The majority of existing methods of risk assessment of trunk oil pipelines is based on probability and classical set theory. These methods do not take into consideration the fact that any complex system, such as trunk oil pipelines, is a dynamic system with a set of uncertain data. The results of risk assessment (likelihood), obtained by the traditional methods, don’t reflect the real situation about the condition of system.
The article presents a methodical approach to identification of risk factors sources and assessment of the level of risk based on system analysis and fuzzy set theory, and this approach allows us to receive output conclusion about risk level in dynamic area of a complex system. The principles of fuzzy logic in the evaluation of diverse parameters of risk factors allow bringing them to a qualitative common denominator. As a result, we can perform comparative research of separate risk factors and also summarize them for assessment of current risk level of trunk oil pipelines.
We reviewed causes of incidents and failures on trunk oil pipelines and have identified the most important risk factors that have greatest consequences. Risk factors in our method are determined by fuzzy set rules, which are a result of data analysis (accident reports, failure records for the similar pipelines) and experts evaluation of related input parameters. Risk factors and their parameters are classified in accordance with their relation to different environment and life-cycle stages.
The article presents an example of evaluation of risk factor <development>. Consequently, the indicator of risk factor was calculated in accordance with procedure of Mamdani fuzzy inference system (using the MATLAB Fuzzy Logic Designer).
This method of risk assessment brings high opportunities for identification of the pipeline segments, where the risk level is most probably high and helps the decision maker to take remedial actions to mitigate potential risks. The operator can more effectively manage resources and improve efficiency of actions. Ability to reflect the current risk level is key advantage of our approach to risk assessment of risk management of trunk oil pipelines.

Abstract: The Preliminary hazard analysis (PHA) is used to essentially identify potential accidents related to the system and its interfaces to assess their probability of occurrence and the severity of the damage they may cause and finally propose solutions that will reduce, control or eliminate. Although essential in the process of analysis and safety evaluation of high-risk industrial system, the method PHA is very differently developed and remained unclear. This method is generally classified in theory by an inductive approach. However, in practice, a deductive approach is essentially used as the Fault tree analysis. To enhance the quality of the safety analysis in terms of completeness and consistency, we suggest a new hybrid method that combines these two modes of reasoning: induction and deduction. Indeed, the safety analysis of a complex system requires from experts in the field implementation of an iterative analysis process involving both inductive and deductive approaches. The ambition of this new method is to deconsolidate and renovate conventional approaches. In addition, this method is based on the use of a standardized vocabulary and a rigorous analytical approach likely to be accepted by all actors who participate in the development of safety documentation. Indeed, in accordance with national and European regulations and in particular the Railway Safety Directive, this paper proposes a generic approach for development assistance, assessment and prevention of risks which takes account of uses, theory and our experience in this domain.

This research aimed to analyze a fatal accident in a power plant construction project using the two methods of MORT and Tripod-Beta, and the comparison of the analyses. First, the report of the selected accident was studied, and the accident was analyzed by the two methods of MORT and Tripod-Beta. The next step was followed by the comparison and assessment of the methods of MORT and Tripod-Beta with the measures of time, cost, training needs, the need for technical forces, the number of causes identified, quantifiable, and the need for software to conduct analysis. The Tripod-Beta accident analysis cost less and requires less time, and less technical experts. Thorough analysis of major accidents needs to identify all the possible causes of the incident, including human error and equipment failure. Therefore, the complimentary use of both techniques of industrial accident analysis is recommended.

Methods for Risk Management of Mining Excavator through FMEA and FMECAtheijes
Management of maintenance systems in the mining industry is an important condition for their operation. If we recognize the need for risk analysis and management of individual maintenance system, it can generate potential overall efficiency and effectiveness. Special importance for the realization of the objectives of the mining industry belongs to redesign, system harmonization between the various technical structure, standardization, technical diagnostics, analysis of different levels of criticality with variant selection and application of optimal solutions. The potential for the destruction of the complex maintenance systems are a reality in the mining industry and their expression in various applications is a realistic one. For different aspects of the analysis it is possible to decrease risk index range from the threshold to the range of high and low threshold of moderate and acceptable risk. Application of FMEA (Failure Mode and Effect Analysis) and FMECA (Failure Modes, Effects and Criticality Analysis) methods are used to manage risk related to the initial phase of defining the prediction of all possible risks, risk factors and RPN budget priorities. Some risks can be grouped according to the type of errors that occur due to their realization. For effective risk analysis and implement measures to reduce their need is and competent team. No matter what the risks involved, FMECA method can reliably estimate the possibility of their implementation with a satisfactory degree of flexibility and compatibility. In this paper an attempt has been made to develop an effective maintenance methodology of excavator such that the maintenance cost is minimized and technical constraints (such as engine, hydraulic and transmission system, break system, electrical and safety system, suspension and track) are efficiently monitored and maintained. These technical constraints depends upon many factors such as a) Geotechnical parameters, b) Geological parameters, c) Mine parameters, d) Production rate, e) Equipment specification and f) Dig ability assessment etc. Based on the above factors maintenance plans are prepared. This paper discusses a risk management strategy system for Optimal Maintenance Program (OMP) of excavators. The OMP includes functional analysis method of FMEA and FMECA. To develop a successful operation system, it is first necessary to create a risk management program. A prudent management program is one that ensures safety and is environmentally and economically responsible.

