April 27, 2015 through October 9, 2015: Project Management Quality Services, Inc. Subcontracted to Metaltek SMCI
Division in Lakeland, Florida. Project Quality Manager responsibilities, including vendor auditing, corrective action program
analysis/implementation. NQA-1-2008 w/ 2009 Addenda Certified Lead Auditor. Metaltek SMCI Division is a metals
fabricator and materials organization supplier. The SMCI Division contracts with nuclear related clients from the Power
industry and the Department of Energy.
December, 2014, through April, 2015: Global Technical Training Services, Inc. Subcontracted to Southern Nuclear
Corporation for Vogtle 3&4 construction quality assurance oversight. Quality Assurance activities with vendor oversight and
support of Licensee Quality Assurance Program.
September, 2008, through July, 2014: Bechtel Power Corporation. Various locations. Project Quality Manager and general
nuclear quality assurance managerial, supervisory, and oversight assignments.
July, 2014 through October, 2014, Bechtel Power Corporation Holding status that allowed retention of benefits while
waiting for assignment opportunity.
From April, 2014 through June, 2014, Project Quality Manager assignment for the Panda Sherman Power Project
(combined-cycle combustion turbine power plant) reporting to the Project Manager during the completion stages of
the project. Responsibilities included staff assignments for KASE (Key Activities for Successful Execution) Gate
development and closeout, resolution of disputed items with the client, punch list performance, and Closed Loop
Corrective Action process implementation.
From August, 2013 through April, 2014, Nightshift Quality representative for the Steam Generator Replacement
Project at the Davis-Besse Power Plant in Oak Harbor, Ohio.
 Provided the project with opportunities to capture lessons learned and act upon program improvements
through use of the Closed Loop Corrective Action process.
 Pursued a risk based graded approach to program implementation and effectiveness.
 Provided motivation and performance recognition through the use of the Leader+ and Quality Coin
programs.
 Aided Environmental, Safety and Health in safety assessments and contributed daily to the behavioral safety
observation program.

 Wrote several Quality Issues and Success Briefing sheets for Project leadership to present to the Bechtel
organization.
 Assisted Field Engineering in effective subcontract monitoring and execution.
 Delivered responsible liaison results with the client’s Quality Assurance oversight personnel in preventing the
escalation of undesirable conditions.
From November, 2012 through August, 2013, held a temporary assignment at the Corporate Headquarters in Fredrick,
Maryland performing various analyses, evaluations, and Supplier Quality Assurance Program reviews. Background and
capabilities had allowed the initial temporary assignment to be extended after successfully performing such
assignments as: gap analysis of Canadian Nuclear Power Standards against the Bechtel Nuclear Quality Assurance
Program, performing as the Lead Auditor for the Quality Management System audit of the Bellefonte Nuclear Plant
joint venture, apparent cause evaluation for the small modular reactor project, root cause evaluation assignments
regarding engineering deliverables and corrective action timeliness, and ISO 9001-2008 audit team member at a
California solar power facility. Was an instrumental team contributor to the Bechtel Construction Operations
Incorporated application and ASME re-certification Survey for Certification Mark authorizations representing N, NA,
NPT, NS, (Divisions 1 and 2), and the National Board Inspection Code NR Stamp for repair and replacement.
From September, 2008, through November, 2012, assignment was at the Watts Bar Unit 2 Construction Completion
project in Spring City, Tennessee. Advanced from Supervisor to Project Quality Assurance Manager in October, 2011.
Responsible for the direction, control, and implementation of ASME Section III Division 1 and Safety-Related/Quality-
Related Quality Assurance Program activities. Managed 26 Quality Assurance Engineers and administrative support in
addition to administration of the Corrective Action Program. Responsible for the successful ASME re-certification
Surveys for N, NA, and NPT Certification Mark authorizations in 2012. Prior to this advancement, responsibilities had
included:
 Primary point of contact and oversight of client subcontractors performing project ASME and Safety-Related
project work that did not have a contract with Bechtel.
 Quality Engineering Supervisor for independent in-line reviews of procedures, procurement requisitions, work
orders, Quality Control inspection points, and Authorized Nuclear Inspector interface. Also accomplished the
development and revision of the department’s procedures and quality assurance manuals.
 Quality Assurance Audits and Surveillances Supervisor for ASME, Safety-Related, and Quality Related
programs.
 Certified Audit Team Leader accomplishing NQA-1, ANSI N45.2, and 10CFR73.56 (Access Authorization) related
audits.
August, 1998, through September, 2008: Duane Arnold Energy Center. Palo, Iowa. FPL Energy, LLC (now called NextEra
Energy Resources). Significant responsibilities assigned and accomplished during this period included:
 Certified Audit Team Leader to NQA-1 1994 and ANSI N45.2 standards for oversight of nuclear power quality
assurance activities including Access Authorization, Security, and Emergency Preparedness. Participation and or
leadership in audits of Nuclear Quality Assurance programs at multiple nuclear power plants.
 Nuclear Oversight Supervisor (August, 2005, through September, 2008). Day-to-day, quarterly, and long range
activity responsibilities for the program and providing staff needs performance evaluation, personnel
development, training, and certification. Collateral responsibilities of taking care of Quality Control program
and inspection personnel.
 Quality Assurance Topical Report Implementation Leader. New Quality Assurance Program umbrella
implemented for fleet standardization for the Nuclear Management Company, LLC (NMC). Implementation
leader for a similar project of a standardized program for Florida Power & Light at Duane Arnold and then again
at Point Beach.

