The following is a brief, highly incomplete case study of a fellow who presents with symptoms of a specific psychiatric illness. He is interviewed and given some IQ testing. IQ testing would not be the best test to understand his problems, in fact it might be contraindicated in his current state but for the purposes of the assignment, IQ testing provides the clearest example of data and its use in a critical thinking exercise which is one of the goals of this assignment.
CASE STUDY
Mr. A. Soprano is a 55-year-old white male who presented as an “emergency” to a psychologist at a local ER. After establishing rules of confidentiality he revealed he was a high level crime boss; essentially a mobster. Throughout the interview he spoke in a rather pronounced Brooklyn accent. He preferred to call himself “a highly creative business man.” The client’s chief complaint was a concern over a vague desire to “hurt” himself, sleep problems and was experiencing disturbing “thoughts that don’t make any sense.” He stated his thinking had become very negative, “and weighs heavy on me”, where he doesn’t see anything good or positive in his life, “and I’m normally a positive kinda guy.” Dr. Psychologist proceeded to request the client cooperate with psychological testing to help determine potential causes for the client’s reported problems. This included a clinical interview.

Week One Assignment Scenario
Suzette is a 47-year old Caucasian female who has been married for 24 years, and has three adolescent children. Her spouse was laid off from his job of 15 years, and she has had to seek part-time work in the retail business. She works 25 hours a week. They own a home, but have been unable to make their mortgage payments for the past 7 months. She states that she always felt they went beyond their means when purchasing this large house. Her spouse has been despondent since his job loss and is having difficulty finding employment in his field, spending most days at home doing very little. He has turned down several jobs, because he considered them “beneath” his skill level, and has started drinking more heavily in recent months. Their children are in school and appear fairly well-adjusted.
Suzette has suffered from headaches for many years of her life and their frequency has increased to the extent that she has headaches “more days than not.” In the past year, she has had occasional shortness of breath and difficulty sleeping. She falls asleep within 15 minutes but frequently wakes up “at 2 or 3 a.m.” and is unable to fall back to sleep. This pattern has worsened and she claims she dreads going to bed because of the worries she has and her fear of not sleeping well.
She has good medical insurance because of her spouse’s COBRA plan, and she has regular visits with her primary care physician. Her medical history includes breast surgery for benign cysts, and she has a history of tachycardia and chronic lumbar pain. She also has evidence of early menopause. Her current medications include Ativan for anxiety and sleep, hormone replacement therapy, and low doses of Naproxen for back pain. She resists taking the Ativan, because it makes her feel “foggy” during the day.
She is of normal weight for her height, and her blood pressure is normal. Her eating patterns are reduced, since she says, “I’m just not very hungry.” There has been no major weight loss or gain in 5 years. She does not use alcohol or other drugs. She does not smoke. Sexual interest and desire have decreased significantly in the past 4 years. Her level of physical exercise is minimal and she states that she used to go to a gym, but finances forced her to quit. She claims that exercise always made her “feel good.” Since then, she spends most of her time at work or home, and has no time to exercise. Her mental status is normal, aside from moderate anxiety. Her level of social support is reduced, because she has few friends and her relationship with her spouse has become distant.
Her major complaint is her insomnia. She has significant daytime sleepiness and often falls asleep while watching TV in the evening. She claims that she falls asleep almost instantly when her head hits the pillow at night, but she wakes up three or four times and usually cannot fall back asleep.

Stella is a 40-year-old woman who is my client. Her eyes are expressionless as she gives an apology for being late for her appointment. On asking her where she was coming from she states that she had gone to see her nephew whom she had taken a few of her electronics she no longer thought she needed and he was better placed to use them. While she appears and admits to still being sad even after letting go of her electronics, she states that the sadness does not compare as to how she felt when she still had the items in her possession. Stella is recently divorced and had lost custody of her two children after being a stay at home mom for more than ten years.
It has been three months since her divorce and contrary to her wishes, we try avoiding talking about her divorce. We discuss her teenage children’s achievements over the years that she believes she contributed greatly. Mark is an achieving athlete while Ann plays the piano with talented mastery. This however, reminds her that she lost custody and her face is filled with the hopelessness we have been trying to help her see is not necessary. She states that she sees no need to continue living without her children whom she has been forced to stay states away from. Upon her divorce and loss of custody, she has had to move back to her parents’ as being unemployed she could not afford to live on her own. She explains she gave up accounting 12years ago to be a hands-on mom and upon inquiring whether she would like to go back to it she starts sobbing unstoppably.
Since our time was moving fast, there was little we could do amidst her sobs and I chose to let her cry as many tears as she could. After calming down she stated that all she knows how to do is take care of her children’s needs by preparing their meals, attending their games and play sessions, and tend to them when they were sick. At this point, she breaks down into tears stating that there was really no need to keep acting strong as she was feeling very weak and unable to take it all in. it is at this point that I notice the marks on her wrists.

