Whither Health Research in Ireland?
Graham Love
Chief Executive
Health Research Board
West/North West Hospitals Group Conference, 09-09-14
From Groups to Trusts – Reforming the Health Service
2
Sample R&D Intensities
• Healthcare is a KNOWLEDGE industry!
• Expectations & costs are rising out of sync with resources. This is not sustainable.
• We need a much smarter use of our resources, people, processes & technology to meet
3
healthcare needs/expectations.
• Our healthcare system needs a much greater self-learning capacity: an ability to research &
develop people/processes/technology both in Ireland and to absorb them from abroad.
• That capacity is not strong in our healthcare system at present.
• And should we be surprised?

HRB Centre for Advanced
Medical Imaging
• €4m for MRI scanner
to be used solely for
patient focused
research
• Available to
researchers
nationwide to
conduct range of
studies
HRB - Perinatal Ireland
• €3.9m for ultrasound equipment and
support staff across 7 hospitals
• Created a research collaboration
harnessing combined medical expertise
and access to large cohorts (75,000)
babies for studies that would have
otherwise been impossible
• Already led to new national guidelines to
protect small babies in womb
• John Foley (Superman), diagnosed with
intrauterine growth restriction and born 11
weeks premature weighing just 2 pounds 6
ounces at birth – now 19 months old, with
his big brother Martin (4 years).
• Grainne Foley (Superman’s mother) was a
participant in Perinatal Ireland research;
• ‘Taking part was a bit of a no-brainer really.
Knowing that there was potential for some
difficulties was a definite worry, but it was
very reassuring to be able to avail of the
resources, monitoring and surveillance that
the research team brought to the hospital.’
HRB Clinical Research Facilities
Original Vision
 infrastructure, physical space
 facilities
 expertise
• Optimally support patient-focused
research studies and clinical
studies
• Translating the knowledge to
reimbursed, regulatory-approved
advances in patient care as
speedily as possible
Benefits for
• Researchers and health
professionals
• Patients/population
• Policy makers

Opening of Cork CRF, 2012
The Groups & the CRFs are
aligning...
Dublin North East
Dublin Midlands
South/South West Dublin East
17
West/North West
So it’s nice to have CRFs in all
the right places….
• But the greatest value is probably not the physical
buildings (facilities are a prerequisite…....)
• BUT the processes and people that will wrap
18
around them add the greatest value
• Research Governance
• Quality
• Safety
• Resources, capacity & skill to link and translate
research into practice
Research & the Groups-to-Trust
transition
• The Groups (Trusts) represent a fantastic opportunity to
19
‘centralise’ research within each region
• Increased
– Visibility/Transparency
– Quality
– Safety
– Translation from research to practice
?? Make research a criteria for Group to Trust transition??

Dr. Graham Love, Chief Executive, Health Research Board

  • 1. Whither Health Research in Ireland? Graham Love Chief Executive Health Research Board West/North West Hospitals Group Conference, 09-09-14 From Groups to Trusts – Reforming the Health Service
  • 2. 2
  • 3. Sample R&D Intensities • Healthcare is a KNOWLEDGE industry! • Expectations & costs are rising out of sync with resources. This is not sustainable. • We need a much smarter use of our resources, people, processes & technology to meet 3 healthcare needs/expectations. • Our healthcare system needs a much greater self-learning capacity: an ability to research & develop people/processes/technology both in Ireland and to absorb them from abroad. • That capacity is not strong in our healthcare system at present. • And should we be surprised? €14,000,000,000 €43,000,000 0.31% £109,000,000,000 £2,400,000,000 2.20% $53,000,000,000 $2,800,000,000 5.28% Total spend R&D spend R&D as % of total

