Change Theory and Models for Change Intervention I selected a behavioral change theory as a framework to guide my practice problem intervention. The Theory of Reasoned Action (TRA) originated in the 1960s, has been used in past to guide clinical practice intervention in behavioral related changes (Chamberlain College of Nursing, 2020). For example, it has been used in smoking cessation, dieting, the use of seat belts, voting, breastfeeding, as well as HIV/AIDS intervention both in and outside the United States (Fishbein et al,1994 as cited in White et al., 2016). The theory of reasoned action has four key components. They are behavior, intention, attitude, and norms (White et al., 2016). The theory is based on the belief that people are rational creatures and that their willingness to change behaviors have a lot to do with their conscious commitment. The following variables according to Fishbein et al, (1994 as cited in White et al., 2016) are interrelated; They are 1. Behavior – what is needed to happen so that the person understands what has to be done to achieve change. 2. Intention – The person’s readiness to change the behavior. That behavior change depends on the last two components attitude and norms. 3. Attitude – what are the approval and disapproval feeling the person may have about the behavior. 4. Norms (subjective)– How does that person who is about to undertaking that particular behavioral change believes others will feel about the behavior. In this particular theory, there is no clear component for networking with the stakeholders during the phases of the practice changes. I was unable to find ways to evaluate the evidence. I was not able to identify any component for identifying nor addressing barriers to implementation. However, TRA has been implemented successfully in multiple clinical interventions to adopt healthy behavior. In a recent research study using TRA to understanding teen pregnancy in the American Indians communities, they found that the subject's attitude about using birth control and the favorable or unfavorable opinions of others indicate whether or not these teenagers used contraceptives (Dippel et al., 2017). For the practice intervention to be successful, the individuals have to be prepared to change their beliefs and attitudes towards obesity. The perspective participants, who are one of the key stakeholders in this project have to be vested in the plan. There need to be a desire to lost weight and work towards maintaining it. They also have to know that their support system, that is family and friends will be supportive throughout. For any change to occur, the door to communicating with all key stakeholders need to remain open throughout the implementing and evaluation phase of the practice change intervention and beyond. In this particular theory, there is no clear component for

evidence. I was not able to identify any component for
identifying nor addressing barriers to implementation. However,
TRA has been implemented successfully in multiple clinical
interventions to adopt healthy behavior. In a recent research
study using TRA to understanding teen pregnancy in the
American Indians communities, they found that the subject's
attitude about using birth control and the favorable or
unfavorable opinions of others indicate whether or not these

Social Cognitive Theory
This theory was introduced by Albert Bandura, which stated that individuals could learn positively by using interactive behavior, human dialogue, direct experiences, and common observations. The sole purpose of the theory is to comprehend and predict the individual or group behavior and to identify methods by which change or modification can be achieved. Stages of Change Theory
Prochaska and DiClementi’s Model of Behavior Change was initially established to target the customers that need a change in health behavior, especially smokers under therapy treatment. There were four stages of this theory at the start, but now it has five different stages along with additional consideration of multiple audiences rather than individual cases. These stages of change theory are as follows:
Precontemplation
refers to the condition in which an individual does not understand and unaware consciously or unconsciously of the fact that change is a necessity for him/her.
Contemplation
indicates the situation of the person who is well aware of the problem and started to think about changing his or her attitude.
Preparation for action
indicates whenever the individual is ready to accept the challenge to change the attitude and start preparing to change is considered as “the act of preparation.” This stage may be clear within 02 weeks after making the decision to change. The
action
starts with the engagement of an individual into the change activities and understands how to cope with the behavioral change.
Maintenance
is considered as the final stage, which varies with individuals, but normally it may take up to six months. Any change in behavior must be strengthened in order to sustain the change.
Appraisal of evidence
There is a lot of evidence that proved that this model or theory could create a difference in the health behavior of an individual. Prochaska, DiClemente, and Norcross defined ten procedures that can evaluate and inspire the movement across the stages, which include Re-evaluation of environmental activities, Individual self-freedom, Social freedom, Sense of dramatic relief, Awareness levitation, Re-evaluation of self-esteem, Improvement in the relationships, Strengthening of management, Incitement control and Counter conditioning. These are some of the processes that can be achieved through this model or theory. The evide.

