This document provides a general risk assessment form for the BBC. The form includes sections to outline the risk assessment, provide details of the site/location, and identify potential hazards. The assessor and an authorizer must sign off on the completed form. An appendix lists situational, physical/chemical, and health hazards to consider when assessing risk, along with a risk matrix to determine the likelihood and severity of potential harm from identified hazards.
