 Assesses for eligibility of case management services

 Process release of information
 Plans, coordinates and reviews all medical records Agency-wide to ensure that service is
documented for each individual client.
 Ensures that record documentation occurs within the time frame as stated in state and
federal standards and New Horizons policy and procedure.
 Ensures that all forms utilized for clinical documentation was approved by New Horizons
to meet the current regulation requirements for reimbursement by third party payers and
other governing bodies.
 Provides resource assistance to staff with regard to location of deficient parts in the
medical records when necessary
 Assist in internal and off-site audits on a monthly basis
 Purge medical records to send to storage and assemble records
 Assist clients when requesting medical records

 Trains and supervise staff in absence of the director
 Ability to process work, error free, with prescribed deadline and time frames.
 Ability to be courteous, tactful and cooperative throughout the working day.
 Fluent in English, and Creole
 Demonstrate independent judgment, ethical behavior, and maintain patient confidentiality,
as according to HIPPA
 Ability to concentrate and maintain accuracy in spite of frequents interruptions.
