Cerebro Vascular Accident - A case presentationAkhil Sai
This document presents a case of cerebrovascular accident (CVA) in a 65-year-old male patient. The patient presented with headache, blurred vision, vertigo, slurred speech, and imbalance. He has a history of hypertension and diabetes. Diagnostic tests revealed lacunar infarcts. He was assessed as having a CVA due to hypertension. His treatment included amlodipine, atorvastatin, meclizine, glycerin, pantoprazole, B complex, and aspirin.
The patient is a 13-year-old male who went into cardiac arrest after being accidentally elbowed in the chest during a baseball game. Paramedics found him in ventricular fibrillation and he regained consciousness after defibrillation. Tests showed mild abnormalities but no underlying heart conditions. The likely diagnosis is commotio cordis, a rare condition where a blunt chest impact causes cardiac arrest through mechanoelectric effects on the heart during a specific part of the cardiac cycle.
This document discusses primary hypoadrenalism (Addison's disease), including its symptoms, causes, diagnosis, and treatment. Regarding diagnosis, it emphasizes that the short-synacthen test is the best way to diagnose Addison's disease. For treatment, it recommends hydrocortisone and fludrocortisone as the standard medications, and notes the importance of monitoring patients to ensure the treatment regimen remains adequate. It also discusses how to manage unwell patients with Addison's disease through increasing their glucocorticoid dose and providing intravenous hydrocortisone if needed.
Dilated cardiomyopathy (DCM) can have several underlying mechanisms and etiologies including genetic factors, viral infections, autoimmunity, and toxins. The document discusses the prevalence, clinical presentations, genetic causes, role of inflammation and autoimmunity, and viral persistence in DCM. It also summarizes diagnostic and management approaches for DCM and reviews specific genetic mutations involved like those involving lamin A/C that are associated with the worst survival outcomes.
The document describes two cases of young male patients presenting with stroke-like symptoms. Both patients were found to have elevated levels of homocysteine in their blood. Elevated homocysteine is an independent risk factor for stroke, especially in young people. The cause of elevated homocysteine in both cases was likely a genetic defect in an enzyme involved in homocysteine metabolism. Treatment for hyperhomocysteinemia focuses on dietary and supplement strategies to lower homocysteine levels and reduce risk of further vascular events.

A 25-year-old male smoker and drug abuser presented with difficulty speaking and weakness on his right side for one day. On examination, he had Wernicke's aphasia and right upper motor neuron 7th cranial nerve palsy with right hemiplegia. CT scan showed an intracerebral hemorrhage in the left parietal and basal ganglia regions. He was diagnosed with an intracerebral bleed, which is a potential cause of stroke in young persons, especially those who abuse drugs.
This document discusses the diagnosis and investigation of secondary hypertension. It describes various reversible and irreversible causes of secondary hypertension including renal, endocrine, vascular, and drug-related factors. It provides guidance on screening investigations including plasma and urine tests. It also outlines specialized tests that may be used to investigate particular causes such as renal artery stenosis, Conn's syndrome, phaeochromocytoma, and renal artery stenosis. These involve tests such as renal artery duplex ultrasound, CT/MRI scans, renal vein sampling, and MIBG scans.
This document discusses thyroid emergencies, specifically myxedema coma. It begins by describing myxedema coma as a rare, life-threatening condition resulting from severe thyroid hormone deficiency. Precipitating factors include infections, drugs that depress respiration, and withdrawal from thyroid hormone replacement therapy. Clinically, patients present with hypothermia, coma, hypoventilation, hypotension, hyponatremia, and impaired renal and gastrointestinal function. Prompt diagnosis and treatment are crucial for optimizing survival.
A 25-year old female presented with sudden onset loss of speech and weakness on the right side of her body. Examination found reduced pulses and bruits in her neck vessels. Tests showed anemia and inflammation. A neck Doppler showed arterial wall thickening, indicating arteritis. She was diagnosed with Takayasu arteritis based on her age and symptoms. Steroids were started and her symptoms improved. Takayasu arteritis is a rare disease that mainly affects women and can lead to complications affecting the eyes, heart, lungs and brain if not treated.

