The document evaluates a new ostomy pouch product from ConvaTec called Esteem+ ConvaTec MoldableTM Technology. Three patients tested the product for 10 days each and found it provided powerful protection. All three patients appreciated the moldable disc technology and found the product easy to use and comfortable to wear for long periods without leaks. Both patients and nurses concluded the moldable technology perfectly fits the stoma to minimize leaks and skin complications.

The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

Today I’m very proud to have been selected to share my personal experience with a new product from ConvaTec.

We are aware that one of the challange that ostomy patients have to face is to preserve an healthy persitomal skin.
Therefore, from an Et nurse perspective, we always look for an ostomy device that offers a good skin protection preventing leakages under the skin barrier.
Of course, there are other features that are important as well, such as the ease of use and the confort during the wearing time.
My objective during the evaluation of this new product was to verify that all those needs were met by Esteem+ Plus Moldable.
Esteem Plus Moldable is a unique product in which we have the advantage of the moldable technology with the confort of a one-piece system.

Let’s go through a short description of the features of the product.
Esteem Plus moldable is designed with a moldable disc that is prepared by molding it with fingers. The customization of the disc allows a perfect fit of the adhesive around the stoma. The other part of the pouch has to be sticked on top if the disc, once molded.
The pouch is very light and flexible during the wearing time and its removal is in a one-time solution without any residue of hydrocolloid on the skin.
The moldable technology allows the product to be easy prepared, in fact no scissors are anymore needed. The rebounding memory of the adhesive create a perfect seal around the stoma that prevents leakages and the Durahesive layer lets the adhesive swallen, in order to maximize the skin protection.

During the product evaluation I enrolled 3 patients.
The enrollment last ten days using a clear drainable pouch.
The three patients involved have very different features.
The first one, Silvia, is a 26 year old ileostomy patient. She was recently operated but she was already using a cut-to-fit one piece product.
The second one, Carlo, is a 48 year old colostomate. He was operated more than 6 months before the enrollment and he was wearing a 2 piece system with flange.
- The last one is, Teresa, a very smart 73 years old lady. She is an ileostomate and the one piece moldable was her first product.

Silvia appreciated a lot this product because she didn’t expierience any leakages and this was very important for her because with the previous product she reported that sometime she had to face leakages under the skin barrier.
At the beginning of the evaluation she had a peristomal skin complication that improved with the use of this product, thanks to the perfect fit that prevented the contact with any liquid effluents.
She found the product easy to apply and confortable for her skin: she experienced a very clean removal with no residue of hydrocolloid.
She would like to change her previous product with Esteem Plus Moldable.

Carlo appraciated a lot to mold the product and prepare it.
He has very abbundant liquid effluents despite of he is a colostomate. The wearing time was 24 hours, a very good performance in this critical situation.
The patient was used to use the paste to seal the edges of the stoma and appracieted that with this new product, he didn’t need the paste thanks to the seal created by the moldable technology. In fact the paste is very sticky and not easy to remove from the skin.

Also Teresa loved to mold the device, she arrived to wear the pouch for 48 hours without any leakages. As this product was not available on the market she asked for another moldable product so she changed a one piece with a two piece system.

From the patients’ perspective I noticed that overall they were enthusiastic to mold the adhesive to prepare it.
All of them considered Esteem Plus Moldable easy to prepare and very confortable to wear.
Another benefit worth to mention is that the moldable technology avoid the use of the paste to seal the edges of the stoma.
2 patients really wanted this as their definitive product; just one preferred to stay with the previous one used because he preferred the drainage system.

From my perspective the great advantage of this product is that the moldable disc allows a maximum personalization around the stoma in order to minimize leakages.
From the experience of the patients involved in the evaluation, the wearing time was excellent considering that in Italy patients are allowed to use only 2 pouches a day.
In the removal phase patient were very confortable because the product was removed in one solution without any residue.
I want also to add that the training session is very important in order to teach patient to use the product properly.
