Protecting bee populations through education, research and compressive policy change promotes biodiversity, continued profitability of nutrient dense produce and decreases medical costs.
Conclusion
Complex economic and cultural shifts have led to changes in food environments for both bees and Americans. The consequences of these shifts include environmental destruction, a decline in bee populations, potential decrease of disease fighting crops and the creation of an agriculture oligarchy. The solution to such an interrelated and far reaching problem will require cross collaboration between policy makers and several fields of science to create legislative changes and prevention programs.

Mr. M. was a 77-year-old decisionally incapacitated
long-term nursing home resident with chronic schizo-
phrenia who was admitted to the hospital with a
bacterial pneumonia. His past medical history was
notable for deteriorating functional status over the
past 2-3 years, urinary retention requiring chronic
indwelling bladder catheterization, and two recent
hospitalizations for urinary tract infections leading
to sepsis. He developed respiratory failure soon after
admission and was intubated and placed on mechani-
cal ventilation. Follow-up studies suggested worsen-
ing pneumonia and acute respiratory distress syn-
drome (ARDS), as well as worsening kidney function.
The patient was unable to participate in any decision
making. His guardian requested that cardiopulmo-
nary resuscitation and all other intensive care be pro-
vided if necessary, including dialysis should Mr. M.’s
kidney failure continue to worsen. After five days of
mechanical ventilation, the patient was weaned from
the ventilator and extubated. The palliative care ser-
vice was consulted following the extubation; his criti-
cal care team questioned whether it would be appro-
priate to re-intubate the patient if he again developed
respiratory failure. The palliative care team contacted
Mr. M.’s brother, his only living relative, who felt the
patient’s quality of life was poor and believed the
patient would not want aggressive medical care. The
staff at his nursing home was contacted, as well as
the patient’s mental health case manager, who had all
known Mr. M. for many years. All concurred with his
brother’s assessment. Additionally, the nursing home
staff said that Mr. M. would not be able to return there
if the plan was to continue more intensive medical
management of his worsening health conditions. Hos-
pice care was discussed with these parties, and it was
thought that choosing hospice would best represent
the patient’s wishes under the circumstances. The pal-
liative care team contacted his guardian and explained
the patient’s medical situation and its implications
for his ongoing care (including the need for physical
restraints, loss of stable nursing home placement, and
confinement to the acute care hospital environment
for the duration of his acute illness). Based on this new

Public health threats, emergencies, and disasters are as diverse as they are devastating to a population. Infections such as pandemic influenza outbreaks cause severe illness and often death in susceptible populations like the very young or old. Disasters such as these require rapid response to vaccinate susceptible populations and educate the public on how to protect themselves from contracting the disease.
Natural disasters like tsunamis, hurricanes, sudden heavy snowstorms or blizzards, and flooding also jeopardize health. These types of disasters create environmental health concerns through the spread of toxic waste, increased water-borne infections and insect-borne disease, as well as loss of power or the inability to navigate roadways during floods or blizzards. In addition, natural disasters create environmental concerns such as low food supply, destruction of crops and livestock, and lack of clean water. Natural catastrophes like these are predicted to increase in frequency and severity as long as climate change with increasing global temperatures continues unchecked through slow reaction or frank inaction by governments.
Organizations across the globe are developing strategies for decreasing their carbon footprint in order to reduce pollution that contributes to climate change. Because no one is immune from this hazard, nurses must have a grasp of the growing concern for the impact of climate change and its associated implications for public health worldwide. It is important for nurses to learn how to increase their leadership capabilities to affect local, state, and national outcomes.
This week, you will consider the role of the nurse in preventing illnesses due to environmental issues. Additionally, you will discuss the role of nurses in developing health promotion and health protection strategies to address global climate change health concerns.

A. Distribution
Epidemiology is concerned with the frequency and pattern of health events in a population:
a. Frequency refers not only to the number of health events such as the number of cases of meningitis or diabetes in a population, but also to the relationship of that number to the size of the population. The resulting rate allows epidemiologists to compare disease occurrence across different populations.
b. Pattern refers to the occurrence of health-related events by time, place, and person. Time patterns may be annual, seasonal, weekly, daily, hourly, weekday versus weekend, or any other breakdown of time that may influence disease or injury occurrence. Place patterns include geographic variation, urban/rural differences, and location of work sites or schools. Personal characteristics include demographic factors which may be related to risk of illness, injury, or disability such as age, sex, marital status, and socioeconomic status, as well as behaviors and environmental exposures.
Characterizing health events by time, place, and person are activities of descriptive epidemiology, discussed in more detail later in this lesson.
B. Determinants
Determinant: any factor, whether event, characteristic, or other definable entity, that brings about a change in a health condition or other defined characteristic.
Epidemiology is also used to search for determinants, which are the causes and other factors that influence the occurrence of disease and other health-related events. Epidemiologists assume that illness does not occur randomly in a population, but happens only when the right accumulation of risk factors or determinants exists in an individual. To search for these determinants, epidemiologists use analytic epidemiology or epidemiologic studies to provide the “Why” and “How” of such events. They assess whether groups with different rates of disease differ in their demographic characteristics, genetic or immunologic make-up, behaviors, environmental exposures, or other so-called potential risk factors. Ideally, the findings provide sufficient evidence to direct prompt and effective publ.

