with necessary healthcare services to maintain optimum health by maintaining consistent telephonic
contact.
 Complete health surveys and assessments with members telephonically in order to determine
healthcare needs.
 Develop health goals with members using motivational interviewing skills and provide coaching and
education to members in order to assist members in reaching their goals.
