This document contains appendices related to risk assessment forms used by the BBC. It includes a general risk assessment form with sections for listing hazards, assessing likelihood and severity of harm, and obtaining authorization. The form is to be used to document what is proposed, associated risks, team members involved, and hazards identified from a provided list. It also contains the BBC risk assessment matrix for determining risk level based on how bad and likely a hazard is.