In considering this question, I have decided that understanding the local health care system to implement EBP is similar to understanding the patient to implement interventions. The nurse must first assess the patient in order to implement appropriate interventions; local health care systems must be assessed in order to know where to start in implementing EBP.
In discussing this concept with my mentor, she seems to agree. One point she made is that we must first know the culture and level of EBP exposure, of the facility and staff. If the facility has not had much exposure, implementation must "start with the basics at a much slower pace" (Rosshirt, 2017, n.p.). After assessment, we can begin to determine interventions that will create staff buy-in, and lead to successful EBP implementation.
In conducting research for this post, sources seem to agree with the position that the system must be understood in order to use relevant strategies and interventions. Individual and organization culture are social systems that must be understood. Change and EBP implementation are complex so communication is essential. We must understand the current relationship between researchers and practitioners. Knowledge gaps are how we determine relevant interventions. Relevance is the first step to creating staff buy-in, and successful implementation. Factors that may effect implementation include organization size, staffing levels, resources, and facility location; we must understand those things before we develop any interventions. Titler (2008) posits that "the strength of evidence alone will not promote adoption" (pg. 11); we must make the evidence relevant to the system. For example, "clinicians tend to be more engaged in adopting patient safety initiatives when they understand the evidence base of the practice" (Titler, 2008, pg. 12), as opposed to adminstrators forcing it upon them.
What works for one agency may not work for another. Warren, et al. (2016) educates that different systems have different barriers, and need different types and levels of support. Demographics, suchs as Magnet designation, staff education level, and employee role, effect successful implementation of EBP practice. We "must consider the work environment and the culture...across the system" (Warren, et al., 2016, pg. 22) as well, when developing strategies to implement EBP.
In my change proposal, I will consider all of the things discussed in this post. Motivators must be considered, and staff will want to know "so what?". In assessing motivators and culture, it will put me in a better position to write a proposal relevant to my audience, which will inspire motivation, and lead to a more successful transition.

This document summarizes a research study that utilized the Transtheoretical Model (TTM) to examine the stages of change that smokers progress through when quitting smoking. The study tracked over 1,400 current smokers in Texas and Rhode Island and grouped them into precontemplation, contemplation, and preparation stages. The study found differences in smoking patterns and attitudes towards the pros and cons of smoking between the stages. Those further along were more active in quitting processes. The TTM was found to be an effective model for evaluating smoking cessation as it accounts for the time and process of behavior change. Widespread adoption of empirically-supported treatment innovations has the potential to improve effectiveness
of treatment received by individuals with substance use disorders. However, the process of disseminating such
innovations has been complex, slow, and difficult. We empirically describe the dissemination and adoption of
a treatment innovation—an alcohol-treatment preparatory therapeutic procedure based on motivational
interviewing (MI)—in the context of Rogers' (2003) five stages of innovation–decision process (knowledge, per-
suasion, decision, implementation and confirmation). To this end, 145 randomly-chosen outpatient addiction
treatment clinics in New York State received an onsite visit from a project trainer delivering one of three
randomly-assigned dissemination intensities: a 15-minute, a half-day or a full-day presentation. Across these
clinics, 141 primary administrators and 837 clinicians completed questionnaires assessing aspects of five innova-
tion–decision stages. At each clinic, questionnaire administration occurred immediately pre- and post-
dissemination, as well as 1 and 6 months after dissemination. Consistent with Rogers' theory, earlier stages of
the innovation–decision process predicted later stages. As hypothesized, dissemination intensity predicted clini-
cians' post-dissemination knowledge. Clinician baseline characteristics (including gender, pre-dissemination
knowledge regarding the MI preparatory technique, education, case load, beliefs regarding the nature of alcohol
problems, and beliefs and behavior with regard to therapeutic style) predicted knowledge and persuasion stage
variables. One baseline clinic characteristic (i.e., clinic mean beliefs and behavior regarding an MI-consistent ther-
apeutic style) predicted implementation stage variables. Findings suggest that dissemination strategies should

In Unit V, we addressed how theories and models such as the social cognitive theory, the health behavior
model, and the theory of planned behavior play a role in intervention planning. This unit, we will build on the
foundation of theories and models and look at how to be strategic in determining interventions. The reading
highlights the concerns that health educators should have as it relates to the design and evaluation process to
determine the successfulness of interventions for a given health behavior.
Intervention Strategizing
When a health educator is developing an intervention strategy to help with a particular health behavior, there
are a few key factors to consider: identifying the target population, selecting a setting, setting goals and
objectives, and identifying resources and a timeline. Each of these factors are a concern for health educators
when developing interventions (Powell et al., 2017).
Target population: Who are you planning the intervention for? Are there any special needs? For instance,
adolescents have special needs because they are in school during the day, so an intervention for them would
need to be after school, on the weekend, or through the school. An intervention for seniors should be held
during the day because seniors normally shy away from being out at dusk or dark. If the intervention were for
the working population, there would be better attendance in the evenings or weekends. The goal with
determining the population for the intervention is to think of alleviating any barriers that may affect most of the

