1. Appraisers Note Taking Template 2016
Start of Year Meeting
1. Greeting and pleasantries
2. Appraisal System and procedure 2016
Do you have any questions about how appraisal will work this year? Have you imported the template into your
wordpress account? How have you been storing evidence of good practice up to now? Are you aware your job
descriptions are based on the 12 RTC, are you familiar with them? James has a well established blog, where he
collects and collates evidence. Regular blogging is evident. There are blog posts and entries across all the
RTCs with bundles of evidence and reflections. As a PRT James will work with one of the SCTs on ensuring his
blog is fit for registration.
3. Affirming Feedback- feel good/affirmation of good teaching
Can you identify a time in Term 1 when a moment/activity/event that went really well? What happened? What
did you do that made it go so well? anything else? What are you going to do to bring this strength/good practice
out more this year? This last year James has been working on building really good Learning Relationships with
the students in his class. As a result of this, some of James’s students compiled an end of year booklet for him
about their experiences in their class. It’s clear this is built on a solid relationship. James is seeking to emulate
this type of professional but meaningful relationships moving forward. James also enjoyed success by the end
of last year around using positive attention within a rounded learning relationship to help manage behaviour.
For 2016 James would like to use this technique again for any troubled students that arise.
4. Student Achievement Data 2015 – view actual data analysis with teacher
Are you pleased with the achievement of your classes last year? How do your classes’ results compare to other
classes at OC and decile 9 averages? what results are you particularly pleased with? Why do you think they did
so well? What results are you disappointed with? Why do you think they didn’t do so well? What needs to be
done this year to make sure they achieve to a higher level? What are you going to do differently this year.
James was happy with his level one history results overall, with achievement mostly above the decile and
often at or above the department pass rate. James set up structures with his class around checkpointing and
making sure expectations were clear and specific. Overall pleasing results for level 1 history.
For level 2 Social Studies the results perhaps weren’t so pleasing. There were some personality issues in the
class, along with some students who were low achieving at an NCEA level. The class was generally more of a
struggle for James, so the structures that were in place for History didn’t work so well. For 2016 James isn’t
taking senior Social Studies, but would look toward greater clarity around structures and sign posting, letting
students and parents/whanau know earlier when there is an issue with keeping up to date for work. James
will also be more careful about NOT assuming he knows a class before he does, so that expectations are more
clear and perhaps less generous with privilege. Trust will need to be earned both ways. A more structured
start to the year.
5. Department Goals and TAI
Are you familiar with our department goals this year? What is your role in our TAI? Do you feel you know
enough about it and what strategies we are implementing? What data will you be collecting towards this?
6. Personal Teaching Goals/TAI for 2015
view final appraisal report form last year, student evaluation summary, achievement data and the completed RTC mapping exercise
and use the iSmart goal method to write goals
Based on what we have discussed so far, what do you want/need to focus on for this year? What have you
done already? So what would you like to be different? When would you like this to take place?
Can we shape this goal now?– By……, I will……… So that…….?(must be a student outcome), Lets write this
down.
2. To start the year at all levels with more structure and clear expectations and to maintain these systems more
efficiently.
Make better use of school support structures to help maintain student behaviour
Working more closely within the Ako model, by clarifying success criteria and implementing plenary’s – moving
towards a more explicit use of Ako
To refer more to the three story intellect and to overtly use these, both in lesson introductions/plenary’s AND
by referring to the new poster
Are there any other goals? (see the documents above to help identify needed goals)
Goal One: (attached to TAI)
By (timeframe)
I will have. (teacher practice)
So that (student outcomes/benefits)
7. SAMR Goal
Are you familiar with the 2 school wide goals? One of the strategies is for all staff to have a SAMR goal, what
would you like to focus on this year to move up the SAMR model? How will you do this? Let’s write it down –
By…., I will……, So that…….
By the end of term 2 for the internal assessment, I have written a fresh unit for history on War Atrocities so
that it includes students working towards transformative learning as outlined in the SAMR model. This will
include a student centred, flipped classroom model.

Thanks for your time today, is there anything I or anyone else can do to help you achieve your goals this year?
How would you like things to run this year? How often do you want me to visit your classes? Would you like me
to add evidence to your blog after observations etc? can you send me your appraisal blog link?
Keep a copy of these notes and give your appraisee a copy so that they can add this information to their
blog in the appropriate “bundles of evidence” section.
