Recognizing the impact security events can have, Fastly developed its Incident Command protocol, which it uses to deal with large-scale events. Maarten Van Horenbeeck, a lead on Fastly’s security team, and experienced incident commanders Lisa Phillips and Tom Daly explore how Incident Command was conceived and the protocols that were developed within Fastly to make it work. The three share a number of war stories that illustrate how Incident Command contributes to protecting Fastly, its customers, and the many end users relying on the service. Examples include a major software vulnerability that affected a Linux component in common use across Fastly and a large attack. The NSW Resources Regulator is committed to supporting the health and safety of those working in the mining industry through the development and distribution of relevant and flexible learning and development programs.
As part of our commitment, we have collaborated to develop three flexible learning programs that specifically incorporate the findings from investigations and independent reviews of mining incidents that have involved multiple fatalities or where the circumstances that occurred presented a significant risk of death to workers or the wider community.
The case studies selected have been deliberately designed to represent all mining contexts. This is to reinforce to the industry that the risk of fatalities occurs in all parts of the mining industry, which requires everyone to remain vigilant and proactive. For this reason, learning from disasters has been directly integrated into the requirements to maintain competence, where relevant, as part of the maintenance of competence scheme for practising certificates.
Learning from experience, preventing devastating reoccurrences and improving the health and safety of all working in this industry is a profound way of acknowledging and recognising all those who have been affected by mining safety incidents throughout history.
Aviation Training, Safety Management System
Depending on the nature of the task, the level of safety management training required will vary from general safety familiarization to expert level for safety specialists, for example:
a) Corporate safety training for all staff,
b) Training aimed at management’s safety responsibilities,
c) Training for operational personnel (such as pilots, maintenance engineers, dispatchers / FOO’s and personnel with apron or ramp duties), and
d) Training for aviation safety specialists (such as the Safety Management System and Flight Data Analysts).
The scope of SMS training must be appropriate to each individual’s roles and responsibilities within the operation. Training should follow a building-block approach. As part of the ICAO requirements, an operator must provide training to its operational personnel (including cabin crew), managers and supervisors, senior managers, and the accountable executive for the SMS.
Training should address the specific role that cabin crew members play in the operation. This includes, but is not limited to training with regards to:
a) Unit 1 SMS fundamentals and overview of the operator’s SMS;
b) Unit 2 Safety policy;
c) Unit 3 Hazard identification and reporting; and
d) Unit 4 Safety Communication.
e) Unit 5 Review of Company Safety Management
f) Unit 6 Review of Safety Reporting
The base content comes from many sources but all aligned to the ICAO syllabus requirements, and created for an international operational airline.

Nick Frost is Co-founder and Lead Consultant at CRMG.
Nick’s career in cyber security spanning nearly 20 years. Most recently Nick has held leadership roles at PwC as Group Head of Information Risk and at the Information Security Forum (ISF) as Principal Consultant.
In particular Nick was Group Head of Information Risk for PwC designing and implementing best practice solutions that made good business sense, that prioritise key risks to the organisation and helped minimise disruption to ongoing operations. Whilst at the ISF Nick led their information risk projects and delivered many of the consultancy engagements to help organisations implement leading thinking in information risk management.
Nicks combined experience as a cyber risk researcher and practitioner designing and implementing risk based solutions places him as a leading cyber risk expert. Prior to cyber security and after graduating from UCNW and Oxford Brookes Nick was a geophysicst in the Oil and Gas Industry.
Simon Lacey
Simon is a resourceful, creative Information & Cyber Security professional with a proven track record of instigating change, disrupting the status quo, influencing stakeholders and developing ‘big picture’ vision across business populations. Multiple industry experience; excels in building stakeholder engagement & consensus; and suporting organisations to make sustainable change.
Simon also has considerable experience of risk management, education and awareness, strategy development and consulting to senior management and is a confident and engaging public speaker.
Simon has previously worked within the NHS, Bank of England and BUPA, before setting out as an independent consultan forming Oliver Lacey Limited, supporting clients in multiple business sectors.
When not working, Simon loves to run – currently training for the Berlin Marathon, a Director of Aylesbury United Football Club, records vlogs and is an experienced standup comic.

Dear Dr. Kornbluth and Mr. Gorenberg,
The US House of Representatives is deeply concerned by ongoing and pervasive acts of antisemitic
harassment and intimidation at the Massachusetts Institute of Technology (MIT). Failing to act decisively to ensure a safe learning environment for all students would be a grave dereliction of your responsibilities as President of MIT and Chair of the MIT Corporation.
This Congress will not stand idly by and allow an environment hostile to Jewish students to persist. The House believes that your institution is in violation of Title VI of the Civil Rights Act, and the inability or
unwillingness to rectify this violation through action requires accountability.
Postsecondary education is a unique opportunity for students to learn and have their ideas and beliefs challenged. However, universities receiving hundreds of millions of federal funds annually have denied
students that opportunity and have been hijacked to become venues for the promotion of terrorism, antisemitic harassment and intimidation, unlawful encampments, and in some cases, assaults and riots.
The House of Representatives will not countenance the use of federal funds to indoctrinate students into hateful, antisemitic, anti-American supporters of terrorism. Investigations into campus antisemitism by the Committee on Education and the Workforce and the Committee on Ways and Means have been expanded into a Congress-wide probe across all relevant jurisdictions to address this national crisis. The undersigned Committees will conduct oversight into the use of federal funds at MIT and its learning environment under authorities granted to each Committee.
• The Committee on Education and the Workforce has been investigating your institution since December 7, 2023. The Committee has broad jurisdiction over postsecondary education, including its compliance with Title VI of the Civil Rights Act, campus safety concerns over disruptions to the learning environment, and the awarding of federal student aid under the Higher Education Act.
• The Committee on Oversight and Accountability is investigating the sources of funding and other support flowing to groups espousing pro-Hamas propaganda and engaged in antisemitic harassment and intimidation of students. The Committee on Oversight and Accountability is the principal oversight committee of the US House of Representatives and has broad authority to investigate “any matter” at “any time” under House Rule X.
• The Committee on Ways and Means has been investigating several universities since November 15, 2023, when the Committee held a hearing entitled From Ivory Towers to Dark Corners: Investigating the Nexus Between Antisemitism, Tax-Exempt Universities, and Terror Financing. The Committee followed the hearing with letters to those institutions on January 10, 202