The purpose of this investigation is to determine if the theory of reasoned action (TRA) and theory of planned
behavior (TPB) can retrospectively predict whether substance-abuse treatment providers encourage their
clients to use medicated-assisted treatment (MAT) as part of their treatment plan. Two-hundred and ten
substance-abuse treatment providers completed a survey measuring attitudes, subjective norms, perceived
behavioral control, intentions, and behavior. Results indicate that substance-abuse treatment providers have
very positive attitudes, neutral subjective norms, somewhat positive perceived behavioral control, somewhat
positive intentions toward recommending MAT as part of their clients' treatment plan, and were somewhat
likely to engage in the actual behavior. Further, the data fit both the TRA and TPB, but with the TPB model
having better fit and predictive power for this target audience and behavior. The theoretical and practical
implications for the developing messages for substance-abuse treatment providers and other health-care
professionals who provide treatment to patients with substance use disorders are discussed.

Evidence-based practice refers to the integration of clinical expertise, research evidence, and application of patient values during the process of making decisions for the care of clients and patients. The practices have been tested and studied comprehensively to ensure that they are conscientious and explicit for the provision of the best care to the patients (Baker & Tickle-Degnen 2014). In the case of the two previously exposed campaigns, project action and teens stopping AIDS in Sacramento there is need to assess the strategies put forward and evaluate whether they are in line with evidence-based practice.
A comparison and contrast of the campaign strategies with evidence-based practice
The strategies which were employed by the two campaigns have some similarity as well as differences in different perspectives. The campaign strategies were divided into phases which are similar to the evidence-based practice. For example, in teens stopping Aids campaign, the first phase is to describe the problem which had led to the conduction of the campaign (Fisher et al., 2014). Similarly, in evidence-based practice, the first thing which is done is to ask a question which provides the problem of the patient or client. Other similar phases include evaluation, planning, and selection of the best interventions.
On the other hand, there is some difference between the campaign's strategies and evidence based care. The two campaigns are employing a group of people who are involved in decision making who represent many groups in the population of interest. However, the nurse for example in evidence-based care is the one who uses their expertise and refers to the documented scientific evidence and then decide what is best for the patient but of course in collaboration with the patients who are being cared for (Rundle, 2014).
Adoption, implementation, and maintenance of the strategies proposed
Prevention of Aids transmission is not a simple task and therefore comprehensive and suitable strategies needed to be adopted, implemented and ensure that they remain active and in use in preventing further cases of infection. The primary focus strategy of the two campaigns was behavioral change. This aspect is supreme in determining whether the policies proposed will be adopted, implemented and maintained in the community or not (Kalichman, 2014).
Through heavy campaigning, the aim was to provide information about the disease, reduce stigma, enhance the accessibility of services, delaying the first onset of sex, reducing the number of sexual partners among others all of which are part of the behavior that needed to be changed. Widespread adoption of empirically-supported treatment innovations has the potential to improve effectiveness
of treatment received by individuals with substance use disorders. However, the process of disseminating such
innovations has been complex, slow, and difficult. We empirically describe the dissemination and adoption of
a treatment innovation—an alcohol-treatment preparatory therapeutic procedure based on motivational
interviewing (MI)—in the context of Rogers' (2003) five stages of innovation–decision process (knowledge, per-
suasion, decision, implementation and confirmation). To this end, 145 randomly-chosen outpatient addiction
treatment clinics in New York State received an onsite visit from a project trainer delivering one of three
randomly-assigned dissemination intensities: a 15-minute, a half-day or a full-day presentation. Across these
clinics, 141 primary administrators and 837 clinicians completed questionnaires assessing aspects of five innova-
tion–decision stages. At each clinic, questionnaire administration occurred immediately pre- and post-
dissemination, as well as 1 and 6 months after dissemination. Consistent with Rogers' theory, earlier stages of
the innovation–decision process predicted later stages. As hypothesized, dissemination intensity predicted clini-
cians' post-dissemination knowledge. Clinician baseline characteristics (including gender, pre-dissemination
knowledge regarding the MI preparatory technique, education, case load, beliefs regarding the nature of alcohol
problems, and beliefs and behavior with regard to therapeutic style) predicted knowledge and persuasion stage
variables. One baseline clinic characteristic (i.e., clinic mean beliefs and behavior regarding an MI-consistent ther-