I believe that, Stella Matthew could potentially be a suicidal patient whom as much as they have not stated the need to end their life, exhibits symptoms and behaviors of one who has, or is about to start having suicidal thoughts. One of the reasons to believe so are her explicit mood changes and extreme sadness, while she is okay one minute she could be sobbing the next and avoiding eye contact or refusing to talk completely in the other minute (Panagioti et al, 2012). For instance, in this session, she kept sobbing and at some point she refused to continue talking citing that she was tired from having .

Your analysis begins with a generic description of the problems associated with intangibility. Remember that this assignment is about the application of theory rather than description. The remainder of your analysis highlights some of the issues in the case study. However, there is no application of theory. You could have used any one of a number of frameworks to frame your analysis and give it greater depth: Servuction model, Flower of Service, Gaps model, etc. Furthermore, I would like to see you explain the reasons for these problems. In this case study, the lack of service culture is a significant issue.
With regard to recommendations, it would again be better if you approach the task in a more systematic way rather than making random suggestions with no indication as to which problems these activities will resolve. For example, are testimonials a priority? Given that the service being offered is poor, there may not be many customers with anything positive to say. Wouldn’t it be better to fix underlying issues first? You recommend personal selling and communications but there is a lack of detail and no explanation as to what problems these activities would address. You recommend that the company needs to act on the feedback it receives (page 6), but you do not say how. Similarly, recommendations like personalised service might be relevant but you do not say what this means in practice.
Overall, there is a lack of application of theory to this response. You need to look back over the lecture slides and use appropriate models to frame your analysis and recommendations. While what you write is sound, it does not directly address the case study. There is insufficient detail.

Her spouse has been despondent since his job loss and is having difficulty finding employment in his field, spending most days at home doing very little. He has turned down several jobs, because he considered them “beneath” his skill level, and has started drinking more heavily in recent months. Their children are in school and appear fairly well-adjusted.
Suzette has suffered from headaches for many years of her life and their frequency has increased to the extent that she has headaches “more days than not.” In the past year, she has had occasional shortness of breath and difficulty sleeping. She falls asleep within 15 minutes but frequently wakes up “at 2 or 3 a.m.” and is unable to fall back to sleep. This pattern has worsened and she claims she dreads going to bed because of the worries she has and her fear of not sleeping well.
She has good medical insurance because of her spouse’s COBRA plan, and she has regular visits with her primary care physician. Her medical history includes breast surgery for benign cysts, and she has a history of tachycardia and chronic lumbar pain. She also has evidence of early menopause. Her current medications include Ativan for anxiety and sleep, hormone replacement therapy, and low doses of Naproxen for back pain. She resists taking the Ativan, because it makes her feel “foggy” during the day.
She is of normal weight for her height, and her blood pressure is normal. Her eating patterns are reduced, since she says, “I’m just not very hungry.” There has been no major weight loss or gain in 5 years. She does not use alcohol or other drugs. She does not smoke. Sexual interest and desire have decreased significantly in the past 4 years. Her level of physical exercise is minimal and she states that she used to go to a gym, but finances forced her to quit. She claims that exercise always made her “feel good.” Since then, she spends most of her time at work or home, and has no time to exercise. Her mental status is normal, aside from moderate anxiety. Her level of social support is reduced, because she has few friends and her relationship with her spouse has become distant.
Her major complaint is her insomnia. She has significant daytime sleepiness and often falls asleep while watching TV in the evening. She claims that she falls asleep almost instantly when her head hits the pillow at night, but she wakes up three or four times and usually cannot fall back asleep.

