Heart of the Village recognizes that each
person is a unique individual. Therefore,
we encourage input from participants and
teams in creating a schedule and develop-
ing outcomes that will maximize the po-
tential of each person involved and pro-
vide for the optimal outcome for success.
Participants are encouraged to self-
advocate and make choices that best fit
their own particular needs, interests, and
goals.
Consistency of care is integral to each
individual’s success. Heart of the Village
believes in building strong networks of
support through communication with all
members of the interdisciplinary team and
by coordinating individual efforts into
common goals.
Heart of the Village prides itself in sup-
porting individuals while they explore
fulfilling activities, develop meaningful
relationships, and maintain a high quality
of life.

Medically stable, and compliant with
medication administration
Not a danger to self or others
Independent in basic mobility and
require no more than a
one-person assist
Have managed incontinence
Free of communicable diseases
Ability to follow the rules of the
Day Program
Hours of Service
Monday—Friday 9:00 am—3:00 pm
Fees
Pending eligibility, fees can be covered by
various government or private funding
sources, including CADI and TBI waivers,
insurance, or private pay.
Staff
Staff members include not only an on-site
director as well as direct care staff, but also
numerous consultants including a physical
therapist, nutritionist, and RN. Every direct
care staff member who works with Heart of
the Village is CPR and First Aid certified and
has passed a Medication Administration
Course.
