SOCIAL SERVICES “ The Department of Social Services is comprised of Master’s level and Licensed Clinical Social Workers that are assigned to the hospital inpatient units and outpatient clinics. In support of CHCC’s mission and values, social workers provide a range of services that promote physical, emotional and psychological well being.” CHCC social workers help guide and support patients and their families through the entire health care process. Social workers assist families with personal, emotional, financial and family problems that may affect a child’s health. Social workers help parents understand treatment plans and goals and offer crisis intervention, information and referral to community support agencies and other services. 
MISSION The mission of CHCC is “ to provide high quality, comprehensive healthcare services to children, regardless of their ability to pay, and to continuously improve the health and well-being of children.”
ORGANIZATIONAL STRUCTURE The hospital as a whole is overseen by the board of trustees.  The board of trustees then assign the President and the CEO; They are in charge of overseeing the day to day events of the hospital. Every department within the hospital then has a Vice President and a Director, who supervise the staff that is under each department they’re assigned to.

COMMUNITY CONTEXT CHCC fit’s into the community in the following ways: CHCC provides medical services to children ages 0-21 in the Central and San Joaquin Valley. Most of our patients are on Medi-Cal or the California Children Services (CCS) program. Social workers at CHCC are sensitive to cultural, social and religious beliefs.  Social workers partner with spiritual and chaplain services, interpreter services, child life, clinical psychology, rehabilitation and clinical nutrition to enhance the well-being of children.
SERVICES Psychosocial Assessments *Social workers spend time talking with the patient and their families to gain an understanding of the family culture, the child and parent’s response to the illness/injury, and any other factors to develop a plan to resolve any problems/challenges Crisis Intervention Supportive Counseling Grief Counseling Support Groups Conflict resolution/mediation with families Coordination/participation in patient/family care conferences Consultation with clinical staff Patient/family education Assistance with discharge planning & coordination Liaison with patient’s school Follow up on medical compliance Advocacy for public and private services Information/referrals Transportation arrangements/coordination Financial Assistance Supportive Services Concrete Services
POPULATIONS AT RISK The main population at risk that CHCC serves is children ages 0 – 21. We also serve single parents, undocumented parents, and families with different social issues.  Every patient admitted to the hospital or who becomes a patient to one of the outpatient clinics is automatically assigned to a social worker.
FUNDING CHCC is a not-for-profit, state-of-the-art children’s hospital. It is funded through: Medi-Cal Different foundations such as The National Hemophilia Foundation Guilds  The Guilds have been raising funds for Children's Hospital for 61 years. They are the largest financial contributors. Radiothon  Donations

We serve families from Modesto to Bakersfield and from the Sierra Nevada Mountains to the Coast. This encompasses 40,000 square miles in the state of California.