Risk assessment on any facility is most efficiently done by first dividing the facility into components with
unchanging risk characteristics. For a cross-country pipeline, this involves collecting data on all portions of the
pipeline and its surroundings and then using this data to ‘dynamically segment’ the pipeline into segments of
varying length. Risk algorithms are applied to each of the segments, producing risk estimates that truly reflect
changing risks along the pipeline
The risk estimating algorithms are conceptually very straightforward. However, as with any assessment of a
complex mechanical system installed in a varying, natural environment, there are many details to consider. This is
illustrated by an example risk assessment on a hypothetical pipeline.
Varying levels of analyses rigor are available to risk assessors. For example, a resistance estimate might be
modeled as simply being related to stress level and pipe characteristics or, for more robust analyses, could include
sophisticated finite element analyses. In this example, a certain amount of detail is omitted in order to better
demonstrate the higher level principles.
To illustrate key concepts, one time-independent failure mechanism (third party damage) and one time-dependent
failure mechanism (external corrosion) are assessed. All other failure mechanisms will follow one of these two
forms. Estimates from all failure mechanisms can be combined in various ways to meet the needs of the
subsequent risk management processes.

FAILURES AND ACCIDENTS FOR TRUNK OIL PIPELINES

Abstract. The majority of existing methods of risk assessment of trunk oil
pipelines is based on probability and classical set theory. These methods do not
take into consideration the fact that any complex system, such as trunk oil
pipelines, is a dynamic system with a set of uncertain data. The results of risk
assessment (likelihood), obtained by the traditional methods, don’t reflect the real
situation about the condition of system.
The article presents a methodical approach to identification of risk factors
sources and assessment of the level of risk based on system analysis and fuzzy set
theory, and this approach allows us to receive output conclusion about risk level in
dynamic area of a complex system. The principles of fuzzy logic in the evaluation
of diverse parameters of risk factors allow bringing them to a qualitative common
denominator. As a result, we can perform comparative research of separate risk
factors and also summarize them for assessment of current risk level of trunk oil
pipelines.
We reviewed causes of incidents and failures on trunk oil pipelines and have
identified the most important risk factors that have greatest consequences. Risk
factors in our method are determined by fuzzy set rules, which are a result of data
analysis (accident reports, failure records for the similar pipelines) and experts
evaluation of related input parameters. Risk factors and their parameters are
classified in accordance with their relation to different environment and life-cycle
stages.
The article presents an example of evaluation of risk factor <development of
the external corrosion>. Consequently, the indicator of risk factor was calculated
in accordance with procedure of Mamdani fuzzy inference system (using the
MATLAB Fuzzy Logic Designer).
This method of risk assessment brings high opportunities for identification of
the pipeline segments, where the risk level is most probably high and helps the
decision maker to take remedial actions to mitigate potential risks. The operator
can more effectively manage resources and improve efficiency of actions. Ability to
reflect the current risk level is key advantage of our approach to risk assessment of
risk management of trunk oil pipelines.

