INTERNSHIP
PROCESS
FLOW
B E F O R E
I N T E R N S H I P
PHASE 1
• Search for possible internship / industry host. Preferable : IT-based
organizations
• Discuss with Program Coordinator / Internship Coordinator on the most suitable
hosts
• Get the student verification letter from Registry
• Contact the host – email, phone, snail mail. Attached your latest resume &
IUMW verification letter
• Send 1 copy of your internship offer letter to Program Coordinator /
Internship Coordinator for record
• Submit SRF to Program Coordinator
D U R I N G
I N T E R N S H I P
PHASE 2
• Submit the REPORT FOR DUTY FORM to Program Coordinator / Internship
Coordinator before end of 1st week of internship
• Fill up the Log Book - daily and weekly
• Get industry supervisor's verification on the log book - WEEKLY only
• Students should maintain high level of disciplines, professionalism and
work ethics. Action shall be taken against students either by the industry host
or the university for any misconduct
• In the event where students believed they are being mistreated by the
industry host, please contact Program Coordinator immediately.
N E A R I N G T H E E N D
I N T E R N S H I P
PHASE 3
• In week 10 of internship, contact Program / Internship Coordinator for
confirmation on industry visit.
• Get a few alternatives dates based on Industry Supervisor's
availability before contacting the Program / Internship Coordinator.
• Once the date is confirmed, send Meeting Invitation to both Coordinators
• In Week 11/12, student shall start writing the Internship Final Report.
• In Week 13, student shall provide the evaluation form to the Industry
Supervisor. After the Industry Host completed the evaluation form, it must be
returned to the student in a SEALED envelope or to be emailed directly to
the Program / Internship Coordinator.

PHASE 4
• Within 1 week after end of internship, students must submit the following
documents to the Program / Internship Coordinator :
• Industry Supervisor's evaluation form
• Student's evaluation on the industry host
• Log book - daily & weekly - verified by Industry Supervisor (comb-
bind)
• Internship Report (comb-bind)
