• Completes a comprehensive case management assessment by gathering pertinent information
about the member/patient’s needs. Coordinates care and services so that the member/patient can
access them easily and quickly to get the best overall care possible. Is responsible for interdisciplinary
individualized careplans to assist member/patient to obtain optimum health and self-management goals.
Identifies member’s/patient barriers of care and implement appropriate intervention and goals within