 Project Manager. Collateral responsibility. January, 2004, through April, 2005. Forced, Planned, and Refuel
Project Manager assignments involving scoping, procurement, contingency planning, resource allocation,
vendor acquisition, budgeting, and project closeout.
 Supplier Quality Group Leader. April, 2002, through November, 2002. Supplier Quality responsibilities began in
July, 2001, and was qualified to perform as a Lead Auditor for both NUPIC and NEI related audits of
Contractor/Vendors for Access Authorization and Fitness- for-Duty areas. Site implementation leader for
Nuclear Management Company fleet standardization of the Supplier Quality function.
 Process Lead Coordinator. January, 2000, through July, 2001. Orchestrated the assessment procedure rewrite
that eliminated audit functional area silos. Created cycle plans, made the quarterly plans, made
recommendations for resource assignments, monitored program implementation, and published the quarterly
reports. These were all collateral responsibilities while certified as an ANSI N45.2.23 Lead Auditor that helped
the organization recognize the need to improve in areas such as Radiation Protection, Emergency
Preparedness, Security, Procedures, Human Performance, Corrective Actions, and Maintenance.
May, 1988, through August, 1998: Quad Cities Nuclear Power Station. Cordova, Illinois. Commonwealth Edison Company
(now called Exelon). Significant responsibilities assigned and accomplished during this period included:
 Nuclear Oversight. February, 1995, through August, 1998. Internal auditor certified to NQA-1 1989 standards.
Auditing occurred within Commonwealth Edison system to include the corporate organization, Quad Cities,
Braidwood, Dresden, LaSalle, and Zion Stations. External oversight assignments included Ginna Station.
 Operations. May, 1988, through February, 1995. Non-Licensed Operator responsibilities to include fire
brigade, hazardous material responder, and RadWaste Operator. Began employment at the Quad Cities Station
as an entry-level plant laborer and worked my way through Operations and then into Nuclear Oversight.
Completed an undergraduate degree while working rotating swing shifts and supporting refuel outage overtime
schedules.
January, 1982, through January, 1988: United States Navy. USS Paul F. Foster, DD964.
Gas Turbine Systems Mechanical Technician (First Class). Supervised personnel in main propulsion and electrical
generation using gas turbines and their support systems. Planned maintenance schedules to include long-term repair
activities that interacted with outside contractors. Also responsible for training and evaluation on the Engineering
Casualty Control and Damage Control teams. Achievements include Honorable Discharge with Good Conduct, Navy
Achievement Medal, recommendation for an Officer's Commissioning Program, and held Leading Petty Officer and
Work Center Supervisor responsibilities.

requirements for training, conference and local chapter attendance, and self-directed learning. Maintaining these

Project Quality Manager and general nuclear quality assurance managerial, supervisory, and oversight assignments. July, 2014 through October, 2014, Bechtel Power Corporation Holding status that allowed retention of benefits while waiting for assignment opportunity. From April, 2014 through June, 2014, Project Quality Manager assignment for the Panda Sherman Power Project (combined-cycle combustion turbine power plant) reporting to the Project Manager during the completion stages of the project. Responsibilities included staff assignments for KASE (Key Activities for Successful Execution) Gate development and closeout, resolution of disputed items with the client, punch list performance, and Closed Loop Corrective Action process implementation. From August, 2013 through April, 2014, Nightshift Quality representative for the Steam Generator Replacement Project at the Davis-Besse Power Plant in Oak Harbor, Ohio.  Provided the project with opportunities to capture lessons learned and act upon program improvements through use of the Closed Loop Corrective Action process.  Pursued a risk based graded approach to program implementation and effectiveness.  Provided motivation and performance recognition through the use of the Leader+ and Quality Coin programs.  Aided Environmental, Safety and Health in safety assessments and contributed daily to the behavioral safety observation program.

