This journal club presents a case of prosthetic
rehabilitation of an amputated thumb. It emphasizes that
prosthetic replacement is a better option for aesthetic and
psychological improvement, particularly in cases where the
victim is unwilling to undergo complicated surgical procedures for reconstruction of thumb or where functioning
of thumb cannot be restored even by multiple surgeries. In
the present case, a 20 years old female patient, with
missing thumb of her right hand was rehabilitated aesthetically by a non-invasive and cost effective prosthetic
procedure by using heat temperature vulcanizing silicone
material. The prosthesis (the thumb) was attached using
medical adhesives. On 3 months recall appointment, no
complications were observed. The prosthesis was in good
shape and required no further intervention. The prosthetic
thumb lacks the sensation of a normal or reconstructed
thumb, although it does not require the multiple procedures
of surgical reconstruction and the accompanying loss of
time for rehabilitation and healing
When treating a patient with a removable partial denture, the natural and artificial teeth, both functionally and esthetically, must co-exist in a harmonious relationship.
Occlusal harmony between a removable partial denture and the remaining natural teeth is a major factor in preservation of the surrounding structures.
In removable partial dentures, because of the attachment of the denture to abutment teeth, occlusal stresses can be transmitted directly to the abutment teeth and other supporting structures, which results in sustained stresses that may be more damaging than those transient stresses found in complete dentures.

This clinical report describes the prosthodontic rehabilitation andfabrication of an obturator used for a partially edentulous patient with anacquired unilateral maxillary defect.
The impression was made in 2 steps in a similar manner as the altered castimpression technique.
Precise impression of the resection defect despite trismus was possible, thus providing this patient an accurately fitting obturator prosthesis.

Fabrication of removable palatal augmentation prosthesis on a complete denture to reduce weight and maintain hygiene
The retention of a palatal augmentation prosthesis (PAP) is negatively affected by its weight thus, making this device as light as possible is important for clinical success.
However, hollowing the device to reduce weight may cause hygiene issues due to moisture intrusion.
An alternative technique with a removable veneer-type PAP for a complete denture was developed.
This resulted in positive outcomes in terms of reducing the weight and maintaining the hygiene of the prosthesis.
There have been several changes since inception in the field of dental ceramics. Need for newer materials with improved aesthetics, flexural strength and optical properties made it necessary for introduction of advanced technology in fabrication of dental ceramics.
The threshold between microstomia and a normal mouth
opening is defined as an interlabial measurement less
than 45 mm and/or an interincisal distance less than 40
mm.20 Zweifel et al6 further refined that statement by
defining the average vertical mouth opening as 40 to 50
mm, a functional opening as 25 to 35 mm, and a severely
limited opening as 10 to 24 mm. However, no definitive
description has been presented of the width of mouth
opening that should prompt the use of a foldable or
sectional denture.
Mouth opening exercises have been described for
patients with scleroderma but not for those with facial
exercises and appliances used long after facial burns is
scarce, but exercises may be of value in the treatment of
these patients.
The use of sectional trays and record bases may be
essential to obtaining accurate impressions of a patient
with microstomia. An accurate tray and record base is
required to consistently obtain the position of the tray
and record base related to the reference anatomic
structures.
While maxillary bone resorbs toward the center of the
maxilla, mandibular bone resorbs laterally in posterior
areas and lingually in the anterior region. This may allow
the fabrication of maxillary dentures with a smaller width.
To provide support, however, the buccal flanges of
mandibular complete dentures should overlay the bone
horizontally at the buccal shelf region, which may extend
buccally 10 mm or more from the residual ridge top.
These factors explain the presence of several mandibular
collapsible or foldable dentures in the literature versus
few articles describing techniques for maxillary sectional
dentures. The use of implants may allow less extension of
the borders, providing comfort and ease of use for
patients with microstomia.26

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

NVBDCP was launched in 2003-2004 . Vector-Borne Disease: Disease that results from an infection transmitted to humans and other animals by blood-feeding arthropods, such as mosquitoes, ticks, and fleas. 4. Anatomy of Dimple
Dimples on the face are commonly situated on the
cheeks andchin.
Anatomically, dimples are thought to be caused by
insertion of fascial bands of zygomaticus major
muscle into dermis or dermal tethering effect of
zygomaticus major muscle which may be bifid.
Studies of human facial anatomy of the dimple
demonstrated that dimple is caused by an abnormal
insertionofthemuscles oftheface.
It is usually formed by a small defect in the
buccinator muscle of cheek, such as dermocutaneous
insertion of the fibers on the inferior bundle of the
bifidzygomaticusmajormuscle.
5. Determination of Location Of Dimple
Shaker et al. proposed a simple technique for
creation of artificialfacialdimple.
According to their technique, they asked the
patients to smile and marked the dimple at a
distance of 2–2.5 cm lateral to the nasolabial
fold at the same level or slightly above the angle
of the mouth.
This dimpleplasty surgery can be done
irrespective of anesthesia, i.e., local or general
anesthesia

from the cosmetic viewpoints;
it is the point of intersection of two lines, a
perpendicular drawn from the external canthus
and another drawn horizontally from the angle of

cavity by asking the patient to suck the cheek

7. Surgical Approach To Dimple
Creation
Many techniques are developed to create dimple artificially; some are discussed below.
Bao et al. reported a technique; in this they guided a monofilament sutre material using a
syringe needle through the dermis and the active facial muscles such as the buccinators and
forming a sling between the skin and it. Dimple is created by tying knot.
The latest method of dimple creation was elicited by Rahpeyma
and Khajehahmadi in 2014; they proposed a conservative
approach for facial dimpling after elimination of the
odontogenic infection in the facial space area andit is a novel
technique that ease the removal of odontogenic infection in the
masseteric space and the buccal space and also gives esthetics
by creation of dimple in the defect area.
9. Complications Associated
Dimple surgery like any other surgicalprocedure is associated with certain
complications such as:
• Mildpostoperative swelling, postoperative hemorrhage.
• Buccalbranch of facialnerve injury, scar formation at operated area.
• Abscess formation, and foreign body granuloma.
• Dimples weakness of the involved muscles after the surgery.
• Judicious use of antibiotics and proper maintenance oforal hygiene
reduce the riskof complications
10. Recovery And Precautions After Surgery
Dimple creation is a day case procedure that takes 20 – 30 minutes.
Recovery depends on age, physical condition, and quality of post-
operative care. Some minimal swelling can be experienced following the
procedure, however, this subsides in a few days. Patient is prescribed
pain and antibiotic medications, and antiseptic mouth rinses to help
copewith painandpreventinfections.
He/She may start eating soft foods a few hours after the effect of your
local anaesthesia has worn off. It is better to avoid hard and spicy food.
Patient can begin all normal activities the next morning following the
procedure. The surgical wounds will heal in one to two weeks, and will
notice a more naturally appearing dimple. The dimples may appear at its
bestin thenext2 to3 months.

Having a perfect nose, perfect ears, and scar less face is asaesthetic as
having a well and perfectly shaped dimple. It can be naturalor well-crafted
by an oral and maxillofacialsurgeon, cosmetic surgeon. Hence, dimple
surgery is very safeprocedure and canbe completed asan outpatient
procedure in less time with minimum discomforts. The various described
procedures for placing the dimple in cheeks are very simple and easy to
perform by the surgeons and canimpart a bold and attractive facial
aesthetics.
Celebrities like Preity Zinta, Deepika Padukone and Kate Middleton have million dollar smiles thanks to their trademark dimples. It’s an enviable look that even if you weren’t genetically given can still be yours with a simple procedure called dimpleplasty.
