Analysis Phase
Parents make several calls to triage without service
Inaccurate or incomplete triage information result in parents being seen in the wrong department OR
Parents being seen without all the necessary practitioners available
Parents are forced to go back on the waiting list for an appointment in the correct department.
A Children’s Hospital has determined that a large number of patients with developmental delays and disorders are forced to make several appointments in several clinics over a long period of time delaying progress for the children in a timely manner and determined that team evaluations would be more helpful however because the clinics are staffed for individual requests the task is challenging and training is needed to meet their goals. In addition to the triage coordinators information via telephone a patient questionnaire will also be sent out, returned and reviewed by directors before an appointment can be scheduled.
Stakeholders
Child Development Directors: Review Intake information provided by triage coordinators and parents to determine evaluation methods to provide evaluation of developmental delays, learning and behavioral disorders to provide diagnosis and therapeutic and medication treatment plans
Triage Manager: Coordinates training and behavioral methods for triage coordinators
Triage Coordinators/Schedulers: Collects information from parents on areas of concern to determine which clinic questionnaire should be sent to the parent for director review and schedule appointment based on Directors analysis
A Director of the team evaluation is assigned to coordinate communication through all the clinics to proceed.
Design Phase
New Patients will be given questionnaire surveys to complete after evaluations for feedback on the process and services
Directors from Child Development Departments and Triage Manager which include Development, ADHD, Psychology, Psychiatry and Neuropsychology shall evaluate the process and identify areas of improvement for triage and practitioners
Triage Manager will meet with coordinators and schedulers individually for observation determine the best methods and identify training needs to meet program goals
Coordinators shall offer feedback through confidential short confidential survey with suggestions and challenges of goals
The need for software and procedure changes for both practitioners and coordinators will be evaluated and implemented if necessary
In order to determine the best methods to identify the patients in need of a team evaluation all the records of such patients in the past must be reviewed.
The directors must provide questions that will determine which patients are eligible for team evaluations to avoid wasting valuable appointments on patients that could have been seen by individual practitioners instead. Patient and coordinator input will be necessary.

Tasks
Part A: Develop a WHS Program or Procedure
Task
In this Unit of Competency we learnt that it is common practice for organisations to adopt the “Six Step Approach” to develop and implement a Work health and Safety Management System (WHSMS).
As you discovered, an organisation’s WHS Policy forms the “corner stone” of the WHSMS and clearly states the organisation’s commitment to WHS and identifies the initiatives that will be taken to ensure a safe work environment is provided to all persons working in, serviced by, or visiting the workplace.
For part A of this Assessment you are required to write one (1) WHS Program or Procedure that aims to fulfil the goals set by an Organisation’s WHS Policy.
To successfully complete this task you must:
1. Read the following example WHS Policy for “Big Corp Travel”.
2. Select one (1) of the WHS programs or procedures listed in the WHS Policy for this tourism company and prepare (write) a detailed program or procedure that can be included in the Organisation’s WHS Programs manual.
3. Your WHS Program/Procedure must contain and address the following paragraph headings:
· WHS Program/Procedure title (name).
· Purpose and objectives (what does the program/procedure aim to achieve?)
· Scope (who does this program/procedure apply to? Departments and personnel)
· Program/Procedure (outline the steps in the program – what, where, how)
· Implementation Strategies (how will the program be implemented and communicated to staff?)
· Monitoring and Evaluation (how will the program be monitored and evaluated?)
· Review (when will this program be reviewed to ensure that it remains current and relevant and who will conduct this revision?)

Safety and Health Training Plan
1.0 Introduction
Training is one of the most important components within our company’s safety management system. It gives
employees an opportunity to learn their jobs properly, bring new ideas into the workplace, reinforce existing ideas
and practices, and it helps to put our Safety and Health Program into action.
Everyone in our company will benefit from safety and health training through fewer workplace injuries and illnesses,
reduced stress, and higher morale. Productivity, profits, and competitiveness will increase as production costs per
unit, turnover, and workers compensation rates lower.
2.0 Management commitment.
We (or company name) will provide the necessary funds and scheduling time to ensure effective safety and health
training is provided. This commitment will include paid work time for training and training in the language that the
worker understands. Both management and employees will be involved in developing the program.
To most effectively carry out their safety responsibilities, all employees must understand (1) their role in the program,
(2) the hazards and potential hazards that need to be prevented or controlled, and (3) the ways to protect themselves
and others. We will achieve these goals by:
• educating everyone on the natural and system consequences of their actions;
• educating all managers, supervisors and employees on their safety management system responsibilities;
• educating all employees about the specific hazards and control measures in their workplace;
• training all employees on hazard identification, analysis, reporting and control procedures; and
• training all employees on safe work procedures and practices.
Our training program will focus on health and safety concerns that determine the best way to deal with a particular
hazard. When a hazard is identified, we will first try to remove it entirely. If that is not feasible, we will then train
workers to protect themselves, if necessary, against the remaining hazard. Once we have decided that a safety or
health problem can best be addressed by training (or by another method combined with training), we will follow up by
developing specific training goals based on those particular needs.
Employees. At a minimum, employees must know the general safety and health rules of the worksite, specific site
hazards and the safe work practices needed to help control exposure, and the individual's role in all types of
emergency situations. We will ensure all employees understand the hazards to which they may be exposed and how to
prevent harm to themselves and others from exposure to these hazards.
We will commit available resources to ensure employees receive safety and health training during the circumstances
below.

2. After completion of the training a test will be given to all learners which they will need to
complete and submit for grading. Home agents will have a link with the test and results
will be automatically compiled as soon as the test is submitted. The staff members who
will do the simulation training on a different date will take the test upon completion of
the simulation training and the results will be automatically compiled.
Results of all test will be documented on the employee’s file as well as recorded and kept
on file in the HIM department for auditing purposes
Any employees who do not pass the test will be able to re-take the training and test by
scheduling with the HIM supervisor.
A survey will be sent to all staff who received the test to determine what was effective
and what changes are suggested or ideas for making the training better.
Note: Refresher training will be done yearly for all employees where any policy changes and
updates will be communicated. We appreciate the loyalty and hard work of our staff. HIPAA
compliance is an ongoing process and we want to make sure our staff is aware and
understands all aspects of policy. It is our responsibility to make sure they are ready for any
situation.
