Catastrophic health expenditure and poverty and Malawi by Martina Rhino MchengaIFPRIMaSSP
Out-of-pocket (OOP) health payments can cause financial hardship to households, which may push them into poverty. The paper investigated the impact of OOP health payments on households’ economic situation in Malawi using data from the Third Integrated Household Survey (IHS3). The study adopts the World Health Organization’s approach in measuring the extent of catastrophic health expenditure and impoverishment. Within the framework of OOP health payments on household’s economic status, the paper computes new poverty estimates. These poverty estimates purportedly take into account the poverty impact of OOP health payments. It is found that if OOP health payments are factored in, the level of poverty in Malawi is higher than official figures suggest. For instance, an additional of 0.93% of households fall below the poverty line after paying for health care. It also uses a logit model to identify the determinants of catastrophic health expenditures. It is found that chronically sick members, large number of illness episodes and large households are highly likely to incur catastrophic health expenditure.
Healthy Savings. Medical Technology and the Economic Burden of DiseaseRevital (Tali) Hirsch
As America ages and sedentary lifestyles and unhealthy diets become more common, experts agree the nation is suffering a sharp rise in the prevalence of chronic disease. As the 21st century unfolds, technology – in the form of advanced diagnostic and therapeutic devices -- can meet the need for early detection and more effective management of illness. Some researchers, however, have questioned whether the overall benefit of technical advances outweighs the costs -- a question this report definitively answers.
Accordingly, researchers at the Milken Institute undertook a comprehensive, quantitative documentation of medical technology's impact on the economic burden of disease. The study also projects how future innovation in this sector would affect the health care system and the larger economy -- a positive benefit of more than $23 billion a year for the United States.
The study takes a systematic approach to documenting the full costs and broader economic benefits of health care investments by examining innovations pertaining to four prevalent causes of disability and death: heart disease, diabetes, colorectal cancer, and musculoskeletal disease. The report considers therapeutics and diagnostic devices that are widely used and have substantially affected the lives of patients as well as the overall U.S. economy. Among the 10 devices or device-based procedures studied are pacemakers, insulin infusion pumps, colonoscopies, and joint replacement surgery.
The data demonstrate that the use of medical technology brings considerable economic benefits. These are seen in both aggregate savings in treatment expenditures and prevention as well as the reduction of "indirect impact" through larger contributions to the economy.

The U.S. is an important non-European country for health economists and decision-analytic modelers because it is a large country in terms of its population size and an even larger market not just but also for health care services and goods. Also, much of not just basic but also translational research including HEOR comes out of the U.S. incl. the original idea for cost-effectiveness analysis.
Regardless of whether you’re American or not, most people have pretty strong ideas about the U.S. Edvard de Bono, not the U2 singer but the originator of the term Lateral Thinking, famously said that the U.S. are not a country but an idea.
This talk attempts to compare the United States’ health care expenditures and outcomes with others around the world; to highlight relevant recent controversies in the U.S. health policy debate related to costs; and to explore why U.S. care is so expensive (and what can be done about it).