  • 11. HRB Centre for Advanced Medical Imaging • €4m for MRI scanner to be used solely for patient focused research • Available to researchers nationwide to conduct range of studies
  • 12. HRB - Perinatal Ireland • €3.9m for ultrasound equipment and support staff across 7 hospitals • Created a research collaboration harnessing combined medical expertise and access to large cohorts (75,000) babies for studies that would have otherwise been impossible • Already led to new national guidelines to protect small babies in womb • John Foley (Superman), diagnosed with intrauterine growth restriction and born 11 weeks premature weighing just 2 pounds 6 ounces at birth – now 19 months old, with his big brother Martin (4 years). • Grainne Foley (Superman’s mother) was a participant in Perinatal Ireland research; • ‘Taking part was a bit of a no-brainer really. Knowing that there was potential for some difficulties was a definite worry, but it was very reassuring to be able to avail of the resources, monitoring and surveillance that the research team brought to the hospital.’
  • 13. HRB Clinical Research Facilities Original Vision  infrastructure, physical space  facilities  expertise • Optimally support patient-focused research studies and clinical studies • Translating the knowledge to reimbursed, regulatory-approved advances in patient care as speedily as possible Benefits for • Researchers and health professionals • Patients/population • Policy makers 13
  • 14. HRB Clinical Research Facility - Galway 14
  • 15. Wellcome Trust-HRB Clinical Research Facility – Dublin
  • 16. HRB Clinical Research Facility - Cork Opening of Cork CRF, 2012
  • 17. The Groups & the CRFs are aligning... Dublin North East Dublin Midlands South/South West Dublin East 17 West/North West
  • 18. So it’s nice to have CRFs in all the right places…. • But the greatest value is probably not the physical buildings (facilities are a prerequisite…....) • BUT the processes and people that will wrap 18 around them add the greatest value • Research Governance • Quality • Safety • Resources, capacity & skill to link and translate research into practice
  • 19. Research & the Groups-to-Trust transition • The Groups (Trusts) represent a fantastic opportunity to 19 ‘centralise’ research within each region • Increased – Visibility/Transparency – Quality – Safety – Translation from research to practice ?? Make research a criteria for Group to Trust transition??

Editor's Notes

  1. €100m portfolio @€40m invested annually In 2013 33 new products and interventions in development (5 patents, 2 Spinouts and 26 healthcare innovations) 78 influences on policy and practice 51 new laboratory methods, training and other materials or sample collections generated 246 international collaborations,  of which 29 are with internationally based companies €10.9M in funding leveraged 126 research-related jobs supported
  2. It relates to a review on interventions for stroke care. It was conducted through a Cochrane fellowship by a guy called Killian O’ Rourke, who worked with Prof Peter Kelly, the lead clinician at the Dublin Stroke Unit and now Clinical Care Lead for the National Stroke Programme (and HRB Clinician scientist). As well as publishing in the Cochrane Library and getting a lot of attention, the review results were also published in Stroke and had massive citations…and the results are locally informing the work of the care programme.
  3. Pat Commins Acting General Manager Galway University Hospitals, Minister Harney, and NUI Galway's Professor Larry Egan, Interim Director of the Health Research Board (HRB) Clinical Research Facility, at the announcement regarding the new funding.
  4. Perinatal Ireland – Prof Fergal Malone (Chairman). Research network linking 7 maternity hospitals in Ireland (access to 75,000 births per annum). Along with primary research the network allows sophisticated follow up studies that would otherwise be impossible. Their PORTO study is changing the way we manage perinatal care of ‘small’ babies (intra uterine growth restricted – is the technical term). Photo on left: Ms Fiona Cody, Perinatal Ireland Research Sonographer and Dr Julia Unterscheider, Lead Researcher of the PORTO Study
  5. Good news for researchers and health professionals because they: Have a dedicated space, equipment and expert staff on the hospital site for conducting latest research. Provide the environment and governance structures for participating in world class clinical trials that may not have possible otherwise. Create a centre for learning and collaboration.   Good news for patients because they: Gain access to trials of new and innovative therapies, technologies and service delivery approaches. Should increase the speed at which these innovations and discoveries are available.   Good for policy makers because they: Establish a good governance structure that penetrates all aspects of work in hospitals (all boats rise on the high tide principle). Provide for partnership between the health system, academia and industry which will help both health system innovation and Economic development OR act as a point of entry for industry collaborators which will help both health system innovation and economic development.
  6. You might note, while support is available for most disease areas, local expertise will see specialisations develop over the years.
