This document provides guidelines for evaluating new fever in critically ill adult patients. It defines fever and recommends a threshold of 38.3°C or higher as triggering a clinical assessment. It recommends obtaining blood cultures when infection cannot be ruled out, with at least 10-15ml of blood drawn from separate venipuncture sites within 24 hours. Rectal temperature measurements and axillary measurements are not recommended. Physical exams and clinical judgement should guide testing rather than automatic orders in response to fever.