One of the main tasks in risk management system of hazardous liquid
pipelines is risk assessment of failures and accidents. The main difficulty of risk
assessment is heterogeneity of the input information about current state of the
pipeline segments based on the results of its examination, information about
operation and maintenance, the influence of external factors. Furthermore, risk
assessment is complicated by the dynamic character of the risk factors which
determine the current level of risk along the trunk pipeline in general [1]. In such
circumstances, most of deterministic and statistical models for risk assessment
don’t reflect the current risk condition of hazardous pipeline.
The theory of fuzzy logic, applied for evaluation of risk factors will enable
us to evaluate the current risk level and help to make decisions in risk
management. Many risk factors cannot be properly analyzed using classical
deterministic and statistical models. This is mainly because of insufficient
knowledge, or imprecise data, or different approaches to data interpretation.
Another challenge is different origin of risk factors and, as a result, different
quality of incoming data. Through the use of linguistic variables in assessment of
uncertain data to describe risk factors we can convert all risk factors indicators to a
common scale. In that case we can summarize risk factors indicators and make risk
assessment in general, which can be interpreted as “acceptable” or “non-
acceptable”. This assessment allows us to compare risk level of different pipeline
segments and choose priority segments for preventive actions.
We introduce the following procedure, based on a system approach, and
aimed at identification of risk level assessment [2, 3]: 1 - analysis of hazards and
their sources; 2 - analysis of risk factors and possible scenarios of failure of
pipeline; 3 - evaluation of share of risk factors in the development of failures; 4 -
calculation of risk indicators for pipeline; 5 –choosing pipeline segments for
remedial actions, taking preventive and mitigation measures, if the risk level is not
acceptable.
For identification of risk factors and for handling them during risk
assessment of trunk oil pipeline let’s assume that the system and all its subsystems
can serve simultaneously as an object of risk and a source of risk; a trunk pipeline
as a complex technical system, from the very start of its operation is exposed to
influence of various factors, specifying its operation - constructional (cracks,
dents, steel degradation, isolation damages etc), technological ( incorrect operation,
incorrect repair, excavating damage etc.) , external (heavy rains, ground movement
etc.). Each risk factor brings different effect of the overall level of risk and every
risk factor will be determined by share of responsibility on the general level of risk.
The share of responsibility will be determined in accordance with results of

analysis of reports and statistical data about accidents, failures and damages with
similar pipelines.
The overall risk level of a trunk pipeline linear section is determined by the
combined effect of multiple risk factors. As we have defined, risk factors come
from3 environments (subsystems) of the pipeline (CE - <constructional
environment > TE - <technological environment>EE - <external environment>)
for the 3 stages of the life-cycle (development – construction - operation and
maintenance). In fact, from this point of view we assume that in one pipeline
segment certain risk factors can show up "stronger" or "weaker" than in another
adjacent pipeline segment.
We have made analysis of incident records and failure reports about similar
hazardous pipelines (located in Central Russia, in operation since 1980) and
determined the causes of failures in the linear parts of a trunk pipeline [4].The
causes of failures were classified according to their relation to environment and life
stages (Fig. 1, where Sr–share of responsibility of the risk factors group, there are
14 groups of the kind Sr = 1, 2, ..., 14) [4]. We identified risk factors that have
greatest consequences and put them in groups. Each group has own share of
responsibility which can be rated from 0 to 1, calculated from percentage
indicators.

We have grouped the main risk factors Yj (j = 1, 2, ..., m) and their
parameters Xi (i = 1, 2, ..., n) by environment and life-cycle stages. The share of
risk factors within the group Sr is determined on the basis of statistical data and
marked as Wrj.

Each risk factor can be defined as function Yj = f (Xi), and the overall risk
level of risk R is defined as the amount of risk factors, taking into account their
share of responsibility in groups of risk factors and the share within the group:

Risk factors can be as one-criterion (one input parameter Xi) or multicriteria
(several input parameters X1 + X2 + ... + Xi, where i – is the number of parameters
which determine the risk factor).
It is proposed to apply Mamdani fuzzy inference system for risk assessment
[5]. The main stages of this fuzzy inference method are as follows: 1) fuzzification
of input parameters; 2) formation of the rule base; 3) aggregation; 4) activation of
subfactors in fuzzy rules of productions 5) defuzzification.
Let’s consider the application of fuzzy inference on example of assessment
of one-criterion risk factor <development of the external corrosion>. This risk
factor comes from a risk factor group, related to <construction environment>,
subsystem stage –<operation and maintenance>.As was defined from accident
reports and failure records data, the share of responsibility of this risk factor among
group<external corrosion>is WZ = 0.3, the share of the entire group is S4 = 24%.
Assessment of risk factor <development of the external corrosion> depends on
input variables <technical condition of the insulation coating>. In this risk factor a
type and effectiveness of cathodic-protection systems was not taken into
consideration. Cathodic-protection was considered in another risk factor group.
Modern methods of pipeline condition insulation monitoring are based on
measurements of potentials, or in other words method of electrometric survey of
trunk pipelines linear parts. The results of insulation surveys are usually interpreted
from the point of view of quality characteristics. It is known that the resistivity of
the initial protective coating for the considered pipeline segment in trench is at
least 5×103
Om·m2
[6]. In this circumstance, the range of the set of input variables
will vary from 0 to 5×103