highlights the concerns that health educators should have as it relates to the design and evaluation process to
determine the successfulness of interventions for a given health behavior.
Intervention Strategizing
When a health educator is developing an intervention strategy to help with a particular health behavior, there
are a few key factors to consider: identifying the target population, selecting a setting, setting goals and
objectives, and identifying resources and a timeline. Each of these factors are a concern for health educators
when developing interventions (Powell et al., 2017).
Target population: Who are you planning the intervention for? Are there any special needs? For instance,
adolescents have special needs because they are in school during the day, so an intervention for them would
need to be after school, on the weekend, or through the school. An intervention for seniors should be held
during the day because seniors normally shy away from being out at dusk or dark. If the intervention were for
the working population, there would be better attendance in the evenings or weekends. The goal with
determining the population for the intervention is to think of alleviating any barriers that may affect most of the
population (Powell et al., 2017).

The purpose of this investigation is to determine if the theory of reasoned action (TRA) and theory of planned
behavior (TPB) can retrospectively predict whether substance-abuse treatment providers encourage their
clients to use medicated-assisted treatment (MAT) as part of their treatment plan. Two-hundred and ten
substance-abuse treatment providers completed a survey measuring attitudes, subjective norms, perceived
behavioral control, intentions, and behavior. Results indicate that substance-abuse treatment providers have
very positive attitudes, neutral subjective norms, somewhat positive perceived behavioral control, somewhat
positive intentions toward recommending MAT as part of their clients' treatment plan, and were somewhat
likely to engage in the actual behavior. Further, the data fit both the TRA and TPB, but with the TPB model
having better fit and predictive power for this target audience and behavior. The theoretical and practical
implications for the developing messages for substance-abuse treatment providers and other health-care
professionals who provide treatment to patients with substance use disorders are discussed.

Background: Recent policy reforms encourage quality improvement (QI) innovations in primary care, but
practitioners lack clear guidance regarding spread inside organizations.
Purpose: We designed this study to identify how large organizations can facilitate intraorganizational spread of
QI innovations.
Methodology/Approach: We conducted ethnographic observation and interviews in a large, multispecialty,
community-based medical group that implemented three QI innovations across 10 primary care sites using a new
method for intraorganizational process development and spread. We compared quantitative outcomes achieved
through the group_s traditional versus new method, created a process model describing the steps in the new
method, and identified barriers and facilitators at each step.

