The Georgia Transplant Foundation (GTF) was founded in 1992 to provide financial and educational support for organ transplant candidates, recipients, and their families in Georgia. The GTF Adolescent Transition Program aims to help adolescent transplant recipients transition to independent adulthood by providing education and experiences to manage their health. Support from the Country Club of the South Charity Guild would allow the program to expand services and increase incentives and apprenticeships to encourage medication adherence and clinic visits. The program costs $95,000 which is fully funded by foundations including a $40,000 grant from the Charity Guild.