Kozich Page 2 of 3  Wrote several Quality Issues and Success Briefing sheets for Project leadership to present to the Bechtel organization.  Assisted Field Engineering in effective subcontract monitoring and execution.  Delivered responsible liaison results with the client’s Quality Assurance oversight personnel in preventing the escalation of undesirable conditions. From November, 2012 through August, 2013, held a temporary assignment at the Corporate Headquarters in Fredrick, Maryland performing various analyses, evaluations, and Supplier Quality Assurance Program reviews. Background and capabilities had allowed the initial temporary assignment to be extended after successfully performing such assignments as: gap analysis of Canadian Nuclear Power Standards against the Bechtel Nuclear Quality Assurance Program, performing as the Lead Auditor for the Quality Management System audit of the Bellefonte Nuclear Plant joint venture, apparent cause evaluation for the small modular reactor project, root cause evaluation assignments regarding engineering deliverables and corrective action timeliness, and ISO 9001-2008 audit team member at a California solar power facility. Was an instrumental team contributor to the Bechtel Construction Operations Incorporated application and ASME re-certification Survey for Certification Mark authorizations representing N, NA, NPT, NS, (Divisions 1 and 2), and the National Board Inspection Code NR Stamp for repair and replacement. From September, 2008, through November, 2012, assignment was at the Watts Bar Unit 2 Construction Completion project in Spring City, Tennessee. Advanced from Supervisor to Project Quality Assurance Manager in October, 2011. Responsible for the direction, control, and implementation of ASME Section III Division 1 and Safety-Related/Quality- Related Quality Assurance Program activities. Managed 26 Quality Assurance Engineers and administrative support in addition to administration of the Corrective Action Program. Responsible for the successful ASME re-certification Surveys for N, NA, and NPT Certification Mark authorizations in 2012. Prior to this advancement, responsibilities had included:  Primary point of contact and oversight of client subcontractors performing project ASME and Safety-Related project work that did not have a contract with Bechtel.  Quality Engineering Supervisor for independent in-line reviews of procedures, procurement requisitions, work orders, Quality Control inspection points, and Authorized Nuclear Inspector interface. Also accomplished the development and revision of the department’s procedures and quality assurance manuals.  Quality Assurance Audits and Surveillances Supervisor for ASME, Safety-Related, and Quality Related programs.  Certified Audit Team Leader accomplishing NQA-1, ANSI N45.2, and 10CFR73.56 (Access Authorization) related audits. August, 1998, through September, 2008: Duane Arnold Energy Center. Palo, Iowa. FPL Energy, LLC (now called NextEra Energy Resources). Significant responsibilities assigned and accomplished during this period included:  Certified Audit Team Leader to NQA-1 1994 and ANSI N45.2 standards for oversight of nuclear power quality assurance activities including Access Authorization, Security, and Emergency Preparedness. Participation and or leadership in audits of Nuclear Quality Assurance programs at multiple nuclear power plants.  Nuclear Oversight Supervisor (August, 2005, through September, 2008). Day-to-day, quarterly, and long range activity responsibilities for the program and providing staff needs performance evaluation, personnel development, training, and certification. Collateral responsibilities of taking care of Quality Control program and inspection personnel.  Quality Assurance Topical Report Implementation Leader. New Quality Assurance Program umbrella implemented for fleet standardization for the Nuclear Management Company, LLC (NMC). Implementation leader for a similar project of a standardized program for Florida Power & Light at Duane Arnold and then again at Point Beach.

Forced, Planned, and Refuel Project Manager assignments involving scoping, procurement, contingency planning, resource allocation, vendor acquisition, budgeting, and project closeout.  Supplier Quality Group Leader. April, 2002, through November, 2002. Supplier Quality responsibilities began in July, 2001, and was qualified to perform as a Lead Auditor for both NUPIC and NEI related audits of Contractor/Vendors for Access Authorization and Fitness- for-Duty areas. Site implementation leader for Nuclear Management Company fleet standardization of the Supplier Quality function.  Process Lead Coordinator. January, 2000, through July, 2001. Orchestrated the assessment procedure rewrite that eliminated audit functional area silos. Created cycle plans, made the quarterly plans, made recommendations for resource assignments, monitored program implementation, and published the quarterly reports. These were all collateral responsibilities while certified as an ANSI N45.2.23 Lead Auditor that helped the organization recognize the need to improve in areas such as Radiation Protection, Emergency Preparedness, Security, Procedures, Human Performance, Corrective Actions, and Maintenance. Non-Licensed Operator responsibilities to include fire brigade, hazardous material responder, and RadWaste Operator. Began employment at the Quad Cities Station as an entry-level plant laborer and worked my way through Operations and then into Nuclear Oversight. Completed an undergraduate degree while working rotating swing shifts and supporting refuel outage overtime schedules. January, 1982, through January, 1988: United States Navy. USS Paul F. Foster, DD964. Gas Turbine Systems Mechanical Technician (First Class). Supervised personnel in main propulsion and electrical generation using gas turbines and their support systems. Planned maintenance schedules to include long-term repair activities that interacted with outside contractors.