Secondary hypertension - Etiopathogenesis, Clinical features, Advances in Man...Chetan Ganteppanavar
Secondary hypertension can be caused by renal or endocrine disorders. Renal causes include renal parenchymal diseases like glomerulonephritis which account for 2-5% of secondary hypertension cases. Renovascular hypertension from atherosclerosis or fibromuscular dysplasia is responsible for 1-3% of cases. Primary aldosteronism is an uncommon but important endocrine cause that can present with hypokalemic hypertension and is diagnosed through elevated aldosterone to renin ratio and saline suppression testing. Imaging and adrenal vein sampling are used to determine unilateral vs bilateral disease.
This document discusses three patient cases presenting with resistant hypertension. Case 1 involves a 49-year-old female with obesity and resistant hypertension who shows signs of primary aldosteronism based on blood tests. Case 2 is a 53-year-old female with a 10-year history of resistant hypertension, low potassium levels, and imaging showing a lesion on her left adrenal gland. Case 3 is a 55-year-old male with resistant hypertension and normal potassium who was confirmed to have primary aldosteronism based on salt loading tests, but no adrenal lesion was seen on imaging. All three patients underwent adrenal vein sampling to further evaluate their condition.
The document discusses two conditions: paroxysmal nocturnal hemoglobinuria (PNH) and atypical hemolytic uremic syndrome (aHUS). PNH is a complement-mediated hemolytic anemia treated with the drug eculizumab, a C5a inhibitor. aHUS is a complement-mediated thrombotic microangiopathy that can cause stroke, heart attack, and kidney failure, and is also treated with eculizumab. The document asks the reader to identify PNH, aHUS, and the mode of action of eculizumab.

This patient is a 25 year old female admitted to the ICU for respiratory failure and hypotension following a 6 day prodromal illness. She is intubated and receiving vasopressors. The differential diagnosis includes sepsis, severe sepsis, or septic shock from an unknown source. Principles of management include identifying the source, administering appropriate antibiotics, optimizing hemodynamics and organ function, and providing lung protective ventilation for her acute respiratory distress syndrome. Early enteral nutrition is important to support her systemic immune response and prevent further organ dysfunction from a cumulative energy deficit.
Clinically, more term babies suffered from hypoxic ischemic encephalopathy (HIE) than premature babies. However, pathologically, more premature babies suffered from HIE than term babies. HIE manifests clinically as symptoms of consciousness changes that can be excitatory like seizures or depressing like coma. Treatment of HIE focuses on monitoring, controlling seizures, maintaining circulation and glucose levels, with therapies like hypothermia and investigates like amplitude integrated EEG shown to improve outcomes. The prognosis depends on the severity of brain damage and treatment, with mild cases often having a full recovery but severe cases having a high risk of mortality or neurological sequele like cerebral palsy.
This document defines and describes shock, including its pathophysiology, types, clinical features, management, and monitoring. Shock is defined as a systemic state of low tissue perfusion due to inadequate oxygen and glucose delivery. The main types of shock discussed are hypovolemic, cardiogenic, obstructive, distributive, and endocrine shock. Clinical features vary based on shock severity from mild tachycardia to profound hypotension and coma. Management involves treating the underlying cause, improving cardiac function and tissue perfusion through fluid resuscitation and vasopressors/inotropes as needed. Close monitoring of vital signs and other parameters is important for patients in shock.
- An 80-year-old woman presented after two falls while ambulating at night in the past month
- Echocardiogram showed diastolic dysfunction and moderate mitral annular calcification
- ECG showed first degree AV block but was otherwise normal
- After the first fall she was advised to use a cane and rise slowly from bed
- Her daughter wants a second opinion
I would recommend a 24-hour Holter monitor to assess for transient heart block or bradycardia that could be contributing to her falls, especially during nighttime ambulation. Close monitoring of hydration and medication review is also warranted given her age and risk factors.

