Having the Right Information at the Right Time in the Right Place
The CTIS-developed mPHR 2® application is designed to overcome the many bottlenecks and redundancies
existing in today’s healthcare system that limit real-time access to individual patient health information. mPHR 2
is an interoperable Personal Health Record (PHR) software solution currently applied on the iPhone and Android
Operating Systems to support chronically ill patients by accurately documenting and communicating patient
history to physicians and other healthcare providers. This product digitally captures all of a patient’s healthcare
information and allows real-time exchange between different web-based PHRs and healthcare providers. Helping
to eliminate the need for redundant patient data entries and assembling information in a single repository will
result in improved efficiency and quality of patient care.
Through the use of mPHR 2, patients can:

Getting Started
mPHR 2 gives patients a simple and intuitive way to record and analyze personal health information
by conveniently providing a one-stop shop for managing this information. Additionally, mPHR 2 also
stores contact information for current physicians and other healthcare professionals (nutritionists,
trainers, therapists), diagnostic laboratories, and pharmacies so that when changes inevitably occur, it
can be automatically relayed to all relevant parties.
Create your mPHR 2 Profile
Individual profiles are the central access point
for users to manage and track all health data.
Note: mPHR 2 will serve as a PHR solution for
both you and your family members with the
ability to manage and view unlimited profiles
with the proper access.
Customize your mPHR 2 Home Page
Your Home Page is the main screen of
the application that is fully customizable,
displaying the icons of the application
modules you use most. The Home Page can
also serve as a brief overview of your personal
health status, as it will display information such
as your current medications, and that day’s
appointments and reminders. As your needs
change you can easily adjust your Home Page
accordingly.

Problems
The Problems Module serves as a library of physical condi-
tions you currently have or have had in the past. It lists your
full history of conditions along with the date for which it be-
gan and subsided.

4. Medications
The Medications Module helps you manage
all the prescription and over-the-counter
medications for which you currently take or have
previously been prescribed. For each medication
listed, the following details are captured:
• Dosage
• Dosage Frequency
• Strength
• Route
• Quantity consumed versus total quantity
provided
• Number of remaining refills
• Colored icon to indicate when it is time to
refill
The Medications Module links to your contact
list, prescribing physician information, pharmacy
information, and your calendar. The automatic
scheduling feature populates reminders
based on defined dosage frequency and your
treatment duration.
Test Results
The Test Results Module provides a summary
of each medical test administered. The latest
seven results are shown on the home screen
to provide a quick status check. Test results are
displayed numerically, and graphically using
Google ChartsTM
. These visual aids serve as a
great, informative, quick overview of how you
have been doing over time and allows you
to conduct trend analysis of your results. This
module also links directly to the Problems
Module.

5. Immunizations
The Immunizations Module provides a central location to store all immunization records. Details
such as dates, series tracking (1 of 3), administering physician, associated conditions, and
reactions can be captured.
Procedures
The Procedures Module logs all administered procedures and associated results. This module
can be linked Medications, Problems, Test Results, and Contacts.
Allergies
The Allergies Module helps track known allergies, symptoms, and triggers. Tracking these
accurately documents allergic reactions and allows reporting to physicians. This Module can be
linked to any associated module.
Wellness Tracker
Wellness Trackers allow you track your wellness metrics so that you can achieve your personal
health goals. These Trackers provide you with the ability to track things such as blood pressure or
caloric consumption on a daily basis. By being able to track your blood sugar and your heart rate,
you can maintain a diary of your progress towards meeting your health goals.

6. Photos
Personalize your profile by uploading photos and associating
to various modules. Take photos of your medications or
prescription label. Take a snapshot of your physical records,
test results, or even medical invoices. Create visual aids across
the application to capture adverse reactions or temporary
problems.
Calendar
The Calendar allows you to manage appointments and keep
you on track with your daily routine, directly on your mobile
device. Scheduled alerts remind you to take your medication
or perform self-tests, such as checking blood pressure or
blood glucose levels. The automatic scheduler links directly
to the Medications module.
Contacts & Directions
The Contacts list provides you the ability to import contact
information of physicians, pharmacies, hospitals, etc., directly
from your Google Health - Medical Contacts. No need for
duplicate entry. Get turn by turn directions to your point of
care using Google MapsTM
.

Call your prescribing physician or pharmacy directly from the medications screen. Quick access to the
help you need whether you have a question or concern or its time for a refill.
Immunizations
Additional data capture including time and location of your immunizations as well as areas to note and/
or schedule upcoming immunizations.
Procedures
Additional data capture including date, time, location, prescribing physician, purpose, required
preparations, recovery status and associated symptoms.
Allergies
Chart your allergies. Log symptoms and associated time period and cause. Graphically view your data and
note any seasonal trends you may have. This will help to better understand your body’s reactions and be
proactive in managing these allergies.