.
The technical condition of the insulation coating for the operated segment of
the pipeline can be defined as one of the linguistic variables Xi = 1,2, ..., 4: X1 = 1 -
<good>, X2 = 2 - <satisfactory> X3 = 3 - <poor>, X4 = 4 - <practically absent>.
We asked 3 groups of experts to indicate the boundaries of resistivity values
in the range of [0; 5×103

] to determine linguistic variables transitions X1,
X2, X3, X4 for each linguistic term. The results of the survey are given in Table. 1.

We defined the intervals in which the opinions of all expert groups have
matched. The indicators in them range [0; 0.9] for X4, [1.4; 1.9] for X3, [2.7; 4.2]
for X2 and [4.8; 5] for X1. Therefore, the remaining interval [0.9; 1,4] [1.9; 2.7]
[4.2; 4.8] has formed a "zone of vagueness " because in these intervals the expert
group doesn’t have full confidence in classification of linguistic variables [7].
Characteristic of the set of fuzzy numbers are the membership functions
(Membership Function). The membership function f (Xi) characterizes the degree
of membership in a linearly ordered set M (in this case the interval [0,1]). In those
intervals where experts reached full confidence in the assessment, evaluation of the
grade of membership Xi is equal to 1, in the "zone of vagueness" Xi assessment
varies between 0 and 1, and is equal to 0 in all other intervals. Thereby, these fuzzy
sets determine the degree of truth that the element belongs to the set.

Fig. 2 shows an example of input fuzzification of input variables for the pipe
sections A: XA = 4.67×103
Om·m2, for which the degree of membership
assessment <satisfactory> is estimated at 0.2, and at 0.81 it is evaluated as<good>.

Let us define a rule base for risk factor <development of external corrosion>
by building fuzzy subsets when each output linguistic variable according to each
rule will be assigned to a single fuzzy subset. The rule is written as a logical
condition <if option Xi = Ai, then the level of risk factor Yj = Ej> [7].

For this task the same groups of experts should set membership estimation
Xi = 1, 2, ..., 4 to fuzzy set Y. This will formulate a rule base of fuzzy inference.
Each expert group fills a table questionnaire on risk factors, which indicates their
opinion on Xi (i = 1, 2, ..., n) properties of fuzzy set Yj (j = 1, 2, ..., m). The
aggregated results of the survey of experts are shown in Fig. 3.
When the basis of the rules at the next stage of aggregation has been formed,
let us determine the extent of truth of each linguistic assessment for each rule of
fuzzy inference systems. This results in one fuzzy set, which will be assigned to
each output variable for each rule. For the fuzzy inference on each rule it is
common to use operations min (minimum) where membership is cut by (according
to) the height corresponding to the degree of truth of rule precondition.

As a consequence, in case variables belong to intersection of 2 fuzzy sets,
we produce activation (accumulation) [8], i.e., union of all max degrees of truth of
sub-conclusions to form one fuzzy subset (Fig. 4, the contour of the figure,
highlighted by purple line).

The final stage – process of defuzzification when the results of all output
variables in the previous stages of fuzzy inference are converted to regular
quantitative values of each of the output variables. Defuzzification of fuzzy set is
implemented by center of gravity method of the figure [9,10]:

Taking into account the share of risk factor in the group Wrj = 0.3, and the
share of the entire group Sr = 24%, the value of risk factor <development of
external corrosion> equals

Obtaining the same risk factor values for adjacent pipeline segments will
allow us to compare these values and make decision about insulation repair
priorities, for example.
The main objective of risk assessment, based on fuzzy sets, is in
determination of fuzzy set rules for different pipelines according to their age. It is
vital to validate and update the set of rules [12,13]. After finalizing of
determination of fuzzy set rules for all other significant risk factors, we will be able
to evaluate the general level of risk for each section of the pipeline. Thereby,
comparison of risk level for a number of pipeline segments (R1, R2, ..., Rn) will
identify a set of priority measures to reduce risk factors in those segments which
receive risk level as “high” or “unacceptable”.
The advantage of this approach is in the possibility to combine existing
methods or models of evaluation of different risk factors (corrosion, cracks, stress
accumulation, ground movement, defective welding etc) as a comprehensive
solution for risk management of hazardous pipelines.