EWOCS-I: The catalog of X-ray sources in Westerlund 1 from the Extended Weste...Sérgio Sacani
Context. With a mass exceeding several 104 M⊙ and a rich and dense population of massive stars, supermassive young star clusters
represent the most massive star-forming environment that is dominated by the feedback from massive stars and gravitational interactions
among stars.
Aims. In this paper we present the Extended Westerlund 1 and 2 Open Clusters Survey (EWOCS) project, which aims to investigate
the influence of the starburst environment on the formation of stars and planets, and on the evolution of both low and high mass stars.
The primary targets of this project are Westerlund 1 and 2, the closest supermassive star clusters to the Sun.
Methods. The project is based primarily on recent observations conducted with the Chandra and JWST observatories. Specifically,
the Chandra survey of Westerlund 1 consists of 36 new ACIS-I observations, nearly co-pointed, for a total exposure time of 1 Msec.
Additionally, we included 8 archival Chandra/ACIS-S observations. This paper presents the resulting catalog of X-ray sources within
and around Westerlund 1. Sources were detected by combining various existing methods, and photon extraction and source validation
were carried out using the ACIS-Extract software.
Results. The EWOCS X-ray catalog comprises 5963 validated sources out of the 9420 initially provided to ACIS-Extract, reaching a
photon flux threshold of approximately 2 × 10−8 photons cm−2
s
−1
. The X-ray sources exhibit a highly concentrated spatial distribution,
with 1075 sources located within the central 1 arcmin. We have successfully detected X-ray emissions from 126 out of the 166 known
massive stars of the cluster, and we have collected over 71 000 photons from the magnetar CXO J164710.20-455217.

The binding of cosmological structures by massless topological defectsSérgio Sacani
Assuming spherical symmetry and weak field, it is shown that if one solves the Poisson equation or the Einstein field
equations sourced by a topological defect, i.e. a singularity of a very specific form, the result is a localized gravitational
field capable of driving flat rotation (i.e. Keplerian circular orbits at a constant speed for all radii) of test masses on a thin
spherical shell without any underlying mass. Moreover, a large-scale structure which exploits this solution by assembling
concentrically a number of such topological defects can establish a flat stellar or galactic rotation curve, and can also deflect
light in the same manner as an equipotential (isothermal) sphere. Thus, the need for dark matter or modified gravity theory is
mitigated, at least in part.

Describing and Interpreting an Immersive Learning Case with the Immersion Cub...Leonel Morgado
Current descriptions of immersive learning cases are often difficult or impossible to compare. This is due to a myriad of different options on what details to include, which aspects are relevant, and on the descriptive approaches employed. Also, these aspects often combine very specific details with more general guidelines or indicate intents and rationales without clarifying their implementation. In this paper we provide a method to describe immersive learning cases that is structured to enable comparisons, yet flexible enough to allow researchers and practitioners to decide which aspects to include. This method leverages a taxonomy that classifies educational aspects at three levels (uses, practices, and strategies) and then utilizes two frameworks, the Immersive Learning Brain and the Immersion Cube, to enable a structured description and interpretation of immersive learning cases. The method is then demonstrated on a published immersive learning case on training for wind turbine maintenance using virtual reality. Applying the method results in a structured artifact, the Immersive Learning Case Sheet, that tags the case with its proximal uses, practices, and strategies, and refines the free text case description to ensure that matching details are included. This contribution is thus a case description method in support of future comparative research of immersive learning cases. We then discuss how the resulting description and interpretation can be leveraged to change immersion learning cases, by enriching them (considering low-effort changes or additions) or innovating (exploring more challenging avenues of transformation). The method holds significant promise to support better-grounded research in immersive learning.