Abstract
This paper investigates the scientific, economic, societal and ethical need of approaching the decline in bee populations as a public health issue from a registered dietitian’s perspective. Bees provide essential pollination services to crops that have been linked with decreased chronic diseases. Their decline in population could lead to shortages of produce and a continued increase in unhealthy food environments in America.
The Pubmed and Florida International University data bases were used to find academic articles that fit specific criteria. The search terms used included decline in bee populations, biodiversity, agriculture, micronutrients, chronic disease and public health. The population focus was Americans because these individuals are at an increased risk of chronic disease and higher healthcare costs due to unhealthy food environments.
The scientific and economical perspective suggest that production and availability of fruits and vegetables is essential for agricultural revenue as well as decreasing medical costs for chronic diseases. The cultural and ethical perspectives suggest that the standard American diet is the driving force of the decline in bee populations. Demand for processed foods and animal products has supported the expansion of monocultured crops as well as market and political control by large agricultural industry. These agricultural practices provide nutritionally inadequate diets for both bees and humans and work to derail environmental policy. The profession of dietetics has recognized its role in preventing chronic disease and supporting environmental stewardship - a fundamental component in protecting the pollinators responsible for producing the foods that fight chronic disease.
Therefore, a preventative approach to the decline in bee populations would have environmental, medical and economic advantages. Protecting bee populations through education, research and compressive policy change promotes biodiversity, continued profitability of nutrient dense produce and decreases medical costs.
Conclusion
Complex economic and cultural shifts have led to changes in food environments for both bees and Americans. The consequences of these shifts include environmental destruction, a decline in bee populations, potential decrease of disease fighting crops and the creation of an agriculture oligarchy. The solution to such an interrelated and far reaching problem will require cross collaboration between policy makers and several fields of science to create legislative changes and prevention programs.

Case Study
Mr. M. was a 77-year-old decisionally incapacitated
long-term nursing home resident with chronic schizo-
phrenia who was admitted to the hospital with a
bacterial pneumonia. His past medical history was
notable for deteriorating functional status over the
past 2-3 years, urinary retention requiring chronic
indwelling bladder catheterization, and two recent
hospitalizations for urinary tract infections leading
to sepsis. He developed respiratory failure soon after
admission and was intubated and placed on mechani-
cal ventilation. Follow-up studies suggested worsen-
ing pneumonia and acute respiratory distress syn-
drome (ARDS), as well as worsening kidney function.
The patient was unable to participate in any decision
making. His guardian requested that cardiopulmo-
nary resuscitation and all other intensive care be pro-
vided if necessary, including dialysis should Mr. M.’s
kidney failure continue to worsen. After five days of
mechanical ventilation, the patient was weaned from
the ventilator and extubated. The palliative care ser-
vice was consulted following the extubation; his criti-
cal care team questioned whether it would be appro-
priate to re-intubate the patient if he again developed
respiratory failure. The palliative care team contacted
Mr. M.’s brother, his only living relative, who felt the
patient’s quality of life was poor and believed the
patient would not want aggressive medical care. The
staff at his nursing home was contacted, as well as
the patient’s mental health case manager, who had all
known Mr. M. for many years. All concurred with his
brother’s assessment. Additionally, the nursing home
staff said that Mr. M. would not be able to return there
if the plan was to continue more intensive medical
management of his worsening health conditions. Hos-
pice care was discussed with these parties, and it was
thought that choosing hospice would best represent
the patient’s wishes under the circumstances. The pal-
liative care team contacted his guardian and explained
the patient’s medical situation and its implications
for his ongoing care (including the need for physical
restraints, loss of stable nursing home placement, and
confinement to the acute care hospital environment
for the duration of his acute illness). Based on this new

These types of disasters create environmental health concerns through the spread of toxic waste, increased water-borne infections and insect-borne disease, as well as loss of power or the inability to navigate roadways during floods or blizzards. In addition, natural disasters create environmental concerns such as low food supply, destruction of crops and livestock, and lack of clean water. Natural catastrophes like these are predicted to increase in frequency and severity as long as climate change with increasing global temperatures continues unchecked through slow reaction or frank inaction by governments.
Organizations across the globe are developing strategies for decreasing their carbon footprint in order to reduce pollution that contributes to climate change. Because no one is immune from this hazard, nurses must have a grasp of the growing concern for the impact of climate change and its associated implications for public health worldwide. It is important for nurses to learn how to increase their leadership capabilities to affect local, state, and national outcomes.

Both in mice and rats, CDH found in the fetal and
neonatal periods was a posterolateral type
of diaphragmatic hernia (DH) showing a distinct side-preponderance: the left-side
preponderance in mice and right-side preponderance in rats. CDH in the offspring,
surviving after weaning, was mostly of retrosternal type in mice and of pericardial type
in rats. CDH induced experimentally in the present study

for human CDH in terms of both anatomical features and the
time of appearance of different types of CDH as well as clinical symptoms.
Lung hypoplasia was observed in the offspring with and without CDH; though its
offspring with severe lung
