Your Life, Your Health Workshop
- 1. Lay Leader Training MAKE A DIFFERENCE! Are you interested in becoming a certified lay leader to facilitate “Your Life, Your Health" workshops in your community? There is no cost for the training and all materials will be supplied. "Your Life, Your Health" is an evidence based workshop developed by Stanford University for people living with chronic conditions (or caregivers) on how to enhance their self-management and promote independence. New topics are presented each week and build on the previous session for maximum outcomes. "Your Life, Your Health" is Louisiana's implementation of the Stanford Chronic Disease Self-Management Program and sponsored by the Governor’s Office of Elderly Affairs. There is no charge for the workshop, “Your Life, Your Health” participant workshops are: Free Conducted once a week for 2.5 hours over a 6 week period in a community based setting such as senior centers, churches, clinics and hospitals. Highly interactive and designed to involve participants. Proven to help participants increase their confidence in managing conditions such as arthritis, heart disease and diabetes, and the problems they experience because of it.
