Medical intervention is a practical knowledge domain, which means that you need to practice to achieve mastery. However, today we are lucky because “the patients’ role” in the learning process has been taken over, at least in part, by simulators. Thanks to the Connect project, medical science is entering a new revolutionary era in which the physical boundaries of sharing knowledge disappear.
VirtaMed AG, a Swiss company founded by innovators and engineers, produces the most realistic medical training simulators in the world. VirtaMed devices are already used all around the world, mainly in medical centers, hospitals and other facilities involved in training healthcare professionals. The company, which began as a small startup now employs over a hundred people on three continents.
Future medical staff can use original instruments tools to practice their surgical skills before they start performing procedures on patients. Being able to learn under such conditions allows for the practical honing of skills which is of incredible value in modern medicine. The simulators also aid learning by allowing both trainees and teachers to monitor a trainee’s progress and identify areas for improvement.

The Health Economics and Decision Science department at the University of Sheffield is developing a new patient-reported outcome measure called Recovering Quality of Life (ReQoL) to assess quality of life for people with mental health difficulties. ReQoL will consist of seven themes identified from a literature review and patient interviews. Over 1,000 patients will help validate an initial pool of 61 questions across clinical settings. Psychometric analysis will produce short and long versions of ReQoL. Validation work will continue into 2016, with the goal of providing a new tool for evaluating mental healthcare outcomes in both clinical practice and economic studies.

This document summarizes the development of an EQ-5D-5L value set for England based on stated preference data from 996 members of the general public. Researchers collected time trade-off (TTO) and discrete choice experiment (DCE) data to value health states. They found evidence that distinguishing between subtle differences in 5L health states placed a high cognitive burden on respondents. The data showed logical inconsistencies and interviewer effects that required censoring some responses. Researchers developed models combining TTO and DCE data to generate utility values for the 3,125 possible 5L health states for use in cost-utility analysis in England.

This document summarizes a review that investigated the diagnostic accuracy and prevalence of clinically relevant pathologies in atrial fibrillation (AF) patients identified by transthoracic echocardiography (TTE). The review included 44 studies on diagnostic accuracy and 16 on prevalence. It found that TTE demonstrated high sensitivities and specificities for selected pathologies. Prevalence data varied widely but were relevant to UK patients. The review concluded that routine TTE in new AF patients would identify many co-existing pathologies, but the impact on patient management is uncertain.
Estimating EQ-5D health state values for Rheumatoid Arthritis patients: a lim...ScHARR HEDS
This document describes a study that developed a new statistical model called a mixture model to estimate health state utility values from the EQ-5D for patients with rheumatoid arthritis (RA). Standard linear regression models performed poorly due to the limited and non-normal distribution of EQ-5D scores. The new mixture model fits the EQ-5D data distribution better without predictions outside the feasible range. It is more accurate than linear models for economic evaluations, with one example showing a difference in estimated quality-adjusted life years of 0.25. While more complex, the mixture model provides less biased estimates of treatment benefits important for health technology assessments.
MEASURING HEALTH RELATED QUALITY OF LIFE IN DIABETES: THE VALIDATION OF THE ...ScHARR HEDS
This study aimed to validate the Diabetes Health Profile (DHP-18), a patient-reported outcome measure with three domains (psychological distress, barriers to activity, disinhibited eating), and develop a shorter 12-item version (DHP-12). Factor and Rasch analyses were conducted on data from over 1,500 patients to validate the domain structure and assess item performance. Based on these analyses, four items were selected from each domain to create the DHP-12. Initial comparisons found the DHP-12 scores were highly correlated with the DHP-18 and had comparable psychometric properties, though the barriers to activity domain showed lower internal consistency.
DEVELOPMENT OF A PREFERENCE-BASED, CONDITION SPECIFIC PATIENT REPORTED OUTCOM...ScHARR HEDS
The SPVU-5D is a new patient-reported outcome measure developed specifically for assessing health-related quality of life in patients with venous leg ulcers. It was developed using a bottom-up approach including qualitative interviews with patients and clinical staff to identify relevant health domains. The final measure includes 5 dimensions (pain, mobility, mood, smell, and social isolation) shown to have good validity and practicality. Health state valuations from the UK general public were used to assign preference weights and create a scoring algorithm for the SPVU-5D.
Accounting for Psychological Determinants of Treatment Response in Health Eco...ScHARR HEDS
This study tested incorporating psychological prediction models of treatment response within an economic model of a diabetes education program (DAFNE). Regression models were used to predict patient response to DAFNE based on psychological characteristics. These prediction models were then integrated into a simulation model to evaluate policies of only providing DAFNE to predicted responders or offering follow-up support to predicted non-responders. The results found that providing DAFNE only to predicted responders increased costs and decreased health benefits compared to current practice. Providing follow-up support to predicted non-responders dominated current practice, but was sensitive to assumptions about follow-up benefit. This methodology demonstrates how psychological factors can be better incorporated into economic models of behavioral interventions.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).

Mainstreaming Assistive Living Technologies
– the MALT project – is a three year project
funded by the Technology Strategy Board
looking at barriers to the uptake of ALTs.
Led by Professor Mark Hawley within
the Health Services Research section in
ScHARR, this ambitious project consisted
of four workstreams to examine individual
facets of this problem.
Workstream 1 has looked at providing more
accessible cost-effectiveness and financial
information to assist in the planning,
commissioning and contracting of telemonitoring
services. This workstream undertook interviews
with stakeholders from the NHS, private service
providers and technology manufacturers, to
identify what information they required to make
informed decisions about the procurement
and provision of telemonitoring services. These
specifications were then transformed into a
research model of financial flows and cost-effectiveness,
which was then tested on two
planned deployments of telemonitoring. Finally,
this research model was converted into an
online model that is free to use.

The key features of the model are that different
telemonitoring service models can be specified
in terms of number of units, effectiveness,
patient sub-group, length of deployment, etc.,
and then cost-effectiveness estimated via a
disease model based on Clinical Commissioning
Group specific hospitalisation data. Costs are
specified for each stakeholder as ‘one-off’,
fixed and variable, thereby allowing detailed
profiles of costs over time to be calculated
for multiple perspectives. It is this ability to
produce detailed financial outputs for service
providers, together with cost-effectiveness
information for commissioners, that makes
this modelling unique.
Piloting of the online model was completed in
October, with final reviews due in November,
with the model going live on December 1st, via the
MALT website (http://malt.group.shef.ac.uk).
