This document contains a general risk assessment form used by the BBC. It lists potential situational, physical/chemical, and health hazards. It also provides a risk matrix to determine the likelihood and severity of potential harm. The form is used to identify risks, how to reduce them, and determine appropriate control measures. It requires information on location, those involved, hazards identified, likelihood of harm, and periodic review. Authorization is needed before activities are undertaken.
