Running head: CASE STUDY 2 1 3 CASE STUDY 2 Congestive Heart Failure Case Study 2 This case study is about Mr. P, a 76 year old male, hospitalized usually to treat cardiomyopathy and congestive heart failure (CHF). The author describes the approach to care, treatment plan, method to provide education and a teaching plan about the CHF. Congestive heart failure is defined as “the state in which the heart is unable to pump blood at a rate adequate for satisfying the requirements of the tissues with function parameters remaining within normal limits usually accompanied by effort intolerance, fluid retention, and reduced longevity” (Denolin, 1983, p. 445). Approach to care · Assessment of patient: Physical examination of the patient (at each visit), with particular attention to assessment of their vital signs, cardiovascular system (including volume status), signs of deterioration and co-morbid conditions. · Assessment and management of the patient’s cardiovascular risk factors (e.g. hypertension, dyslipidaemia, diabetes, smoking and obesity). · Assessment of the patient’s nutritional status. · Assessment of the patient’s potential for adverse effects of medicines · Regular reassessment of the patient’s biochemistry (including urea and creatinine) and haematology (including haemoglobin) parameters. · Assessment of the patient’s daily fluid and salt intake from food and drink. · Ongoing monitoring of the patient’s electrolytes (particularly serum sodium and potassium levels) and renal function. · Monitor intake and output strictly and take daily weight. Treatment Plan CHF is a complex clinical syndrome; therefore, treatment plan includes the focus on the fundamental causes. Appropriate combination of medicines, control over lifestyle with careful monitoring is the basic treatment for CHF. · An Angiotensin-Converting Enzyme (ACE) inhibitor (captopril, enalapril etc) or an Angiotensin II receptor blocker (ARB) helps to relax the blood vessels and enhance the blood flow. · Beta-blockers (bisoprolol, carvedilol etc) reduce the blood pressure and stabilize the heart beat rate. · Diuretics (lasix, bumex etc) remove more sodium and water from the body, which helps in lowering blood pressure. Also, spironolactone and eplerenone called as potassium-sparing diuretics help to retain potassium. · Other drugs include anticoagulant, statin and digoxin as per doctor’s recommendation. Besides medication and diet alteration, devices and surgical procedures are available for the treatment of heart failure. An implantable cardiac defibrillator (ICD), coronary artery repair and valve repair or replacement (as appropriate) can be done (Heart.org, 2016). Abstract Objective: Finding an effective therapy against Alzheimer’s disease (AD) has been associate increasingly pressing issue and traditional Chinese medicine (TCM) is widespread in the prevention and treatment of AD in China. The aim of this study was to judge the efficaciousness and safety of TCM kidney-nourishing (the changes of pathological state of kidney deficiency by means of TCM treatment and so on) formula (TKNF) for AD in comparison with donepezil. Methods: The retrieval period of seven databases was from the establishment of each database to April 2019. Two authors independently identified randomized controlled trials (RCTs), fetched data and assessed bias risk. Comprehensive analysis process was conducted with review manager for eligible and appropriate RCTs. Results: A complete of 981 AD patients from 13 studies were enclosed. Meta-analysis of RCTs showed that there was no significant difference in the improvement of Alzheimer's disease assessment scale-cognitive subscale score between 2 groups in short term, but the effect of long-term treatment may exceed donepezil; there was a significant difference in the improvement of activities of daily living score between 2 groups; there was a significant difference in TCM curative efficacy between 2 groups with long-term treatment. There was no significant difference in the incidence of adverse events between 2 groups. The quality of the evidence was high or moderate. Conclusion: Compared with donepezil, TKNF was an effective drug for AD patients and the clinical application of TKNF was safe. TKNF's long-term benefits need more evidence to verify.

Also, are these decisions influenced by whether the subject lives in a rural or urban setting. The research proposal was the cultural issues effected participants decision making as well as place of residence. This research used a previous qualitative research design in which 42 cardiac patients (21 urban, and 21 rural) were interviewed about their diet and physical activity. The researchers then designed a model for interviewing regarding the decision-making process. The combination model was then given and tested with 647 cardiac patients (327 urban and 320 rural) from Canada. The results were based on 93.5% accuracy for diet and 97.5 % accuracy with physical activity. Results indicated that decision-making was less about place of residence and more about perception of control over health including time, effort, or competing priorities, receipt of appropriate clear information, and appeal of the activity. A three-staged, multi-methods approach was used to develop and analyze the descriptive decision making model that patients use in making decisions regarding their cardiac lifestyle. A cross-sectional survey was used to interview patients one year post-cardiac catherization. These interviews were performed via telephone. A three stage decision tree model was then used to analyze the information offered. The stages were as follows: 1. Factors that were influential in decision making. 2. If and where failure had occurred for patients. 3. Did patients consistently, sometimes, or not at all engage in physical activity and a heart healthy diet. Results were then analyzed using statistical analysis. Information was gathered from a previous series of qualitative interviews conducted with 42 cardiac patients (21 rural, 21 urban). Heart disease continues to be a primary cause of death in most of the developing countries worldwide, followed by cancer, osteoporosis, arthritis and many others. Consumers being frustrated with the expensive, high-tech, disease-treatment approach in the modern medicines are seeking complementary or alternative beneficial products in the form of nutraceuticals. Nutraceuticals are the emerging class of natural products that makes the line between food and drugs to fade .Nutraceutical is the hybrid of ‘nutrition’ and ‘pharmaceutical’. Nutraceuticals, in broad, are food or part of food playing a significant role in modifying and maintaining normal physiological function that maintains healthy human beings. The principal reasons for the growth of the nutraceutical market worldwide are the current population and the health trends. The food products used as nutraceuticals can be categorized as dietary fibre, prebiotics, probiotics, polyunsaturated fatty acids, antioxidants and other different types of herbal foods. The nutraceuticals facilitate in combating the key health complications of the century such as obesity, cardiovascular diseases, cancer, osteoporosis, arthritis, diabetes, cholesterol etc. In whole, ‘nutraceutical’ has led to the new era of medicine and health, in which the food industry has become a research oriented sector.