Stella is a 40-year-old woman who is my client. Her eyes are expressionless as she gives an apology for being late for her appointment. Your introduction is not terribly relevant. You would do better to keep it simple and signpost the reader.
Your analysis begins with a generic description of the problems associated with intangibility. Remember that this assignment is about the application of theory rather than description. The remainder of your analysis highlights some of the issues in the case study. However, there is no application of theory. You could have used any one of a number of frameworks to frame your analysis and give it greater depth: Servuction model, Flower of Service, Gaps model, etc. Furthermore, I would like to see you explain the reasons for these problems. In this case study, the lack of service culture is a significant issue.
With regard to recommendations, it would again be better if you approach the task in a more systematic way rather than making random suggestions with no indication as to which problems these activities will resolve. For example, are testimonials a priority? Given that the service being offered is poor, there may not be many customers with anything positive to say. Wouldn’t it be better to fix underlying issues first? You recommend personal selling and communications but there is a lack of detail and no explanation as to what problems these activities would address. You recommend that the company needs to act on the feedback it receives (page 6), but you do not say how. Similarly, recommendations like personalised service might be relevant but you do not say what this means in practice.
Overall, there is a lack of application of theory to this response. You need to look back over the lecture slides and use appropriate models to frame your analysis and recommendations. While what you write is sound, it does not directly address the case study. There is insufficient detail.

These two conceivably innocuous items triggered a flood of articles, petitions, and videos, denouncing their perceived underlying messages.
The first controversy that erupted surrounded the Bic Cristal For Her pen. This pen was created and packaged specifically for women to use. Several groups lashed out at Bic, calling their attempt to target women with "lady pens" sexist and demeaning. Its detractors felt the campaign was degrading and fed into stereotypes by highlighting the thin design and the use of pastel colors. The negative press was overwhelming, although the pens have remained on the market.
Consumers also targeted those responsible for marketing the Easy Bake Oven by sending a petition asking its parent company Hasbro to make the ovens in colors other than pink and purple. Thousands of individuals signed the petition asking for alternative oven colors after a teenage girl from New Jersey was angered that her younger brother would have no other option but to use an oven in the colors that are considered stereotypically female. It was argued that the colors supported the stereotypical view that only young girls would want to bake. The signers of the petition felt that young boys who might want to use the toy would be more likely to practice their baking skills if the color of the oven was gender neutral.

This week, you viewed a video with historical footage of immigrants arriving at Ellis Island in New York Harbor. If you have not viewed this video, stop and do so now—or watch it again, this time with the perspective of what it would feel like to be in that scene. You may have relatives that arrived through Ellis Island or a similar entry point in other parts of the country. Imagine being in that moment, having endured great hardship, and then facing a test that checks your intelligence—and for no small reason. It will determine whether you can enter the United States.
Based on what you are learning about culture, is it likely that test would be fair in assessing the knowledge of those who speak languages other than English and have wildly different sets of experiences? Additionally, different cultures have different definitions of intelligence. Some cultures integrate movement and emotions into the concept of intelligence. Other cultures value analytical thinking. Still others highlight nonverbal communication. Sadly, many Ellis Island immigrants were labeled “feeble-minded” by extremely biased intelligence tests and, after all those hardships, were refused entry into America.
You will return to the issue of bias by examining how intelligence tests can favor some cultures over others. Through the Learning Resources, you will experience
two
types of intelligence tests and imagine how you might do if each test had major stakes for you—such as whether you could vote, be accepted by a college, or be allowed to enter a country. Intelligence tests have been used for those and other purposes—and sometimes with the express purpose of excluding some groups over others.
For this Assignment, you will examine how culture influences the definition of “intelligence” and what is considered “common knowledge” and analyze your own experiences with differing cultural views of these concepts.
To prepare:
Review the Week 2 Learning Resources, with particular attention to Chapter 5 in the course text and the media piece on Ellis Island immigration.
Take the
two
short assigned tests, “The Original Australian Test of Intelligence” and “The Chitling Intelligence Test,” in the Learning Resources. Note the tests cannot be filled in online. Use paper and pencil to record your answers, and then click the scoring sheet link to check your results.
Consider how well you did on each test. What kind of knowledge was needed to do well, and how “common” is that knowledge? Consider questions in both tests that stand out to you as examples of cultural bias.
Based on your experience, consider what conclusions you can draw about the meaning of “intelligence” and “common knowledge” across cultures and the potential for constructing an intelligence test without cultural bias.
Consider the various views of intelligence discussed in your Learning Resources and consider how they are similar or different from your own views.