Evidence-based practice refers to the integration of clinical expertise, research evidence, and application of patient values during the process of making decisions for the care of clients and patients. The practices have been tested and studied comprehensively to ensure that they are conscientious and explicit for the provision of the best care to the patients (Baker & Tickle-Degnen 2014). In the case of the two previously exposed campaigns, project action and teens stopping AIDS in Sacramento there is need to assess the strategies put forward and evaluate whether they are in line with evidence-based practice.
A comparison and contrast of the campaign strategies with evidence-based practice
The strategies which were employed by the two campaigns have some similarity as well as differences in different perspectives. The campaign strategies were divided into phases which are similar to the evidence-based practice. For example, in teens stopping Aids campaign, the first phase is to describe the problem which had led to the conduction of the campaign (Fisher et al., 2014). Similarly, in evidence-based practice, the first thing which is done is to ask a question which provides the problem of the patient or client. Other similar phases include evaluation, planning, and selection of the best interventions.
On the other hand, there is some difference between the campaign's strategies and evidence based care. The two campaigns are employing a group of people who are involved in decision making who represent many groups in the population of interest. However, the nurse for example in evidence-based care is the one who uses their expertise and refers to the documented scientific evidence and then decide what is best for the patient but of course in collaboration with the patients who are being cared for (Rundle, 2014).
Adoption, implementation, and maintenance of the strategies proposed
Prevention of Aids transmission is not a simple task and therefore comprehensive and suitable strategies needed to be adopted, implemented and ensure that they remain active and in use in preventing further cases of infection. The primary focus strategy of the two campaigns was behavioral change. This aspect is supreme in determining whether the policies proposed will be adopted, implemented and maintained in the community or not (Kalichman, 2014).
Through heavy campaigning, the aim was to provide information about the disease, reduce stigma, enhance the accessibility of services, delaying the first onset of sex, reducing the number of sexual partners among others all of which are part of the behavior that needed to be changed. His desire to have a family was a goal he decided he needed to pursue with someone else. He left Kimi six months ago for a much younger woman and filed for divorce. During college, she sought treatment because her roommate became alarmed by her issues around eating. She never told her parents about this and felt it was behind her. Her parents are Danish and value privacy. They always expected Kimi to be independent. Her lack of communication about her private life did not concern them. They are troubled by Robert’s behavior and consider his conspicuous infidelity as a poor reflection upon their family. Kimi has moved in with her parents while she and Robert are selling the house, which has upended the balance in their relationship. For a third case, choose one of these videos: •The Case of Reese: -Reese is a 44-year-old married African American female. Her parents live in another state, and she is their only child. Her father is a retired Marine Lieutenant Colonel who was stationed both in the United States and overseas while Reese was growing up. Why the Flint-Rock Cannot Fight Back
Sto-Way’-Na—Flint—was rich and powerful. His lodge was toward the sunrise. It was guarded by Squr-hein— Crane. He was the watcher. He watched from the top of a lone tree. When anybody approached, Crane would call out and warn Flint, and Flint would come out of his lodge and meet the visitor.
There was an open flat in front of the lodge. Flint met all his visitors there. Warriors and hunters came and bought flint for arrow-points and spear-heads. They paid Flint big prices for the privilege of chipping off the hard stone. Some who needed flint for their weapons were poor and could not buy. These poor persons Flint turned away.
Coyote heard about Flint and, as he wanted some arrow-points, he asked his squas-tenk’ to help him. Squas-tenk’ refused.
“Hurry, do what I ask, or I will throw you away and let the rain wash you— wash you cold,” said Coyote, and then the power gave him three rocks that were harder than the flint-rock. It also gave him a little dog that had only one ear. But this ear was sharp, like a knife; it was a knife- ear.
Then to his wife, Mole, Coyote said: “Go and make your underground trails in the flat where Sto-way’-na lives. When you have finished and see me talking with him, show yourself so we can see you.”
Then Coyote set out for Flint’s lodge. As he got near it, he had his power make a fog to cover the land, and thick fog spread over everything. Crane, the watcher, up in the lone tree, could not see Coyote. He did not know that Coyote was around.
Coyote climbed the tree and took Crane from his high perch and broke his neck. Crane had no time to cry out. Then Coyote went on to Flint’s lodge. He was almost there when Flint’s dog, Grizzly Bear, jumped out of the lodge and ran toward him.
Coyote was not scared, and he yelled at Flint: “Stop your grizzly bear dog! Stop him, or my dog will kill him.”
That amused Flint, who was looking through the doorway. He saw that Coyote’s one-eared dog was very small, hardly a mouthful for Grizzly Bear. Fli.

The Theory of Reasoned Action (TRA) originated in the 1960s, has been used in past to guide clinical practice intervention in behavioral related changes (Chamberlain College of Nursing, 2020). For example, it has been used in smoking cessation, dieting, the use of seat belts, voting, breastfeeding, as well as HIV/AIDS intervention both in and outside the United States (Fishbein et al,1994 as cited in White et al., 2016). The theory of reasoned action has four key components. They are behavior, intention, attitude, and norms (White et al., 2016). The theory is based on the belief that people are rational creatures and that their willingness to change behaviors have a lot to do with their conscious commitment. The following variables according to Fishbein et al, (1994 as cited in White et al., 2016) are interrelated; They are 1. Behavior – what is needed to happen so that the person understands what has to be done to achieve change. 2. Intention – The person’s readiness to change the behavior. That behavior change depends on the last two components attitude and norms. 3. Attitude – what are the approval and disapproval feeling the person may have about the behavior. 4. Norms (subjective)– How does that person who is about to undertaking that particular behavioral change believes others
- 2. will feel about the behavior. In this particular theory, there is no clear component for networking with the stakeholders during the phases of the practice changes. I was unable to find ways to evaluate the evidence. I was not able to identify any component for identifying nor addressing barriers to implementation. However, TRA has been implemented successfully in multiple clinical interventions to adopt healthy behavior. In a recent research study using TRA to understanding teen pregnancy in the American Indians communities, they found that the subject's attitude about using birth control and the favorable or unfavorable opinions of others indicate whether or not these teenagers used contraceptives (Dippel et al., 2017). For the practice intervention to be successful, the individuals have to be prepared to change their beliefs and attitudes towards obesity. The perspective participants, who are one of the key stakeholders in this project have to be vested in the plan. There need to be a desire to lost weight and work towards maintaining it. They also have to know that their support system, that is family and friends will be supportive throughout. For any change to occur, the door to communicating with all key stakeholders need to remain open throughout the implementing and evaluation phase of the practice change intervention and beyond.
