Certain measures showing concentrated safety improvements, such as reductions in falls, infections, and wrong-site surgeries, were analyzed using the Authority's reporting data and estimates of prevented deaths and costs. The study found evidence of pent-up demand, as newly enrolled individuals had higher rates of new patient visits during the first 6 months compared to continuously enrolled recipients. However, overall utilization was not higher for new enrollees. Utilization declined over time for new enrollees, suggesting pent-up demand was met initially through preventive and diagnostic care. This study examined the effects of California's 2011 Medicaid expansion on inpatient utilization at safety net hospitals. It found that Medicaid admissions increased at safety net hospitals while uninsured admissions decreased. The proportion of admissions that were uninsured also declined significantly. Additionally, the number of preventable admissions decreased more for uninsured patients at safety net hospitals compared to non-safety net hospitals. However, the acuity level of Medicaid patients increased more at safety net hospitals. The changes suggest the Medicaid expansion improved payer mix for safety net hospitals but new Medicaid patients were sicker.
OpenNotes: Transparent Clinicians' Notes for Health & Illness
OpenNotes began as a research project that studied the effects of allowing patients access to their primary care doctors' visit notes. The study found that patients who read their notes felt more engaged in their care, better understood their conditions, and were better prepared for visits. Doctors had initial concerns about increased workload, but found little real impact. After one year, 99% of patients and the majority of doctors wanted to continue open notes access. The research demonstrated that open notes can help patients manage their health more effectively with little negative impact. Tethered PHRs link to electronic medical records and allow consumers to view provider information, but few offer secure online communication with doctors currently. Interconnected and shared PHR models that link multiple data sources could provide more complete health histories but also pose greater privacy and security risks.

Electronic personal health record systems (PHRs) are introduced to develop patient-centered health care by providing medical records and other required information accessible to patients with the idea of helping patients in health self-management. There is a number of different fundamental designs for PHRs. As primary care physicians will play a major role in patient health, PHRs are also to be linked to physician’s electronic medical record systems, so the adoption of PHRs dependent on growth in electronic medical record adoption. Also, many PHRs are physician-oriented and which does not include patient orientated functionalities. PHRs need to provide to support self-management and disease prevention if improvements in health outcomes are to be expected.
There has been a surge in activity around the adoption of PHRs for patients and consumers. There are some issues surrounding PHRs where they are more than just a static repository for patient’s data, they help combine data and convert that into information and provide knowledge which helps patients to participate actively in their own care.
Benefits:
PHRs have a wide variety of potential benefits for consumers. Out of it one of the most important PHR benefits is it has greater patient access to wide array of useful health data, information, and knowledge. Patients can use that to improve their health and manage their diseases. Those kinds of information are highly customized to make PHRs more successful and useful. Patients with illness will be able to track their diseases in conjunction with the providers, by providing earlier help when they see a problem. PHRs will help in improving communication between patients and caregivers which will make it easier for patients and caregivers to ask general questions, to request refills, to set up appointments and also to report problems. PHRs will also benefits to the payers and purchasers for health care include lower diseases, lower management cost, lower medication cost.
Risks:
Although there are many benefits to a PHR system, which may also include some risks. Like, it is very difficult and time consuming to collect all current and past medical information. Patients have to request information from multiple health providers to get complete medical history. Once all the information is collected patients must manual enter the data into their PHR.

PHRs could allow individuals to manage their own healthcare by communicating with providers and accessing health information. However, PHRs currently lack standardization and the ability to exchange information between different systems. Addressing issues such as determining security protocols, exchanging data between health information exchanges, and clarifying legal policies will help realize the full benefits of PHRs. Widespread adoption also requires resolving questions over who pays for and controls PHR data. Further research is needed to understand how to best design PHRs and incentivize their use.
